A1. Having a continuing care plan is one step. Creating an easy way to share records between practices is helpful. Knowing if there is a local care navigation program to refer to or creating one is even more helpful. #KareoChat

I think there has to a "patient champion" in each practice. It may be the HCP or it may be a nurse, MA, or other team member. As long as one person knows it is their responsibility to coordinator care, it should work. #Kareochat https://t.co/cNRzQjEqoD

@TheSocialNurse Typically each patient will be assigned to one care navigator, who works within a single county. A county may have several navigators that share local resource information with each other, but not patient information. To get info from...#KareoChat

@TheSocialNurse The patient’s providers or their school system we just have a lot of paperwork signed that allows us to communicate on different levels. All the way from being able to call and ask basic questions to being able to get copies of records or even attend appointments. #KareoChat

@TheSocialNurse @GetSocialHealth @KansasAFP Our area has been lacking this type of coordination. I was part of the pilot program, so it’s still a small group. All of the navigators are independent of a practice. We are trained through another nonprofit that was established to remedy the lack of coordination. #KareoChat
