IAPMD @IAPMDglobal Hi! This is Amanda and I am the Co-Founder and Executive Director @IAPMDglobal IAPMD - International Association for Premenstrual Disorders and I am based just outside Boston #PNDHour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul Hello #pndhour! I'm Tory Eisenlohr-Moul, PhD-- I'm the Chair of the @IAPMDglobal Clinical Advisory Board and Associate Director of the Women’s Mental Health Research Program at UI Chicago. I’m the lead scientist at @clearlabuic; we use hormone experiments to study #pmdd and #pme.

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A1. Premenstrual Dysphoric Disorder (#pmdd). Premenstrual = the two weeks before menses. Dysphoria = a state of unease, dissatisfaction with life, feeling upset, and general agitation. Disorder = symptoms causing interference with life activities or extreme distress. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul RT @viciouscyclepmd:
IAPMD @IAPMDglobal A1 - #PMDD stands for Premenstrual Dysphoric Disorder. It is an often debilitating condition that affects about 2-10% of women/AFAB individuals of reproductive age. #PNDHour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A2. #PMDD is a hormone-sensitive emotional disorder characterized by severe distress ONLY in the two weeks before menstrual bleeding. About 5.5% of cycling individuals meet strict criteria for the disorder, which was just added to the diagnostic manual (DSM-5) in 2013. #pndhour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A3. #PMDD by definition has to include dysphoria-- severe emotional symptoms-- as a central element, whereas #PMS does not. #PMDD emotional symptoms are severe enough to be upsetting or impairing, whereas #PMS symptoms don’t have to be. #pndhour

Core #PMDD emotional symptoms in the DSM-5: (1) mood swings, sudden sadness or tearfulness, rejection sensitivity, (2) irritability, anger, conflicts, (3) depression, hopelessness, self-deprecating thoughts, (4) anxiety, tension, on edge. One of these is required. #pndhour

Victoria @Rayvon_V @viciouscyclepmd @eisenlohr_moul Hey! I’ve had a sort of diagnosis of #PMDD GP said my symptoms fit, but is there a way to formally diagnose? Only treatment option suggested was low dose anti-depressants. Trying to understand if this is the long term source of anxiety, low mood or a new thing! #PNDHOUR
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul What a great question! I know that many people with #pmdd have migraines, and many do not. I believe they are separate but overlapping conditions. Menstrual migraine is typically found during estrogen withdrawal, a few days after #pmdd symptoms peak. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul This is a great question as well. There have been no studies evaluating how often #pmdd and #endo co-occur! We need to change that. #pndhour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A5. Before I answer that, it’s important for me to say what I mean by “mental health disorder”. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A5. As a scientist studying the impact of hormones on the brain and behavior, I define “mental health disorder” as a disorder that is characterized by altered brain function and behavior and therefore requires research by brain experts. #pmdd fits that criteria. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A5. So yes, #pmdd is a mental health disorder, since the problem is in the brain's response to hormones-- but it is just as biological as any other medical disorder. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A5. #pmdd is characterized by ABNORMAL brain activity during NORMAL hormone changes, I’d say we need brain experts working to cure #pmdd and I’d call it a mental health disorder #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A5. Since #pmdd symptoms are ONLY present during hormone flux, gynecologists are helpful in later stages of treatment for #pmdd (medical or surgical menopause). But technically hormone function is normal in #pmdd, so we don’t call it a gynecologic/endocrine disorder. #pndhour
IAPMD @IAPMDglobal A5 - 2/2 IAPMD is working to bridge the gap between mental health and premenstrual health. #PMDD is unique in that removing the ovaries and/or ceasing the cycling of estrogen and progesterone causes symptoms to stop. #PNDHour
IAPMD @IAPMDglobal A6 - 1/3 While much more research is needed, #PMDD appears to be a severe negative response to the natural rise and fall of the reproductive hormones estrogen and progesterone. It is not a hormone imbalance but rather a severe #hormonesensitivity. #PNDHour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A6. When we use experimental methods to manipulate hormones in women with and without #PMDD, we see only women with prospectively-confirmed premenstrual symptoms (#pmdd) show a negative emotional reaction to hormone changes. Women without #pmdd don’t. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A6. Contrary to the popular culture myth of "hormone imbalance", levels of hormones are normal in people with #PMDD, but they have an abnormal mood reaction to normal hormone changes that occur across the menstrual cycle. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A6. Therefore, we say that #pmdd is an abnormal brain response to normal hormone changes, or #hormonesensitivity. #pndhour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A7. You can have dysmenorrhea and #pmdd, but the symptoms can't overlap. the #PMDD symptoms have to be separate from the menstrual bleeding-related symptoms. #pndhour
IAPMD @IAPMDglobal A7 - While some with #PMDD may have heavy menstrual bleeding, heavy bleeding (aka Menorrhagia) is not a symptom of PMDD #PNDhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul Absolutely. At the end of the day, mental and physical health are both affected by context and biology-- there is no distinction between "mental health" and "physical health". It's all genes, environments, and their interactions. https://t.co/h0kjCq6Vmp #pndhour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A9. The first-line conservative treatment for PMDD are SSRIs (selective serotonin reuptake inhibitors, for example: sertraline)— about 60-75% of women with PMDD benefit from SSRIs. SSRIs seem to work faster in PMDD (working better than a sugar pill after about a day!). #pndhour

IAPMD @IAPMDglobal A9 - For about 60% of those with #PMDD firstline treatment options work to manage symptoms incl SSRIs, hormones, and lifestyle changes. For 40% shutting down the ovaries through chemical or surgical means is an effective lastline treatment option. https://t.co/jjdmKk1aZs #PNDhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A9. The second-line treatment is drospirenone-containing oral contraceptive pills that contain estrogen (.02 mg of ethinyl estradiol) plus a new progestin (3mg drospirenone), taken on a 24-4 schedule (24 active pills, 4 inactive pills). These pills prevent ovulation #pndhour.
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A9. If neither SSRIs nor OCs have been effective for PMDD after a few cycles, and the cyclical symptoms are severe, a doctor can prescribe other, more intensive cycle suppression treatments to suppress the cyclical hormone changes that are known to trigger PMDD. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A9. The best-studied cycle suppression method is a monthly injection of GnRH agonist (the most-used example is called Lupron), which shut down hormone production by the ovaries, causing a reversible menopause in which your hormone levels stay very low and flat. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A9. If the GnRH agonist plus hormone addback (“menopause trial”) goes well, many people will choose to have a THBSO (removal of the uterus and both ovaries) to once-and-for-all remove the hormonal flux that causes #pmdd. #pndhour

Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A13. #PME is a more common but still disabling disorder. PME happens when someone with a chronic psych disorder has symptoms all the time that are troubling, but develops much more distressing/disabling symptoms in the premenstrual or menstrual weeks. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A13. To meet DSM-5 diagnostic criteria (est 2013) for #PMDD, you must have daily ratings to show that you have at least 1 emotional symptoms (and at least 5 symptoms total) that follow a cyclical pattern becoming severe premenstrually, then "shutting off" during menses. #pndhour

IAPMD @IAPMDglobal A13 - 2/2 There are no blood tests that can diagnose #PMDD. To get an accurate assessment, you will need to provide a detailed history of your symptoms including frequency and severity #pndhour https://t.co/GX4O45lP92

IAPMD @IAPMDglobal A13 - 3/3 PMDD can not be diagnosed via blood, hormone, or saliva test. These tests can, however, rule out any underlying disorders including hormone imbalance or thyroid issues. The only way to test for #PMDD is by tracking symptoms for a minimum of two cycles or more #PNDHour
PMDDOZ @pmddoz RT @viciouscyclepmd:
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A14. In the only study to use daily ratings to diagnose #PMDD, they found that that was NOT an elevated risk of #PND among those with #PMDD.. This is probably because PMDD and PND are driven by different mechanisms. See this paper: https://t.co/vTE4q2VK0h #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul A14. However, since both #pmdd and #pnd have subtypes that are not well understood yet, it is very possible that there are subtypes of pmdd and pnd that overlap-- we just don’t know yet. #pndhour
PMDDOZ @pmddoz RT @eisenlohr_moul: A6. Therefore, we say that #pmdd is an abnormal brain response to normal hormone changes, or #hormonesensitivity. #pndhour

PMDDOZ @pmddoz RT @IAPMDglobal: A1 - An estimated 5-10% of women/AFAB of reproductive age have #PMDD. This number does not account for missed or misdiagnosis nor women whose ovulatory cycle is suppressed by hormonal, chemical, or surgical means. #pndhour https://t.co/GwUjPvIxXo

IAPMD @IAPMDglobal Is there a link between #PND/#PPD and #PMDD? See @eisenlohr_moul answer below! #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul There is definitely evidence that #pmdd worsens with age, and it's possible that this is due to pregnancies somehow increasing #hormonesensitivity. #pndhour
Valerie Minnich @peridotmage RT @viciouscyclepmd:
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul It's also possible that the treatment-resistant subtypes of #pmdd and #pnd overlap. More work needed! #pndhour @IAPMDglobal
Gabrielle Rocha Rios, MA (She/Her) @IntPsyGabrielle RT @eisenlohr_moul: A13. #pndhour
IAPMD @IAPMDglobal A14 - While research does not show a link between #PND/#PPD and #PMDD doctors should provide a similar standard screening for women/AFAB of reproductive age. #PMDD is often misdiagnosed as other mental health conditions. Screening would expidite diagnosis and treatment. #pndhour
Tory Eisenlohr-Moul, PhD (CLEAR Lab) @eisenlohr_moul @IAPMDglobal And subtypes of each disorder should be further studied to see if treatment-resistant women with #hormonesensitivity disorders of #pnd #pmdd might overlap to a greater degree than those who respond well to SSRIs, which are first-line treatments for both disorders. #pndhour |
