@naomi_gallant Often for lack of resources, people who have capacity to communicate (receptive and expressive) but not in normative oral and written form will be skipped over in research not specifically directed at them but v important to them due to "capacity" #OTalk

In my experience (@hooper_ek) this can be a real problem .... people can act as gatekeepers and make assumptions without making the information accessible so that the person can make their own choice #OTalk

@naomi_gallant #otalk people with dementia who may not have the ability to verbalise their opinion, some service users with Parkinson’s who again may not be able to understand. There are lots. I work with older people who I find some of them can not always engage with me.

@OTalk_ @naomi_gallant Recognising both consent and assent concepts helps. Barriers include some ethics committees, institutional gatekeepers, and time taken to flexibly schedule participation with fluctuating capacity and will #OTalk

@preston_jenny @Carolin64109281 @naomi_gallant Yes, this kind of "protection" happens from multiple sources. Also with people whose actions or opinions may be presented in ways that are imperfect or embarassing (sometimes to the one saying no, rather than the individual). #OTalk

@naomi_gallant #otalk I was thinking in their best interest? Speak with family/advocate about advanced decisions?? Would they be interested in using research for e.g. to help others? Had they had any interest in ‘making things better for others’?

There is provision under the mental capacity act to use a personal or nominated consultee in order to facilitate involvement of a person who lacks capacity @shirleypearceot @naomi_gallant #OTalk

#OTalk I think practical tips in terms of increasing possibility of capacity to consent - easy to read information guides, or video clips, communication aids, reduced environmental distraction, giving time to respond, working with families

Having conducted a research study with pwd in acute care, I think the barriers start with ethics committee - we talk about the need to include these groups but the reality is ethics boards are still very against researching people who lack capacity #OTalk

@naomi_gallant Do we need better education and guides for ethics committees (another place of limited clinician involvement)? I've encountered great advocates, ones that had a scary degree of trust in me, and ones that I never did see the ethical issue in their barrier #OTalk

@sherlynmelody @naomi_gallant This will definitely be challenged by the ethics committee specifically in relation to the potential benefits or otherwise of research participation. This could be quite hard to argue despite the fact that all research helps others in some way #OTalk

@OTalk_ @emilyoliverdem @naomi_gallant #otalk I’ve used an iPad to help a lady understand what I was asking her. She was a bit deaf but could read my questions ‘typed in 72 point font’ this helped with communication and establishing capacity.

@OT_Jen @naomi_gallant There are opportunities for us all to get involved with your local ethics committee. I thoroughly recommend it. Joining the ethics committee was one of the best things I did as a novice researcher #OTalk

@naomi_gallant I think that depends on the research question. I worry about choosing methodologies ahead of the question. Whatever the methodology there needs to be a genuine need for the research and for evidence that all ethical principles are being adhered to #OTalk

As long as the research needs to involve people who lack capacity the mental capacity act can enable participation by using a nominated consultee .... but .... the researcher MUST attend to the responses of the participant i.e. ensure that assent is present #OTalk
