So lets get the elephant in the room out first.... 3 hours of exercise per week for at least 4 months for effective falls prevention.... thoughts? Sherringtons review shows any exercise for 3 hours per week but should predominantly be challenging balance work #physiotalk

@physiotalk Hi all, tweeting from Dublin, Ireland and working in rehab with frail elderly #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Something I’ve been looking at in terms of follow up in ed! Some primary fractures are not seen in fracture clinic so miss out on the excellent fracture liaison service! Trying to improve this especially in our older population. #physiotalk
Janet Thomas @JanetThomas47
@physiotalk Yes - but I should as that's the area I work in! All our patients get a Q# at least #physiotalk
physiotalk @physiotalk
@physiosoc Lesson learnt ;) Enjoy! #physiotalk
Nicola Lauchlan @NicolaLauchlan
@LaterLifeTrain Having seen the effects on people who adhere to this guideline I can say it makes the biggest difference to falls risk, and has the best long term results in terms of adherence because they really buy into the approach. But I do think anything is better than nothing #physiotalk
kate bennett @kategahr_kate
@physiotalk It is a standard part of our comprehensive falls assessment. A frax is completed and the relevant action taken for every falls referral unless they are already on bone protection but even then we may ask for a review. Advice leaflets given to every person who falls #physiotalk
Will Gregory @physiowillgreg
@LaterLifeTrain Not realistic for rehab on the NHS (budget), but suggest this is where community leisure trusts come in. In Salford we are very fortunate to have 12 week postural stability classes provided by @salfordleisure #physiotalk

If you are NOT working with frailer older adults - do you think about bone health? What about all you #MSK physios out there! #physiotalk
Nicola Lauchlan @NicolaLauchlan
@JudeVBolton I don't think you're alone Judith. Most of us have other priorities and sometimes getting/keeping people on their feet feels like an uphill struggle in itself #physiotalk
Later Life Training @LaterLifeTrain
But of course, she was meta-analysing data from research trials aimed at reducing risk, so all included a balance element and none were seated for eg. For very frail, walking on uneven ground can be highly chall bal, for others, circuits & structured exercise prog #physiotalk

@physiowillgreg @LaterLifeTrain @SalfordLeisure We can't offer this as community physios. We can do 12 weeks then refer onto classes but as classes now run in blocks not rolling programme by the time there is space the impetus and motivation often lost #physiotalk
Later Life Training @LaterLifeTrain
Indeed Will, transition on after you have worked your magic is vital :) But need them to be adhering to home exercise in between seeing you weekly too.... #physiotalk

so important to seek out PSIs and OEP Leaders and other good S & B programmes to ensure speedy onward referral from you so don't lose them at that point as well :( #physiotalk

@physiotalk Not always when it comes to younger patients. But always patients 60+ I work in primary care, though most of my patients are older #physiotalk
physiotalk @physiotalk
@JoFallsPhysio Good to have you back Jo! #physiotalk
Later Life Training @LaterLifeTrain
YES :) but they need to really understand WHY this is so important - we need to be PASSIONATE so they are - its all about informing them so they can understand the need #physiotalk but this is the case for ALL exercise - and especially if recently diagnosed with OP #physiotalk
Will Gregory @physiowillgreg
@LaterLifeTrain Thanks @laterlifetrain not quite magic but certainly a big emphasis on educating re: why home exercise is needed, ans also throw in peer support and goal setting plays a big role #physiotalk
Mari @mariand84
@physiotalk not as much as I should… shamed to admit. #physiotalk

physiotalk @physiotalk
Great stuff! Lots to talk about so… Q2: Strong: Do you currently adapt strength exercises for people with osteoporosis? #physiotalk
kate bennett @kategahr_kate
@physiotalk @AideenPhysio Yes...but we don't tend to being perfectly honest. Younger patients that fall don't tend to come through falls referral route so dependent on MSK for this (as if having fracture rehab will tend to be MSK) #physiotalk

Aideen Larmer @AideenPhysio
@physiotalk Within outpatients without any other risk factors I don't think directly - I would then shift emphasis to giving general guidance on exercise as that it likely to help anyway - although this wouldn't necessarily include impact exercises #physiotalk
Later Life Training @LaterLifeTrain
You need to FIGHT for 4 patients rights ;) if there is a waiting list offer a list of self employed local PSIs that pts MAY feel inclined to use? Speak to powers to be that the waiting list is losing people that need help and you have to be able to offer choice #physiotalk

Will Gregory @physiowillgreg
@physiotalk Q2 #physiotalk yes, strength work has been a big part of our osteoporosis rehab. Love the study that equates back extensor muscle strength (and spinal ROM) to quality of life. So strenghtening exercises to all, and individualisation / adapt in one-to-one or in the circuit class.
physiotalk @physiotalk

Dr Melrose Stewart @MelroseStewart1
@mariand84 @physiotalk I emphasise importance of bone health, especially with students. I encourage them to have it as part of their health promotion package. #physiotalk

@NicolaLauchlan Happy for them to progress to loaded flexion in later rehab stages? #physiotalk
kate bennett @kategahr_kate
@LaterLifeTrain Sadly hands are tied! When I go back to work I will pick up the mantle but we are so tied into current pathways we aren't able to look elsewhere...no funding to begin with. And not necessarily priority following the findings of the hip.sprint audit for Southampton #physiotalk

@physiotalk Not as much as I would like too. #PhysioTalk
Nicola Lauchlan @NicolaLauchlan
@physiotalk Depends on the patient and clinical reasoning. At later stages, instead of abdominal crunches, for example, I'll use adaptations of the 'plank'. Neutral position, improving core strength, with added benefit of practicing on/off floor. #physiotalk
Jo Kitchen @JoFallsPhysio
@physiotalk Yes! More weight bearing exercises. More caution building reps/weights. Maybe too much caution...#physiotalk
Later Life Training @LaterLifeTrain
@l_giangregorio @kategahr_kate Yes :) but need to still ensure a reasonable dose and adherence. Maybe some new prompt technology such as @PreventitEU will help support adherence? #physiotalk
Kirstie Stenhouse @Kirstieahp
@LaterLifeTrain Yes many patients really don’t understand the huge impact exercise can make in their #bonehealth need to make sure as much emphasis on this as meds #physiotalk
Judith Bolton @JudeVBolton
Maybe telling someone just diagnosed with #osteoporosis that they can do moderate impact activities might help them deal with the diagnosis. It can be the fear which is the biggest factor. #PhysioTalk
Bex Townley @BexTownley
Gate crashing this #physiotalk as an exercise professional
Nicola Lauchlan @NicolaLauchlan
@LaterLifeTrain Moderate activities are relative to the patient's current functional ability so I'd be comfortable with this for everyone by tailoring moderate to what's moderate for them #PhysioTalk

Will Gregory @physiowillgreg
@LaterLifeTrain Depends if/when fractured. Have had instances of seeing patients too soon post # & missing chance to follow up at a better post# time. No discomfort if recommending evidence based treatments :) But, yes, need time to get to know pts motivations / story to then tailor #physiotalk
Shakila Devi Perumal @SdPerumal
@physiotalk How do physios in respiratory care view their contribution to the community, which is the first point of contact for patients? #physiotalk
Lora Giangregorio 🇨🇦 @l_giangregorio
@LaterLifeTrain @kategahr_kate @PreventitEU agreed! to be honest, after seeing adherence drop in our home exercise trial, I try to encourage people to attend supervised exercise even if intermittently, to encourage adherence and social engagement, in addition to ensuring sufficient dose and progression. #Physiotalk
Later Life Training @LaterLifeTrain
@Kirstieahp You are so right. Kinesphobia (avoidance of activity) is common in people diagnised with #osteoporosis and this just worsens their bone health. Really vital to educate the importance of regular wt bearing and strengthening activities to help maintain or promote #physiotalk
Nicola Lauchlan @NicolaLauchlan
@MelroseStewart1 @physiotalk Far too common sadly #PhysioTalk

Amy Tildesley @amytids
@physiotalk I try and adapt all exercises to my particular patient. What I gave would be based on them and their ability and not a diagnosis of OP #physiotalk
Rob Wagner @SaSSyWagner
@physiotalk This may not be the right place to ask but I believe some studies have shown that what we traditionally call 'bad form' when lifting can actually reduce load on the spine - is there any prospective data showing 'good technique' reduces the rates of vertebral fracs? #physiotalk
Kirstie Stenhouse @Kirstieahp
@MelroseStewart1 @physiotalk And younger patients too! Terrified to do anything in case cause a fracture as a health professional or gym instructor has given them the fear. The new guidelines should really help this #physiotalk

physiotalk @physiotalk
Ok, moving on! Q3: Strong: Will you change your practice after reading the consensus statement? #physiotalk
Later Life Training @LaterLifeTrain
Ah yes, one persons moderate is not another persons moderate - and always relative to current ability and function of course #physiotalk
Claire O' Connell @ClaireOCPhysio
