@JHospMedicine A1: I learned that I have a lot more I could be doing from the article, but I try to sit down, make eye contact, repeat back what they say to me, and ask permission to ask them why they are making a decision. The whole "don't think about your response" is hard. #JHMChat

@JHospMedicine For patients who have been hospitalized a long time or multiple times, trying to know as much of their past history as possible. I learned this the hard way during a care conf when a patient’s family mentioned a significant event in the past that I didn’t know about. #JHMChat

We have some great local projects on food insecurity that I try to learn about and utilize. It's progress. #JHMChat

@JHospMedicine A4: It's hard, but ultimately it's their life and their values. I still try to show support and tell them I'm still going to care for them. Or even if a patient leaves earlier than planned, they are always welcome back to receive care. Keeping the connection is #1. #JHMChat
