Where medical information is incomplete when a person with dementia is transferred to a new home , a carer, family member or supporter should see it as a new start and get everything updated. Positive from negative #diversealz 2/2

@DiverseAlz A1. My preference would be to make sure that my loved one would see same team of physicians if hospitalized that s/he sees in office. Continuity of care can be valuable & facilitate decision-making. #diversealz

Several views for question 3 so will be good to hear your thoughts about preparedness and asking questions. Time is money so that can be a factor for some, time limits on consultation and, importantly, to get right answers at times, need to ask right questions #diversealz

Many people on the timeline could say, and rightly so, that they know more about the particular type of dementia and the person living with it, than the healthcare professional (hcp) they come into contact with. Ideally, one hopes hcp would take up joint partnership. #diversealz

RT @LynnGamble20 I'm surprised that my husband gets nothing other than an annual medication review which basically is the GM asking if he's eating and saying carry on as things are. No blood pressure, blood tests, physical check over. Things could be being missed. #diversealz

As mentioned before, think it helps where health care professionals have experience of dementia and especially by experience. A dental hygienist was brilliant and why? Grandmother had dementia #diversealz
