Janet Kennedy @GetSocialHealth I like this topic for the #KareoChat today. Helping patients navigate care outside the practice. How many people only have one HCP anyway?
Annie Beth Donahue @anniebdonahue @GetSocialHealth I can’t think of anyone! #KareoChat
Annie Beth Donahue @anniebdonahue Hate that I only get 30 minutes to spend in the #KareoChat today. I do Pediatruc Care Navigation with our nonprofit.
Janet Kennedy @GetSocialHealth I'm a Healthcare & Social Media #podcaster and consultant. Also Partner with @TheSocialNurse in the @HMNwriters - We're always looking for new writers! #KareoChat
Melissa Mills @melissamwriting #KareoChat Hello! I am a nurse of 20 years and a Healthcare content writer. Glad to be here. Oh, and a professor. :)
Tebra @TebraOfficial We'll get started with our first topic in just a minute! #KareoChat
Tebra @TebraOfficial Q1. What should practices be equipping patients and families with in order to link them to with needed follow-up services & information? #KareoChat https://t.co/gliQoc4uug
Janet Kennedy @GetSocialHealth Is this a #PatientPortal issue/question? Or more complicated than that? #KareoChat https://t.co/IYbukYdsEt
Dr. Tom Giannulli @drtomg_HIT morning k chatters #kareochat
Janet Kennedy @GetSocialHealth Good Morning @drtom_kareo Happy to see you in the #KareoChat today.
Melissa Mills @melissamwriting This is a GREAT topic and question! Tangible knowledge. As a nurse, we often don't have time to fully educate patients with what they need for self-care. We have to create an individualized care plan that meets the patient and family where they are #kareochat https://t.co/nd6P9HR1fx
Jeff Quinn @JQuinn_Buffalo What about looking in to different ways to communicate with patients. I understand there are certain regulations, but I think text messaging is allowed through HIPAA now #KareoChat https://t.co/WhMCiaIgEm
Tebra @TebraOfficial RT @JQuinn_Buffalo: What about looking in to different ways to communicate with patients. I understand there are certain regulations, but I think text messaging is allowed through HIPAA now #KareoChat https://t.co/WhMCiaIgEm

Annie Beth Donahue @anniebdonahue A1. Having a continuing care plan is one step. Creating an easy way to share records between practices is helpful. Knowing if there is a local care navigation program to refer to or creating one is even more helpful. #KareoChat
Melissa Mills @melissamwriting Yes, each action plan we make for a patient must be individualized. Healthcare is not a "one-size-fits-all" product. #Kareochat https://t.co/aSsPZZxRsE
Melissa VanHouten @melissarvh A1 Recognize patients often don't know all the issues/difficulties they'll encounter. Need info re what 2 do when new ?s arise #kareochat
Janet Kennedy @GetSocialHealth Who should "Equip patients & families with a plan of action, resources and referrals." The HCP or a separate person at the practice? #kareochat
Tebra @TebraOfficial RT @anniebdonahue: A1. Having a continuing care plan is one step. Creating an easy way to share records between practices is helpful. Knowing if there is a local care navigation program to refer to or creating one is even more helpful. #KareoChat
Janet Kennedy @GetSocialHealth Is this something only a big practice can afford to do? #KareoChat https://t.co/zPiSkQp5Y9
Jeff Quinn @JQuinn_Buffalo @GetSocialHealth If the practice is large enough to have a dedicated person that would make the customer experience even better #KareoChat
Melissa Mills @melissamwriting I think there has to a "patient champion" in each practice. It may be the HCP or it may be a nurse, MA, or other team member. As long as one person knows it is their responsibility to coordinator care, it should work. #Kareochat https://t.co/cNRzQjEqoD
Tebra @TebraOfficial RT @melissarvh: A1 Recognize patients often don't know all the issues/difficulties they'll encounter. Need info re what 2 do when new ?s arise #kareochat
Janet Kennedy @GetSocialHealth Would a digital library work? Use the patient portal of a service like @GetBeeWell? #KareoChat
Annie Beth Donahue @anniebdonahue @TheSocialNurse Typically each patient will be assigned to one care navigator, who works within a single county. A county may have several navigators that share local resource information with each other, but not patient information. To get info from...#KareoChat
Jeff Quinn @JQuinn_Buffalo @mkngspacenurse Holding someone accountable always helps to get things done! #KareoChat
Tebra @TebraOfficial We like the idea of a "Patient champion." #KareoChat https://t.co/ARJ5K2H859
Annie Beth Donahue @anniebdonahue @TheSocialNurse The patient’s providers or their school system we just have a lot of paperwork signed that allows us to communicate on different levels. All the way from being able to call and ask basic questions to being able to get copies of records or even attend appointments. #KareoChat
Pharmacy Podcast Network @PharmacyPodcast Yaas Jeff!! We recently did a podcast on Text Communications w/ patients, we must meet patients where they are!! #KareoChat https://t.co/qYiwxLF3g6 @zipwhip https://t.co/D38dAr9l0Y
Jeff Quinn @JQuinn_Buffalo @PharmacyPodcast @zipwhip Looks like I have my next podcast to listen to. Thanks! #KareoChat
Melissa VanHouten @melissarvh @TheSocialNurse I am actually a patient, but I wanted to convey to those practicing how lost we sometimes feel -- not even certain what questions to ask while we are at an appt. Helps to have a sense of where to direct ?s & who to turn to. #KareoChat
Tebra @TebraOfficial What about that #PatientPortal? https://t.co/8W6smpkxWS can we use our patient portals to inform & educate about the plan of care? #KareoChat https://t.co/b4lfdJAoR4
Annie Beth Donahue @anniebdonahue @TheSocialNurse The Navigators meet each other at local training sessions where we can get certifications from another program that focuses on best practices and does trainings. #Kareochat
Melissa Mills @melissamwriting Q2: We have to make sure the portal has actionable information that the patient understands. #kareochat
Shereese Maynard, MS; MBA 😷 She/Her @ShereeseMayMba Hi guys. I'm currently being interviewed for a podcast. I'll join shortly #kareochat https://t.co/Y3KkkumL7c
Annie Beth Donahue @anniebdonahue @TheSocialNurse @GetSocialHealth @KansasAFP Our area has been lacking this type of coordination. I was part of the pilot program, so it’s still a small group. All of the navigators are independent of a practice. We are trained through another nonprofit that was established to remedy the lack of coordination. #KareoChat
Pharmacy Podcast Network @PharmacyPodcast Technologists get too excited to release the next “app” or “portal” for patient engagement & communications. We have to think “SIMPLIFICATION” & use what’s already existing. Make it as easy as possible. (For providers & patients) #KareoChat https://t.co/Paa8SdCsQQ
Tebra @TebraOfficial @ShereesePubHlth See you soon in the #KareoChat!

Melissa VanHouten @melissarvh A2 Portals underutilized. Easy & convenient 4 me as pt 2 access. Could announce support mtgs, educational papers/videos, etc. #kareochat
Janet Kennedy @GetSocialHealth Say lots of smart things - as I know you will! #KareoChat https://t.co/3dicb5rrVy
Annie Beth Donahue @anniebdonahue My 30 minutes are up! Have fun, everyone! #KareoChat
Jeff Quinn @JQuinn_Buffalo @PharmacyPodcast Having a patient champion to guide them through the portal a few times when they first join the practice I think can help to increase engagement. Also making it seem simpler for the patient. #KareoChat
Tebra @TebraOfficial @anniebdonahue Thanks for joining us Annie. Excellent information and I'm sure we'd love to hear more! #KareoChat
Tebra @TebraOfficial Q3. How can clinical teams use health IT to help patients be prepared, understand more about their medications, keep track of when and how to take them, access resources or specialists who might help them? #KareoChat https://t.co/1hOW3u2uq8
Melissa Mills @melissamwriting This is a good idea. Maybe even a "virtual notepad" to type of questions. #Kareochat https://t.co/FJy0wMUrSM
Shereese Maynard, MS; MBA 😷 She/Her @ShereeseMayMba A2 Patient portals have to consider the patient POV 1st in the design https://t.co/dfFQegJyR3 shouldn't be an after-thought or a "fix" #KareoChat
Dr. Tom Giannulli @drtomg_HIT IMO the patient portal is being replaced by care specific apps, one for each problem DM, depression etc #kareochat
Melissa VanHouten @melissarvh @TheSocialNurse Absolutely! Tell us what you want us 2 think about (in terms of illness/care) ahead of time & what specific info to bring - & then have someone actually read that & prepare for our specific set of issues AHEAD of our visit if possible. #KareoChat
Melissa Mills @melissamwriting First, we have to make sure the healthcare team knows how to use the portal and EMR to its fullest capability. I often see clinicians who don't know how to tech to its fullest! This could hurt patients. #Kareochat https://t.co/tL7N1Vdoj0
Dr. Tom Giannulli @drtomg_HIT By using point apps that are problem specific we also loose coordination unless centrally administrated #kareochat
Dr. Tom Giannulli @drtomg_HIT Today care managers need ot engage with patients using problem point apps as part of their toolkit, but EHR cant talk to all #kareochat
Janet Kennedy @GetSocialHealth @drtom_kareo Are these apps that the HCP has access to or are they for self-navigation/care? Does that make it hard to integrate the care? #KareoChat https://t.co/5q9wQcD0cn
Melissa VanHouten @melissarvh A2 Would be fabulous if all involved in my care used same portal 2 message me & each other, coordinate care, ask ?s, gather info #kareochat
Tebra @TebraOfficial RT @ShereesePubHlth: A2 Patient portals have to consider the patient POV 1st in the design https://t.co/dfFQegJyR3 shouldn't be an after-thought or a "fix" #KareoChat

Dr. Tom Giannulli @drtomg_HIT As remote care becomes more complex and powerful the more a navigator or care manager is needed #kareochat

Memorizing Pharmacology @Tony_PharmD A3. As a #pharmacist I see technology magnify both good practices and bad. I think it comes down to sitting down with the patient to ask what tech they prefer and taking the time to show them how to use it and empower them #kareochat https://t.co/IAgJE4q3Eu

Melissa VanHouten @melissarvh Q4 Can't adequately stress how valuable this would be. Could help w/referrals, care & support resources, insurance issues, med ?s #kareochat
Dr. Tom Giannulli @drtomg_HIT In many small practices a care manager / care navigator presents a staffing challenge unless reimbursement model is aligned #kareochat
Janet Kennedy @GetSocialHealth How many patients would need to see/use a patient navigator or care coordinator? 10%? 50%? #KareoChat

Health IT Advice @HealthITAdvice @GoKareo A1: -Practices should have a patient portal. -Equip the patients/families with the knowledge to access and utilize the portal. It doesn't do the provider or the patient any good if no one knows how to use it. #KareoChat https://t.co/l2urgGIRwS

Melissa Mills @melissamwriting I think it depends on the population of patient and the navigator program. Many are specific to certain populations. #Kareochat https://t.co/NQaiUZ1t5e
Melissa VanHouten @melissarvh @HealthITAdvice @GoKareo Had my account set up & rec'd instructions re access while still in hospital just after diagnosis. It is still today my favorite means of communication w/doc. #KareoChat
