Thanks for joining the #KareoChat today! We're thrilled to have @Capture_Billing's Manny Oliverez as our Host today.
Tebra @TebraOfficial
Introduce yourselves and say hello to the other members of the #KareoChat and we'll post the 1st question in a minute.
Janet Kennedy @GetSocialHealth
RT @GoKareo: Thanks for joining the #KareoChat today! We're thrilled to have @Capture_Billing's Manny Oliverez as our Host today.
Capture Billing @Capture_Billing
I'll start! My name is Manny Oliverez and I am the CEO and co-founder of Capture Billing #KareoChat
Tebra @TebraOfficial
It's a #MedicalBilling and #Claims topic on the #KareoChat today! Thanks for being here @Capture_Billing! https://t.co/tsfjVui9hT
Tebra @TebraOfficial
Tome for the first question! What Is the Difference Between a Rejected Claim and a Denied Claim? #KareoChat #MedicalBilling https://t.co/OIhHUnNoYH
Capture Billing @Capture_Billing
Medical billing is a complex process and our chat today will help you focus on claims rejections #KareoChat
Capture Billing @Capture_Billing
A rejected claim is a claim that never actually makes it to the insurance company and is never processed #KareoChat
Capture Billing @Capture_Billing
A denied claim is one that is received and processed by insurance but does not actually get paid #KareoChat
Capture Billing @Capture_Billing
Hello Carol @TheSocialNurse Glad to see you joined us. #KareoChat
Capture Billing @Capture_Billing
When we initially submit a claim via our clearinghouse it goes through a "scrubbing" process and claims that may be missing information get rejected #KareoChat
Annie Beth Donahue @anniebdonahue
@Capture_Billing Thanks for the clarification! #kareochat
Janet Kennedy @GetSocialHealth
Hi, Janet Kennedy of Get Social Health here. Today I represent the patient perspective. #KareoChat

@GetSocialHealth I'll be following your comments closely, then! #kareochat
Capture Billing @Capture_Billing
When submitting claims our main goal is to submit a "clean" claim, one that is error free #KareoChat
Janet Kennedy @GetSocialHealth
Is that the first place that rejection problems happen? Practice team members don't fill the paperwork out or is this a technology issue? #KareoChat https://t.co/ny73Comawc

Capture Billing @Capture_Billing
Hi Annie Beth @anniebdonahue thanks for joining us! #KareoChat
Capture Billing @Capture_Billing
Sometimes the practice management software will catch simple mistakes that can be correctly immediately, before sending it to the clearinghouse. Kareo does this #KareoChat
Annie Beth Donahue @anniebdonahue
@GetSocialHealth Great questions! And is there anything the patient can do to help ensure the paperwork is filled out correctly? Is there a place where the information could be incorrectly provided by a patient? #kareochat
Janet Kennedy @GetSocialHealth
No! Please don't make me involved in claims in any way. #LazyPatient #KareoChat https://t.co/19sMfdFmAX
Capture Billing @Capture_Billing
Good practice management software will check eligibility and help us catch data entry errors like a transposed number in a patient's id #KareoChat
Tebra @TebraOfficial
Q2) What Causes a Claim to be Rejected? #KareoChat https://t.co/hiKo0txGVl
Capture Billing @Capture_Billing
Hello @HMNwriters #KareoChat
Capture Billing @Capture_Billing
Patients can help us by making sure we have complete information by providing us with the most current insurance card and demographics #KareoChat
Memorizing Pharmacology @Tony_PharmD
It really depends, but often it's that the book of medications is so onerous, that there's no possibility of reading each detail before submitting #kareochat
Capture Billing @Capture_Billing
Claims can be rejected when the patient does not update their demographic information or provides the wrong information #KareoChat
Janet Kennedy @GetSocialHealth
You know that alternate at-home test for colon cancer screening? My physician told me to check with my insurance company. I didn't. Claim was rejected. I think the physician needs the tools to check to see if I'm covered before they recommend something. #KareoChat #LazyPatient
Capture Billing @Capture_Billing
Wrong information can include old insurance information, incorrect date of birth, or if a form is not completed properly #KareoChat
Healthcare Writers @HMNwriters
RT @Capture_Billing: Claims can be rejected when the patient does not update their demographic information or provides the wrong information #KareoChat
Capture Billing @Capture_Billing
Sometimes we make data entry errors, like a transposed ID number which can easily cause a claim to reject. #KareoChat

Annie Beth Donahue @anniebdonahue
@GetSocialHealth This would be amazing. I'm sure someone could come up with a database that tells you what each type of insurance covers. Providers would be able to search any insurance they accept to rule out automatic "nos." #kareochat
Capture Billing @Capture_Billing
@GetSocialHealth Janet, unfortunately doctors can't know the ins and outs of every patient's specific policy, but they should do their best to help patient's understand their benefits #KareoChat
Annie Beth Donahue @anniebdonahue
@GetSocialHealth It should also include things that are covered only with a letter of medical necessity so the provider knows they can just write something up for the patient. #kareochat
Janet Kennedy @GetSocialHealth
Part II of my story. I ignored the bill for the alternate #ColonCancer screening test and eventually it must have been deemed acceptable to the insurance company. #KareoChat (Not a good idea, generally.)
Capture Billing @Capture_Billing
@GetSocialHealth Janet, did the claim deny or did it reject? #KareoChat
Janet Kennedy @GetSocialHealth
I don't know. The rationale was that the test was too new. It must have gained wider acceptance over the next 6 months. #KareoChat https://t.co/hmMxH5DgNu
Capture Billing @Capture_Billing
There are tools in the EHRs to help check formularies to see if certain medications are covered by the patient's individual plan #KareoChat
Dr. Tom Giannulli @drtomg_HIT
denial may be due to prior auth reguirements as well #kareochat
Memorizing Pharmacology @Tony_PharmD
RT @GoKareo: Thanks for joining the #KareoChat @HMNwriters @Tony_PharmD @AnnieBDonahue @TheSocialNurse @GetSocialHealth Always great to see you.
Capture Billing @Capture_Billing
The best way to ensure that there are no rejected claims is to make sure that we have the most up to date information from the patient, no data entry errors and correct code linkage for CPT and ICD-10 #KareoChat
Janet Kennedy @GetSocialHealth
As a patient, I dread having to call my insurance company. I want my physician's office to handle everything because I just expect to hear "No". #KareoChat
Capture Billing @Capture_Billing
Claims can also be rejected immediately when CPT codes and ICD-10 codes don't match. For example a baby well check for a 25 yr old #KareoChat
Janet Kennedy @GetSocialHealth
To be fair - I have used my insurance company's #PatientPortal and participated in some wellness opportunities. #KareoChat
Dr. Tom Giannulli @drtomg_HIT
I deal a lot with lab denials and they are often a loss absorbed by the lab #kareochat
Tebra @TebraOfficial
Time for Q3) How can you Reduce Claim Rejections? #KareoChat https://t.co/mugybT6qOX
Capture Billing @Capture_Billing
@drtom_kareo Sometimes claims do reject for improper auths, but typically we see that as a claims denial vs a rejection #KareoChat
Dr. Tom Giannulli @drtomg_HIT
Complete documentation and correct coding - simple to say hard to do #kareochat
Tebra @TebraOfficial
RT @Capture_Billing: A denied claim is one that is received and processed by insurance but does not actually get paid #KareoChat
Tebra @TebraOfficial
RT @Capture_Billing: A rejected claim is a claim that never actually makes it to the insurance company and is never processed #KareoChat
Capture Billing @Capture_Billing
The most common claims rejections include incorrect patient demographics, incorrect coding, incorrect place of service, out of date information, duplicate claims and eligibility #KareoChat
Capture Billing @Capture_Billing
In our experience the most common rejection from the clearinghouse has to do with patient eligibility #KareoChat
Janet Kennedy @GetSocialHealth
Rejected = Insurance company dropped the ball. Denied = No pay for you! Is that about right, @Capture_Billing? #KareoChat
Dr. Tom Giannulli @drtomg_HIT
So rejections fail the first test of eligibility #kareochat
Capture Billing @Capture_Billing
@drtom_kareo Yes but with new technology and EHRs the documentation and correct coding issues can be addressed in a much faster and more efficient manner #KareoChat
Janet Kennedy @GetSocialHealth
So, for instance, now that the recommendation is for #ColenCancer screenings should start at age 45 (for some patients), how long does it take for new recommendations to be covered by #HealthInsurance? #KareoChat https://t.co/N6vG2osW8z
Capture Billing @Capture_Billing
Rejected = Patient or Billing dropped the ball Denied = No pay for you until you appeal it #KareoChat
Annie Beth Donahue @anniebdonahue
@Capture_Billing @drtom_kareo So an observation here is that EHR can help avoid these rejections by catching fields where information is missing, incorrect, etc. #kareochat
Janet Kennedy @GetSocialHealth
Spelling is not my friend today. #KareoChat

RT @Capture_Billing: Rejected = Patient or Billing dropped the ball Denied = No pay for you until you appeal it #KareoChat
Capture Billing @Capture_Billing
@drtom_kareo Software can verify eligibility, the tools are there, they just need to be implemented by the staff at the time of visit #KareoChat
Janet Kennedy @GetSocialHealth
Taped to my wall now. Thanks @Capture_Billing! #KareoChat (I guess it's not fair to make #HealthInsurance companies the bad guy in every instance. https://t.co/6gPS6RxHRf
Tebra @TebraOfficial
A twist on the last question: Q4) Why do some practices have claim rejection problems? #KareoChat https://t.co/cnlOd2UB1n
Capture Billing @Capture_Billing
@GetSocialHealth Janet, the newer tests and procedures always go through an acceptance process, on a payer by payer basis. For example it took a long time for all insurance companies to approve HPV vaccines for males #KareoChat
Capture Billing @Capture_Billing
Yes @anniebdonahue a good EHR can help your tremendously #KareoChat
Capture Billing @Capture_Billing
Some practices have a claims rejection problem because they may not have the proper procedures in place for patient demographic intake. We see this all the time, patients get tired of filling out these forms over and over #KareoChat
Janet Kennedy @GetSocialHealth
Q4 - I assume that every practice has a well-training billing expert. Would that be true? #KareoChat
Capture Billing @Capture_Billing
@GetSocialHealth Yes the insurance company is not always the bad guy. #KareoChat
Healthcare Writers @HMNwriters
RT @Capture_Billing: Some practices have a claims rejection problem because they may not have the proper procedures in place for patient demographic intake. We see this all the time, patients get tired of filling out these forms over and over #KareoChat
Capture Billing @Capture_Billing
Poor training, inexperienced staff, overworked front desk - these are all things that might contribute to claim rejections #KareoChat
Janet Kennedy @GetSocialHealth
I remember coming into my physician's offices 10 years ago to see a mountain of patient folders when I checked in. Now, it's one piece of paper. Electronic systems have improved the administrative/billing process, haven't they? #KareoChat
Tebra @TebraOfficial
RT @Capture_Billing: Poor training, inexperienced staff, overworked front desk - these are all things that might contribute to claim rejections #KareoChat
Tebra @TebraOfficial
Q5) What features should you require in billing software that can help reduce rejected claims? #KareoChat https://t.co/4aK0IWH5Aj
Capture Billing @Capture_Billing
@GetSocialHealth That would be ideal!! However this is not the case unfortunately. Not having an experienced biller on staff may seriously disrupt the cash flow and A/R cycle for our hard working doctors #revenuecyclemanagement #KareoChat
Capture Billing @Capture_Billing
One stat from the #MGMA states that only 75%-85% of claims are clean and go through the clearinghouse the first time #KareoChat
