A1. As OT practitioners, we are responsible for addressing their needs across the lifespan. #otalk
#OTalk @OTalk_
@SarahCordeaux Welcome! :D #OTalk
#OTalk @OTalk_
Question 1 for tonight's #OTalk! https://t.co/Uez9Df5D2x
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@OTthoughts Not a problem! Really interested in learning more about the topic! #Otalk
Nina Franziska (she/her) @ninafoxglove
@OTalk_ Very excited to be joining you again #OTalk

RT @OTalk_: Question 1 for tonight's #OTalk! https://t.co/Uez9Df5D2x
Bill Wong, OTD, OTR/L @BillWongOT
A1. A basic thing we can do is to help develop necessary skills for daily living tasks. Down the line, we have an important role in transitioning them to adulthood. #otalk
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@OTalk_ I may be bit naive as a new grad but i would say the role is to address basic needs eg eating, washing, dressing but also facilitate leisure activities and promote activity and QoL #Otalk
ElspethOT @Els_OT
@RCOT_PLD Occupational therapists are good at supporting people with PMLD to develop meaningful roles in their lives - what ever their abilities #Otalk
#OTalk @OTalk_
Always the best way to sum up OT, no matter who we're working with! :) #OTalk https://t.co/y8L4QFkxvm
Bill Wong, OTD, OTR/L @BillWongOT
@jo3grace Sometimes that can include basic things like self care. Heard of a presentation about an elderly Down Syndrome patient... and the OT was presenting from a sister’s perspective. #otalk

@RCOT_PLD @OTalk_ Great #OTalk -Quite a challenge for me to process information and respond with concise replies-first week back after maternity leave
Bill Wong, OTD, OTR/L @BillWongOT
One thing we also have to address early on is family-centered solutions. 2 days ago, I shared a sad news article where a grandma who developed depression from providing care of her grandson suspected of having ADHD. #OTALK
sunny ot @ot_sunshine
@LeoKellerOT @RCOT_PLD Completely agree its all about engagement in meaningful activity, it takes lots of different shapes #OTalk
Fiona Cairns @fionamcairns
@OTStudentDianne @RCOT_PLD #OTalk participation in activities ie integration into groups with more able customers (eg sensory baking/ cooking)

RT @jo3grace:
Bill Wong, OTD, OTR/L @BillWongOT
Since we are talking about family centred care, I think a good idea is to use COPM on caregivers. #OTALK
Yee Wan Tham @yee_wan_tham
@OTalk_ #Otalk one of them might be addressing their sensory needs through creative activities that are sensory stimulating and cater to their level of ability :)
ElspethOT @Els_OT
Pleased to have time this evening to be joining in with the discussion on #Otalk about the role of occupational therapists who work with people with profound and multiple learning disabilities (PMLD). Very diverse and varied job!
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@RCOT_PLD Gardening groups, groups such as tac pac, sensory sessions eg sensory art, sensory cooking #Otalk
#OTalk @OTalk_
The main thing coming across so far in response to Q1 seems to be that there's a huge amount of variety in our roles! That's probably true of OTs working in most settings, but especially so when the needs of people with PMLD can vary so widely #OTalk https://t.co/7p8BpdpMwG
Bill Wong, OTD, OTR/L @BillWongOT
As for strategies, I feel that structure is extremely important. Moreover, hopefully there will be consistency between environments they frequent- school, home, etc. #otalk
#OTalk @OTalk_
That's a very interesting point @SkillyOT! #OTalk https://t.co/OGGtL6qxE3
Bill Wong, OTD, OTR/L @BillWongOT
Also, it is important to have a team approach between health professionals, teachers, other support workers, and the parents. #otalk

Second question! #OTalk https://t.co/g3CTJElL1b
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@RCOT_PLD I used to use tac pac sessions, sensory room sessions, sensory art or food sessions and music sessions with PMLD #Otalk
Fiona Cairns @fionamcairns
#OTalk involvement in intensive interaction, and training staff in using this technique
Bill Wong, OTD, OTR/L @BillWongOT
Given we are in digital era, though... it is also important for us to be aware of high tech options that can help- such as apps. We should try them out to see if they are appropriate. #otalk
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@RCOT_PLD One of my favourite sessions was having therapy dogs in and allowing service users to brush the dogs, stroke them or just watch the dogs chasing the ball around the garden #Otalk

Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@OTthoughts @RCOT_PLD I used it a little in practice and went on a training session for it, absolutely love the structure of it and the chance for the service user to lead the session #Otalk
Bill Wong, OTD, OTR/L @BillWongOT
Common challenge can be- aside from unpredictable behaviors sometimes, finding appropriate services as they age... as they often might age out of available services. #otalk
Nina Franziska (she/her) @ninafoxglove
@RCOT_PLD Getting to know the individual in a variety of different settings and using observation and Intensive Interaction to communicate and learn about the person's likes and dislikes.#OTalk
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@OTthoughts @RCOT_PLD Im hoping to get my dog @PetsAsTherapyUK accredited! Love animal therapy #Otalk
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@Stellawellawoo @RCOT_PLD I've used this before to help facilitate and create therapeutic relationships, it really helped #Otalk
#OTalk @OTalk_
Intensive Interaction is coming up in a lot of the answers to Q2 - seems to fit well with OT philosophy of being client-centred #OTalk https://t.co/oiQ2frVYFn
Bill Wong, OTD, OTR/L @BillWongOT
An added one that will present itself in a place like Los Angeles is- finding healthcare practitioners that are multi-lingual for this population. #otalk

#OTalk @OTalk_
@Kirstie_OT @colourful_ot Thanks Kirstie! :D #OTalk
Bill Wong, OTD, OTR/L @BillWongOT
Another challenge is- although I hinted this with my last point, I will say families need help too! #otalk
Fiona Cairns @fionamcairns
@OccupationalFox @RCOT_PLD @philking53 #OTalk yes, I work closely with our SLT introducing and using object signifiers for daily routines (eg cloth, toothbrush, cutlery) to add concrete meaning to activity
Nina Franziska (she/her) @ninafoxglove
@IamAnOT17 @RCOT_PLD #OTalk joint work with those who know the person and an MDT holistic approach so important! Have used MOHO and POOL to write up OT findings and recommendations. Am Exploring the use of the Model of Creative Ability at the moment
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@SarahCordeaux @RCOT_PLD I've used most of these forms of communication before, found it really useful #Otalk
#OTalk @OTalk_
Definitely! Also connects to what someone was saying earlier about working closely with speech & language therapists - they have lots of expertise to share re: communication methods! #OTalk https://t.co/68ibNEZ98Q
Nina Franziska (she/her) @ninafoxglove
@sue_mesa @RCOT_PLD #OTalk -yes the historical information is so helpful to form a complete picture and track any changes.
Bill Wong, OTD, OTR/L @BillWongOT
I just have to have a learn if they come approach for question 4. After all, I do have some such patients at my work... but they are either in locked units or bed ridden. #otalk

@RCOT_PLD #OTalk the biggest challenge is the health inequalities still faced by many and that basic health needs are not being met therefore leading to less opportunities for the individual to fully participate in every day activities.
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@OTalk_ Finding the best form of comminication for the person you are working with, understanding and picking up on really small cues such as vocalisations or eye movement #Otalk
Yee Wan Tham @yee_wan_tham
@RCOT_PLD #Otalk adapting our approach and communication to cater to their needs and maximize their level of engagement in performing occupations. Pool Activity Level (PAL) Instrument for Occupational Profiling can help identify a person's functioning level :)
#OTalk @OTalk_
Coming back to Q1, one of the roles of an OT in this area is probably to advocate for clients and try to challenge low expectations of people with PMLD - definitely a challenging one! #OTalk https://t.co/MThSZFQsVH

Answer to question 5- I don’t have much to comment because such patients at my work are either referred by primary physician or nursing (if they experienced decline). #otalk
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@philking53 @RCOT_PLD As an OTA i used to hate staff telling me a su wasn't interested or wouldn't want to engage without even asking/offering choice! It's a big pet peeve of mine #Otalk
Nina Franziska (she/her) @ninafoxglove
@Yee_Wan_Tham @RCOT_PLD #OTalk-yes and just reminded em that it is abrehviated PAL not POOL-ha!
Yee Wan Tham @yee_wan_tham
@RCOT_PLD #Otalk concentration and attention, I find that this client group can benefit more from shorter sessions or longer sessions with frequent breaks
Amie Jayne💚(she/her)🏳️‍🌈 @Amie_OT
@OTStudentDianne @JHedgesStearn @RCOT_PLD @jo3grace I previously used objects with someone to link to activities eg a drum for a music session or a paint brush for art, found it useful #otalk
Fiona Cairns @fionamcairns
#OTalk the debate about cognitive functioning age V chronological age, and how to pitch activities so they are meaningful and suited the the individual

@RCOT_PLD This is a really good question, I guess I would ask if there could ever be enough research into OT practice?! The evidence base for some interventions in this area needs to be strengthened....that’s why we need more OT’s involved with research! #Otalk
