MEQAPI @meqapi ... and #MEQAPI is live! Today we are discussing the often-infuriating barriers and obstacles people encounter trying to open pill bottles, blister packs, and other medication containers. Please introduce yourself, and maybe a picture of medications that were difficult to use https://t.co/dkJhFRJBXD
MEQAPI @meqapi Just a note on quality. A core principle in quality improvement is to view the processes and workflow from the eyes of the "process customer" In the case of medications, this is the patient. So quality as measured from pt perspective #MEQAPI
Matthew Loxton is also on Mastodon @mloxton Matthew the healthcare improvement gnome. Working from Denver and DC, and using magic to make healthcare STEEEPA #MEQAPI
MEQAPI @meqapi #MEQAPI T1 Policies: How do national, state, or organizational policies, laws, and processes cause or reduce medication usability and #WrapRage? https://t.co/Fs173Ck97B
Shereese Maynard, MS; MBA 😷 She/Her @ShereeseMayMba Hi Guys. How are we all today? Shereese here. #MEQAPI https://t.co/HVIOUvuyBj
Erin Moriarty Wade @EMoriartyWade @meqapi Hi #MEQAPI I’m a freelance writer specializing in healthcare...and I manage lots of prescription meds for my daughter.
Kimberly Amelia @fireflyrn84 Hey all, Kim here from WI! Glad to be here! #MEQAPI
Matthew Loxton is also on Mastodon @mloxton T1 although many federal and local gov rules and surveillance programs look at drug safety, few look at usability. Very little policy matter on practical usability aspects #MEQAPI
MEQAPI @meqapi @fireflyrn84 Glad you could join #MEQAPI, Kimberly!
MEQAPI @meqapi @EMoriartyWade Welcome back to #MEQAPI Erin!
MEQAPI @meqapi @ShereesePubHlth Hooray, Shereese is here! #MEQAPI
Shereese Maynard, MS; MBA 😷 She/Her @ShereeseMayMba T1 Policies that encourage price gouging, limit competition, and allow muti-patents by primary patent holders reduces medication usability #MEQAPI https://t.co/ZmDBWPeDYw
Erin Moriarty Wade @EMoriartyWade @meqapi T1 I remember covering the early days of the crystal meth epidemic...and watching how state laws changed to put cold/allergy meds behind the counter requiring ID and packaging became way more time-consuming and difficult. #MEQAPI
Heather McCullen @H_SalemOaks Hi #MEQAPI, Heather representing @SalemOaks. I've never participated in this chat before.
Kimberly Amelia @fireflyrn84 T1: Policies for packaging especially within a hospital can differ. Some are standard manufacturer packages for single doses. Other hospitals package themselves for custom doses. OTC meds have policies set by the powers that be for requirements :) #MEQAPI
Kimberly Amelia @fireflyrn84 @EMoriartyWade @meqapi Hi Erin! #MEQAPI
Matthew Loxton is also on Mastodon @mloxton @EMoriartyWade @meqapi I wonder if they also monitored the effects of those security measures on the patients who needed the medication? #MEQAPI
Kimberly Amelia @fireflyrn84 @meqapi Wouldn't have missed this one for the world! #MEQAPI
Kimberly Amelia @fireflyrn84 @H_SalemOaks @SalemOaks Hi Heather, we're seeing a lot of each other lately! #MEQAPI
Rie Lopez, MPH (she/her) @RieOfLetters Hi everyone! I'm going to be lurking mostly as I tend to pressing matters. Great topic! #MEQAPI
Kimberly Amelia @fireflyrn84 @RieOfLetters Hi Rie! #MEQAPI
Erin Moriarty Wade @EMoriartyWade @fireflyrn84 @meqapi Hi Kimberly! #MEQAPI
Matthew Loxton is also on Mastodon @mloxton T1 seems like very little in policy or process that pushes manufacturers to be familiar with how and where the products would be or are used. Simple feedback on usability might change many problems with pill size, blister packs, etc. #MEQAPI
Rie Lopez, MPH (she/her) @RieOfLetters @fireflyrn84 Hey Kim! #MEQAPI https://t.co/OZDCt7EFDK
MEQAPI @meqapi @RieOfLetters Very welcome Rie! Hope you can squeeze in a few thoughts #MEQAPI
ElizabethKelly, Ph.D @Elizabe85727641 Joining a bit late, Liz Kelly, glad to share the time & thoughts. #MEQAPI
Heather McCullen @H_SalemOaks @EMoriartyWade @meqapi When I was getting ready for #MEQAPI I was thinking about physical packaging. I didn't even consider this! Having to ask/give ID to get cold medicine can be a barrier for a number of different reasons. Anxiety, language barriers, is there an option if you don't have ID?
Erin Moriarty Wade @EMoriartyWade @mloxton Maybe it’s a job for @savvy_coop #MEQAPI

Matthew Loxton is also on Mastodon @mloxton @fireflyrn84 When the hospital packages, how are they eliciting feedback or incorporating patient feedback in the choice of packaging, doses, presentation, etc? Did you get any insights? #meqapi
Kimberly Amelia @fireflyrn84 @H_SalemOaks @EMoriartyWade @meqapi At this point, I don't believe there is. At some juncture I think the system will be changing but for now it is ID or no meds unless someone else gets them #MEQAPI
MEQAPI @meqapi @H_SalemOaks @SalemOaks Welcome to #MEQAPI, Heather!
MEQAPI @meqapi @Elizabe85727641 Welcome back Liz! #MEQAPI
Kimberly Amelia @fireflyrn84 @mloxton There is no patient feedback involved in the packaging process. Even while I was still working as an RN before I got ill, we had no input. We took what we got, even if that meant that we broke pills, cut them, used scissors or whatever necessary #MEQAPI
Shereese Maynard, MS; MBA 😷 She/Her @ShereeseMayMba T2 Plastic! The environmentally conscious don't want to use them. Also, for the elderly, once they get home, the environment itself (low lighting and hard to manage medicine cabinets san be an issue. #meqapi https://t.co/l10G6oZj0R
Heidi Grabenstatter @PatientIntv Hi #meqapi. I am sorry to join so late. I like the choice of topics this week, Matthew, an interesting one, from many perspectives.
Erin Moriarty Wade @EMoriartyWade @H_SalemOaks @meqapi I actually remember seeing a Twitter post a while ago from a women physician who went to buy cold medicine for her sick teenage daughter and was turned away because she didn’t have her driver’s license on her. Super frustrating and ironic. #MEQAPI
ElizabethKelly, Ph.D @Elizabe85727641 T1 Is consumer utilization of prescription ever included in the business plan? Has this topic ever received research Monet & efforts? #meqapi
Matthew Loxton is also on Mastodon @mloxton @H_SalemOaks @EMoriartyWade @meqapi At the risk of starting a firestorm, @meganranney commented about how Sudafed has more restrictions than buying a gun. As a physician, she was refused Sudafed at a pharmacy bc she didn't have ID on her. Had to settle for a lesser drug. #meqapi
Kimberly Amelia @fireflyrn84 @EMoriartyWade @H_SalemOaks @meqapi exactly, but if you get narcotics you don't need proof of ID many places unless someone else is picking up the medication. #MEQAPI
PF Anderson @pfanderson@disabled.social @pfanderson RT @fireflyrn84: @mloxton There is no patient feedback involved in the packaging process. PF Anderson @pfanderson@disabled.social @pfanderson @fireflyrn84 @mloxton This! This is a big one. I've been asking the pharmacies where I could buy the nice pill slicers that they use, because I tend to crush them using knives at home. #MEQAPI

Shereese Maynard, MS; MBA 😷 She/Her @ShereeseMayMba @pfanderson @fireflyrn84 @mloxton Those are usually giveaways from vendors. Ask a pharmacist to just give you one. #MEQAPI
Kimberly Amelia @fireflyrn84 @pfanderson @mloxton Usually you can buy them at the pharmacy. I have two of them at home. Many of my medications need split. #MEQAPI Also can get on amazon for very inexpensive
Erin Moriarty Wade @EMoriartyWade @mloxton @H_SalemOaks @meqapi @meganranney Maybe that was the post I saw...I just don’t remember the doctor’s name. #MEQAPI
PF Anderson @pfanderson@disabled.social @pfanderson @mloxton @H_SalemOaks @EMoriartyWade @meqapi @meganranney I was cleaning in my house and found some Sudafed. Thought, oops! MEQAPI @meqapi @PatientIntv Welcome to #MEQAPI, Heidi, glad you could make time!
Kimberly Amelia @fireflyrn84 T2: I honestly don't think any environmental factors go into packaging and that includes the environment in which they are being used. Patients' homes, traveling, vehicles, etc. Not convenient if on many meds. #MEQAPI

PF Anderson @pfanderson@disabled.social @pfanderson @Elizabe85727641 @fireflyrn84 @mloxton And here is an example from Amazon https://t.co/spWDfYTsOX I've had problems when they couldn't get the right size of med to fill the script. Had to go all over town, never did find it #meqapi
PF Anderson @pfanderson@disabled.social @pfanderson RT @ShereesePubHlth: Acu-Life Pill Splitter / Cutter EZY DOSE https://t.co/GdVHRKFZwh via @amazon @pfanderson #MEQAPI
Matthew Loxton is also on Mastodon @mloxton T2 legibility! For many patients, ambient light is never going to be enough for the tiny font they use on many packages and package inserts And for crying in a bucket, what's with the light grey font? #MEQAPI
Erin Moriarty Wade @EMoriartyWade @meqapi T3 I realize it’s a convenience for people, but I personally don’t like the trend of so many medications being sold as pre-filled syringes. I totally get it for diabetics (like my Dad) but I don’t need it for my daughter’s weekly injection. Packaging was wasteful. #MEQAPI
Kimberly Amelia @fireflyrn84 T3: Sometimes the packaging, for example insulin pens--a good amount of education is needed for correct use. Same with inhalers, nebulizers and medications that have to be taken at different times to avoid interactions. The possibilities are endless. #MEQAPI
Kimberly Amelia @fireflyrn84 @pfanderson @Elizabe85727641 @mloxton So frustrating! #MEQAPI
Heather McCullen @H_SalemOaks @mloxton Even just reading the name. If you get more than one prescription filled at the same pharmacy it's not easy to tell them apart. #MEQAPI
PF Anderson @pfanderson@disabled.social @pfanderson RT @mloxton: T2 sometimes the packaging doesn't consider that the environment may be humid and hot, and that disposal of huge amounts of plastic may be hard. #MEQAPI
Erin Moriarty Wade @EMoriartyWade @meqapi T3 And honesty the pre-filled syringe we tried was so much harder to administer...you had to hit her leg very hard in order to get the spring to engage and if you didn’t do it correctly it wouldn’t fully release the medicine. Ahhh, no thanks! #MEQAPI
Matthew Loxton is also on Mastodon @mloxton T3 people factors are huge. Many people lack the visual acuity, dexterity, grip strength, etc to open tough blister packs and safety-seals, or read those tiny instructions Need to be engineered to suit the user, not require helpers and power tools! #MEQAPI
PF Anderson @pfanderson@disabled.social @pfanderson @mloxton I definitely just grab scissors when opening one of those blister packs. Have learned to cur off to the side. And sometimes the blister pack is too tough for the scissors! #meqapi
Kimberly Amelia @fireflyrn84 T2: My migraine nasal spray comes in a box of eight. It has to be assembled and then is only good for 1 hour once it's opened. The cost is $7000 to insurance for 8 doses!!! #MEQAPI

Matthew Loxton is also on Mastodon @mloxton @PatientIntv @fireflyrn84 Yes! Such a foundational need to make the "process customer" i.e. the patient a member of the team making those decisions.
