Janet Thomas @JanetThomas47
Hi I'm on the physiotalk account tonight tweeting from a crisp and chilly Edinburgh #physiotalk
Dr Susanne Arnold @FinneganSusanne
@JanetThomas47 Ooops #physiotalk
Janet Thomas @JanetThomas47
@physiotalk I do think terms such as supervision, stand by assistance, close supervision etc when used for walking are very nuanced - and my meaning may not match others! #physiotalk
HelenPR @HelenPR9
@physiotalk #physiotalk I work in Hull Royal Infirmary
Dr Susanne Arnold @FinneganSusanne
@physiotalk It has been some time since I wrote clinical notes but do remember some comments from various members of the MDT about our abbreviations being a special physio language #physiotalk
Dr Susanne Arnold @FinneganSusanne
@physiotalk Do other disciplines use their own special language? #physiotalk
Dr Susanne Arnold @FinneganSusanne
@JanetThomas47 @physiotalk Is this the way we taught as students that has just stayed with us? #physiotalk
Carla Ellis @carlatrottellis
@physiotalk #physiotalk Am certain other professions do not understand. Do we encourage them not to read our notes by filling them with jargon?
Janet Thomas @JanetThomas47
@FinneganSusanne @physiotalk Possibly? But is it different on different placements? Wonder if any lurking students could let us know if you have experienced different meanings to terms on different placements? #physiotalk
Dr Susanne Arnold @FinneganSusanne
@physiotalk How often do we all read each otherโ€™s notes? Is this different now with the use of electronic records?#physiotalk
Dr Susanne Arnold @FinneganSusanne
@JoJenningsNHS @physiotalk Do you think it has become a time saving technique @JoJenningsNHS #physiotalk
Janet Thomas @JanetThomas47
@FinneganSusanne @physiotalk We had shared notes before the introduction of electronic notes - but did we all read each others entries? A skim at least to ensure safety of course. #physiotalk
Dr Susanne Arnold @FinneganSusanne
@JoJenningsNHS @physiotalk @chris_richards2 I find myself writing physio abbreviations in every day life writing so they no I would still be โ€œguiltyโ€ of it in clinical practice #physiotalk
Carla Ellis @carlatrottellis
@physiotalk #physiotalk definitely not but there is a line to be drawn as cannot write an essay on every note, every prof has jargon, and every prof needs to be understood by others. If your communication is not clear then it is not effective
Janet Thomas @JanetThomas47
@JoJenningsNHS @FinneganSusanne @physiotalk I do remember when I first qualified notes could be a line - now they can be half a page at a time (or more!) Will discuss legal and litigation later in chat - but wonder if this is an issue? #physiotalk
Dr Susanne Arnold @FinneganSusanne
@JoJenningsNHS @physiotalk I went on a medico-legal course once and that scared me so much that I was always extra careful about notes (as much as I could be!!) #physiotalk
Janet Thomas @JanetThomas47
@physiotalk I THINK they do? But could I be clearer - probably I could! #physiotalk
Francis @F_MagUidhir
@FinneganSusanne @physiotalk @JoJenningsNHS #physiotalk why? What did you learn?
Dr Susanne Arnold @FinneganSusanne
@physiotalk @JoJenningsNHS It did for me but maybe wore off eventually??? #physiotalk
HelenPR @HelenPR9
@physiotalk our ICU has some new documentation which has boxes to fill in. It is great. Covers everything as there is also room to put other things down. #physiotalk
Dr Susanne Arnold @FinneganSusanne
@physiotalk @AKimboCake @JanetThomas47 I expect local dialects and terminology do make a difference #physiotalk
Francis @F_MagUidhir
@JoJenningsNHS @physiotalk @FinneganSusanne If you are doing SOAP notes (or a derivative) are you not obliged to write all findings regardless of their outcome? #physiotalk
Janet Thomas @JanetThomas47
@FinneganSusanne @physiotalk @AKimboCake Its a long time now since I moved from England to Scotland but I remember that certain well known and used terms in hospitals in England were just not known in Scotland (and I'm sure the other way as well!) #physiotalk
Dr Susanne Arnold @FinneganSusanne
@physiotalk @Harris_AC @JanetThomas47 For me assistance meant some kind of hands on help/guidance whereas supervision meant just standing by but no physical help - it still feels like a โ€œgreyโ€ area as I write it now!! #physiotalk
Dr Susanne Arnold @FinneganSusanne
RT @JanetThomas47: @FinneganSusanne @physiotalk @AKimboCake Its a long time now since I moved from England to Scotland but I remember that certain well known and used terms in hospitals in England were just not known in Scotland (and I'm sure the other way as well!) #physiotalk
Francis @F_MagUidhir
@JoJenningsNHS @FinneganSusanne @physiotalk Sorry to be a pain but how is it you now write how a patient was informed? Do you write "patient informed of x" ? #physiotalk
Janet Thomas @JanetThomas47
@chris_richards2 @physiotalk I do use abbreviations (I'm just not as good as you!) but we do have a standardised list. When developing it many year ago there were so many discrepancies in meaning between teams! #physiotalk
Dr Susanne Arnold @FinneganSusanne
@F_MagUidhir @JoJenningsNHS @physiotalk I think I used to write โ€œadvised patient to ask for assistance if wishing to walkโ€ or something like that #physiotalk
Carla Ellis @carlatrottellis
@physiotalk #physiotalk We all use jargon but in acute settings (maybe all settings) can assume too much is shared with other members of MDT
Carla Ellis @carlatrottellis
@physiotalk #physiotalk interesting to assume that if others donโ€™t understand recommendations then they donโ€™t understand your role
Francis @F_MagUidhir
@physiotalk #physiotalk only ever use abbreviations that is in common parlance. I never use abbreviations that are only known in the medical world.
Francis @F_MagUidhir
@JoJenningsNHS @FinneganSusanne @physiotalk Very interesting. Thanks #physiotalk

Janet Thomas @JanetThomas47
@hfdub @physiotalk @JoJenningsNHS @FinneganSusanne Yes! It was when I was asked to read a set of note (many years ago now) for a complaint that I realised so many abbreviations were adding confusion SOBOE, SOB, SOOB, SOOBE for example! #physiotalk
Dr Susanne Arnold @FinneganSusanne
@physiotalk Probably only when working with students or another member of the MDT who was shadowing me?!? #physiotalk
Carla Ellis @carlatrottellis
@physiotalk #physiotalk yes and often had to clarify as the trouble with jargon is weโ€™re so used to it we donโ€™t realise weโ€™re doing it. Have you tried looking up the CSP list of accepted abbreviations? It might surprise you whatโ€™s on it, tribute to @PhizzyLizzy
Dr Susanne Arnold @FinneganSusanne

HelenPR @HelenPR9
to be fair the worst physio notes I've read have to be far better than any doctor's notes I've read. And that's aside from the scrawly handwriting. #physiotalk
Dr Susanne Arnold @FinneganSusanne
@JanetThomas47 @hfdub @physiotalk @JoJenningsNHS We used to try to do regular notes audits (when we had time) but that was to check on content i.e. problem lists etc. rather than abbreviations #physiotalk
Francis @F_MagUidhir
@physiotalk #physiotalk all this talk of abbreviations and you go and use an abbreviation! tisk tisk https://t.co/9CndeVDf6c
Janet Thomas @JanetThomas47
@FinneganSusanne @hfdub @physiotalk @JoJenningsNHS Now that's a reminder for us to do our 6 months notes audit - usually comes up in December but I think has gone slightly awry with some enforced closures over the past year! (audit does ask if abbreviations are on our standard list) #physiotalk
Dr Susanne Arnold @FinneganSusanne
@JanetThomas47 @hfdub @physiotalk @JoJenningsNHS Impressive stuff @JanetThomas47 ๐Ÿ‘๐Ÿป #physiotalk
Dr Susanne Arnold @FinneganSusanne
@BunceLaura @physiotalk Should our physio language be standardised across under graduate training? Do bad habits sneak in after we qualify? #physiotalk
Dr Susanne Arnold @FinneganSusanne
@BunceLaura @physiotalk I agree that it would probably be very hard to implement @BunceLaura #physiotalk
Carla Ellis @carlatrottellis
@chris_richards2 @physiotalk @PhizzyLizzy @PhizzyLizzy maybe able to help as she led the notes audit #physiotalk
Dr Susanne Arnold @FinneganSusanne

@carlatrottellis @chris_richards2 @physiotalk @CSPScot The joy of electronic notes - we have been able to create a pre-programmed list of abbreviations we like to appear at the end of every notes entry. Which is really cool and a great time saver! #physiotalk
