@JHospMedicine #JHMchat great question seems like a BIG one that comes up for us is "bedtime" and "wake up" time. esp for younger patients with shifted circadian rhythm... can a patient post a "do not awaken until 10am" sign?? what does that do for flow of day, labs, RN med pass, prerounds?
Vinny Arora MD MAPP @FutureDocs
T1 I would say many patients we talk to about sleep here don’t even realize they can advocate for better sleep by asking work to be batches or moving 4a labs or vitals to 6a for uninterrupted sleep #JHMChat

@RebeccaEBerger Very few meds 4a - so if you actually need vitals and labs 4a those are key to batch. Time the vitals with when phlebotomy goes in. Often the funny thing is 4a labs are started at 4a but the sweep is 4a to 6a bc so many patients. So key is do you need vitals at 4a ? #jhmchat

@subhaairan @RJmdphilly @Potato_Chip #JHMchat the trapped in a room experience was far from enjoyable but also a really valuable learning experience...

Subha Airan-Javia (she/her) @subhaairan
@signaturedoc @wargote4 @FutureDocs @RJmdphilly @Potato_Chip @aoglasser fair point. but the key to design is making it appropriate & applicable. #hospitalnightmode only works for non-critical care situations where people truly can go ~6hrs without active monitoring. even in ICUs tho, we can do a better job of batching interactions #JHMchat
Avital Y. O'Glasser, MD FACP SFHM (she/her) @aoglasser
@RJmdphilly @subhaairan @alfandremd #JHMchat great analogy!
Rebecca Jaffe, MD @RJmdphilly
@subhaairan @signaturedoc @wargote4 @FutureDocs @Potato_Chip @aoglasser Yes! Batching is key. You might need VS, and Neuro check, and labs over night, but let’s at least do them all at the same time! And if you need them once in 8 hours, there must be some sleep Med science on when in cycle sleep interruptions are best tolerated. #jhmchat
Brian Locke @doc_BLocke
@FutureDocs Ahh it's 2 simultaneous twitter-chats converging in my feed... #ATSchat (sleep medicine opportunities) #JHMChat (patient experiences of hospitalization) (in seriousness, this is such as needed area of attention! πŸ‘) https://t.co/NJetIm19yf
Leilani Graham @LeilaniRGraham
@SocietyHospMed @JHospMedicine So, so many things, but predominantly that just because I’m so lucky to be educated, articulate, & privileged to receive top care: that doesn’t mean I’m not scared as shit. Speak to me as an equal, but comfort me w/wisdom I don’t have. Then do it for every patient. #JHMChat
Subha Airan-Javia (she/her) @subhaairan
@jenreadlynn @aoglasser @Potato_Chip @RJmdphilly @JHospMedicine @SocietyHospMed @AJenkins_MD and i'm sure often these hospitals are not necessarily in the same "health system" so communicating as care providers can be challenging #JHMchat
Rebecca Jaffe, MD @RJmdphilly
@subhaairan @signaturedoc @wargote4 @FutureDocs @Potato_Chip @aoglasser Also we all know there are plenty of people who are in the hospital who can’t leave but don’t need VS or other intervention overnight. Let’s not let our decisions/design be guided by our demons (ie capitulating to perverse incentives) #jhmchat

My siblings & I had our parents in the hospital at the same time. Our Dad was transported to Ottawa for CABG surgery while our Mom was having hip surgery & was in excruciating pain. As they said goodbye, they didn’t know if they would see each other again. Actions & words matter.
Brian @BMont81
We just honestly want to be heard. I was on a vent for 4 weeks, 3 of them wide awake. An understanding on what that does to the patient mentally, how draining those situations are, can lead to compassion during communication on next steps for care, even if uneasy news #JHMChat

Bingle @TheBingle
RT @EMoriartyWade: I think it’s important to understand how tough the logistics can be for families. We were told we’d need 3 nights in the hospital each month for the foreseeable future for an IV treatment. It was an absolutely daunting prospect for us with two careers and two other kids. #JHMChat
Mark Shapiro, MD @ETSshow
@aoglasser @FutureDocs @subhaairan @RebeccaEBerger I’m so bummed to have missed #JHMChat tonight. Will catch up on the discussion
Jon McBride @jamcbride
RT @EMoriartyWade: Consider the possibility that maybe your patient might not need to be hospitalized. We learned that we could avoid some hospital stays through a combination of outpatient care and home health nurses who taught me to administer an IV. #JHMChat
Mark Shapiro, MD @ETSshow
This #MedThread from @brookevitti is heartfelt & impactful. Highly Recommended #JHMChat

RT @esteckler2: Topic #1 Both my kids have diabetes due to pancreatitis. We struggle with hospitalizations due to protocol has to adhere to what hospital does to address diabetes instead of what patient usually does. Also kids can’t check own bs need to wait even though episodes drop #JHMchat
Subha Airan-Javia (she/her) @subhaairan
RT @WrayCharles: Great insight here. Our nursing colleagues are often the ones driving patient experiences. Instead of thinking of this as a two-way conversation (patient-doctor), it should probably be a 3-way conversation (patient-RN-doctor) #JHMChat
