physiotalk @physiotalk
@SamHBooth Hi Sam! just pop a "#physiotalk" in each tweet so we can see what you have to say
physiotalk @physiotalk
@IanTyrrell5 Evening Ian! #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Where to start!! Patient can’t get out of bed until seen by a physio. Patient is in a residential home doesn’t need to be seen by a physio. Patient has a ?dvt so can walk just a few of my main sweary bugbears at the minute #physiotalk
Sam Haworth-Booth @SamHBooth
@physiotalk D’oh - haven’t done this for a while #physiotalk but will try to remember!
Dale Whelehan @Daleyfurter
@physiotalk 'That physio didn't do anything - wouldn't put any machines or massage the pain away. All they gave me were these really specific exercise' #physiotalk
Claire Campbell @ClaireC_PT
@physiotalk That you need to ‘do’ something to ‘get people on board’ #physiotalk
KB @KBNeuroinfo
@physiotalk Referred for ‘intensive’ physiotherapy #physiotalk
Dale Whelehan @Daleyfurter
@physiotalk Though I think as a profession we are victims of our own demise in this sense - using non evidence based treatment to build private business models #physiotalk
physiotalk @physiotalk
@Daleyfurter Interesting - myths from a patient's perspective. Where do you think this mindset originates? #physiotalk
Ellena @ellena_farrow
@physiotalk “I have to wait for the physio to come back and see me before I can get out of bed again” 🙈 #physiotalk
Aideen Larmer @AideenLarmer
pain = damage = avoid #physiotalk
Claire Campbell @ClaireC_PT
@physiotalk That physio is an intervention, not a profession #physiotalk
Martyn Wyres @MartynWyres
@physiotalk That talking to your patient is not an effective treatment on its own #physiotalk
Ian Tyrrell @IanTyrrell5
@KBNeuroinfo @physiotalk Aggressive chest physio! #physiotalk
Jo Kitchen @J0Physio
@KBNeuroinfo @physiotalk Can you explain what is the myth part of this? That intensive physio will happen? Or that the patient does nothing and the physio does intensive stuff? #physiotalk
physiotalk @physiotalk
@AideenPhysio Current areas of contention in physio... yes, it will be good to see what crops up in discussion! Any one in particular that you think is damaging? #physiotalk
physiotalk @physiotalk
@SamHBooth You're dong fine Sam. Wont take long to get back into it. #physiotalk
physiotalk @physiotalk
Interesting! We're seeing some familiar fallacies come up here… Let’s move on to Q2 #physiotalk
physiotalk @physiotalk
Q2 How do you identify that a practice or intervention might be based on misinformation? #physiotalk
Martyn Wyres @MartynWyres
@physiotalk The dreaded ‘no rehab potential’ quote #physiotalk
Ian Tyrrell @IanTyrrell5
@MartynWyres @physiotalk Think we neglect how important history taking is. I sa a quote the other day “a bad history is exactly like walking into a maze with a blindfold on” actively listening and watching patients is key! #physiotalk
Andy @themcmullansNI
Q2 It doesn't work well, it lacks plausibility, it is marketed more than it is researched #physiotalk
Aideen Larmer @AideenLarmer
@physiotalk Manual therapy - not so much it's use but the justifications behind its use... moving things, putting things back in place etc #physiotalk
Pamela Kipps Hansford @PamKHphysio
@physiotalk #physiotalk That because we have licence to touch, we can touch people whenever we like without asking permission!
Lewis Lovegrove @_lewadam
@IanTyrrell5 @MartynWyres @physiotalk 100%, should always keep an open ear, especially incase vital information was not mentioned during the initial assessment #physiotalk
Martyn Wyres @MartynWyres
@physiotalk No clinical reasoning to back it up, it was magic #physiotalk https://t.co/NJbGWjPR1W
physiotalk @physiotalk
RT @themcmullansNI: Q2 It doesn't work well, it lacks plausibility, it is marketed more than it is researched #physiotalk
Tylah Marston @marston_physio
RT @MartynWyres: @physiotalk The dreaded ‘no rehab potential’ quote #physiotalk
Ian Tyrrell @IanTyrrell5
@_lewadam @MartynWyres @physiotalk And asking difficult questions that may make you uncomfortable but need to be addressed. #physiotalk
KB @KBNeuroinfo
@J0Physio @physiotalk That physio is an intervention that is ‘done’, and can only be performed during physio sessions- need to empower self practice #physiotalk
Cheryl Grindell @CherylGrindell
@J0Physio @KBNeuroinfo @physiotalk We get referrals like this a lot I think the myth is that intensive physio is the physio ‘doing something’ to the patient at frequent intervals and that exercise programmes esp home ex isn’t intensive physio because the physio isn’t there doing their ‘magic thing ‘ #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk @MartynWyres Don’t forget the hashtag! 😉 #physiotalk
physiotalk @physiotalk
Speaking of major myths in physiotherapy.. #physiotalk
Claire Campbell @ClaireC_PT
@physiotalk A lot of treatments or practice that we now consider ineffective seemed to make logical sense (e.g. Rx of trigger points), so we cannot rely on our biases, or theory, but test it. Unfortunately, that is notoriously hard to do due to non-specific factors. #physiotalk
Suzanne Brown 💙 @suzeeyre
@IanTyrrell5 @physiotalk Aarrgghh! This #physiotalk is not going to be good for my mental health 😡 Your first 2 are particularly annoying to me 😬
physiotalk @physiotalk
We’re doing well with the hashtagging tonight - just a gentle reminder to continue using #physiotalk in all your tweets #physiotalk #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Questioning the clinical reasoning, evidence base! If it’s a case of “we’ve always done it this way” arouses suspicion. #physiotalk
physiotalk @physiotalk
RT @ClaireC_PT: @physiotalk A lot of treatments or practice that we now consider ineffective seemed to make logical sense (e.g. Rx of trigg…
Sam Haworth-Booth @SamHBooth
@physiotalk Non compliance Aggressive or intensive rehab Not having perfect posture leads to low back pain #physiotalk
Ian Tyrrell @IanTyrrell5
@suzeeyre @physiotalk I’m good at that! People find me highly irritating! I take it as a compliment! 😀 #physiotalk
Action Potential @DNApolymers
@physiotalk Not sure about time will it take to implement, will I have to adjust my schedule #physiotalk #EBP
Anna Spiteri @spiteri_anna
@themcmullansNI Have to look at conflict of interest here. Is there a financial benefit to be gained. The NHS has strict disclosure criteria #physiotalk
physiotalk @physiotalk
Q3 coming up now... #physiotalk
Action Potential @DNApolymers
RT @ClaireC_PT: @physiotalk A lot of treatments or practice that we now consider ineffective seemed to make logical sense (e.g. Rx of trigg…
Action Potential @DNApolymers
RT @themcmullansNI: Q2 It doesn't work well, it lacks plausibility, it is marketed more than it is researched #physiotalk
Martyn Wyres @MartynWyres
@physiotalk If we are talking about identifying this in others then it is important to have a working environment where everyone can be respectfully and constructively challenged, no matter there role or banding. Supervision, peer learning, IST #physiotalk
Martin Billing @BillingMartin
@physiotalk #physiotalk how long you got?
Jo Kitchen @J0Physio
@KBNeuroinfo @physiotalk Right, yes I agree that’s a dangerous myth as it alters patient expeditions about what ‘good’ rehab might be #physiotalk
physiotalk @physiotalk
Q3 Why does misinformation seem to be shared more widely than EBP – or is it? #physiotalk
Andy @themcmullansNI
Q3 requires less effort #physiotalk
Aideen Larmer @AideenLarmer
Always suspicious when people's views aren't up for discussion/willing to be challenged... in my experience normally means that it's more an opinion mascaraing as a fact or a deeply held bias (that they maybe don't realise) #physiotalk
Lewis Lovegrove @_lewadam
@physiotalk Because some beliefs and perceptions are so engrained in society. #physiotalk
Matthew Breeze @MBreeze09
@physiotalk When other healthcare professionals direct patients to physio for everything but physio , acupuncture, massage, manual therapy! #physiotalk
physiotalk @physiotalk
@BillingMartin haha ok give us your top 3.. #physiotalk
Ieuan Ellis 🎓🏃🏴󠁧󠁢󠁷󠁬󠁳󠁿 🌍 💙 @Prof_IeuanEllis
'I just know' ....cause for concern over the evidence-base of practice ! #physiotalk https://t.co/EwBicCqPq2
Dale Whelehan @Daleyfurter
@physiotalk As a profession we aren't unified on accepting EBM as daily practice i.e difference in practice in private practitioners. In other HCP roles i.e pharmacological or surgical interventions everything HAS to be evidence-based as the risk of adverse events are much higher #physiotalk
Jo Kitchen @J0Physio
@physiotalk Twitter?! In particular about pain education I’ve been directed towards research/articles/blogs I wouldn’t usually #physiotalk
Action Potential @DNApolymers
@physiotalk because we take misinformation seriously nd wide spread of problem become someone opportunities #physiotalk #misinformation #EBP
Ellena @ellena_farrow
@physiotalk Is it? Or is it just the misinformation is much easier to remember than the EBP? Much like remembering the one bad thing someone did more than the 100 good things. #physiotalk
Martyn Wyres @MartynWyres
@physiotalk Because evidence based practice is sometime difficult to understand? Misinformation tends to be simpler, therefore people get it and pass it on. #physiotalk
Lewis Lovegrove @_lewadam
@physiotalk Obviously it depends what environment you are in. But in gyms for example, unless a large proportion of clientele are well read HCP's, I suspect the majority hold non-evidence based beliefs due to what they have read/heard on the internet etc #physiotalk
physiotalk @physiotalk
@_lewadam So people will share info that is in alignment with their health beliefs - how might people react when presented with info contrary to their beliefs? #physiotalk
Claire Campbell @ClaireC_PT
@physiotalk I think it’s really hard when a lot of treatments have mild to moderate effects to ‘sell’ that. Most of what people need to do is repetitive and common sense. Physios and patients can get distracted by the new, shiny thing that had really positive results once... #physiotalk
Suzanne Brown 💙 @suzeeyre
@IanTyrrell5 @physiotalk 🙈 Not you Ian! I get so sick of the misinformed nurses (that is a personal statement to my situation, not a generalisation) telling people to stay in bed until a physio can assess them 🙄 It’s actively disabling people! #physiotalk
Pamela Kipps Hansford @PamKHphysio
@physiotalk #physiotalk As a 'physio-as-rehab patient' I've been horrified that the myth that the physio knows best is still in evidence! And that stretching and strengthening is where neuro rehab begins and ends (rather than release and lengthening & a,b,c (alignment, balance and control)
Sam Haworth-Booth @SamHBooth
@physiotalk I don’t think we are good at translating it for patients so they feel like they are able to choose it, we don’t ‘market’ it in the same way, it is hard work and sometimes a bit scary for people. #physiotalk
Dale Whelehan @Daleyfurter
@physiotalk Also research in the area not as developed or standardised when compared to other medical/surgical interventions. Necessary to foster a culture of participating in meaningful research from UG teaching #physiotalk
Emily O’Driscoll @BucksPhys
@physiotalk It can look like the new best thing! New graphics allow information to look legitimate or better than before!! Gadgets and People looking for jazzy information. Lazy! #physiotalk
Suzanne Brown 💙 @suzeeyre
@physiotalk #physiotalk “He has no rehab potential because of his dementia” 😡 #stopgivinguponpeople
Claire Campbell @ClaireC_PT
@Daleyfurter @physiotalk #notallsurgery #notallpharma #physiotalk
Sam Haworth-Booth @SamHBooth
@physiotalk I also think we probably register mid-information as we find it irritating. The interesting thing is that we may not all agree that the same thing is misinformation #physiotalk
Aideen Larmer @AideenLarmer
The 'myth of myths' is that we know all the answers...that we have somehow 'arrived' at the peak of human science and that the answers we have are 'facts' Physio is a science- we don't have the all the answers- just our best attempt at them at any given moment in time #physiotalk
physiotalk @physiotalk
@Physio_myRehab Interesting, so someone is benefiting from misinformation - do you think the same benefits exist for those who generate/spread EBP? #physiotalk
physiotalk @physiotalk
@wolstenholme_d @CherylGrindell @AideenPhysio Embracing social media is a good start like we are all doing this evening. #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Creatures of habit! We learn by osmosis at times and it’s hard to challenge. Also looking at new area of practice or research is often difficult with huge clinical caseload hence status quo. Also we are poor at sharing what we do so loudest voices are heard. #physiotalk
Jo Kitchen @J0Physio
@physiotalk More difficult message to share. E.g. a variety of exercise helps LBP compared to core stabilty exercise helps LBP #physiotalk
physiotalk @physiotalk
@suzeeyre Sadly we've all heard this far too often #physiotalk
Lewis Lovegrove @_lewadam
@physiotalk The general public will share what they have been told, especially if such information is from HCP's, hence the multiplier effect of cementing such beliefs. I would hope the general public would take it on board alternate view points/evidence #physiotalk
Claire Campbell @ClaireC_PT
@Daleyfurter @physiotalk It’s really hard to get surgeons (as you gave that example) to stop doing interventions that are no longer supported by the evidence. People have spent a lot of time and money gaining the skills to perform X, they are not always going to let that go. #physiotalk
physiotalk @physiotalk
RT @Daleyfurter: @physiotalk Also research in the area not as developed or standardised when compared to other medical/surgical interventio…
physiotalk @physiotalk
@Daleyfurter Very valid point re research culture. We all can take part in generating more research and EBP #physiotalk
daniel wolstenholme @wolstenholme_d
@physiotalk @CherylGrindell @AideenPhysio There are definitely a few different stages, social media can raise awareness but active and collaborative processes needed to get knowledge into practice @CherylGrindell #physiotalk
Sam Haworth-Booth @SamHBooth
@physiotalk Does anyone think that the misinformation about physiotherapy approaches might also be coming from well meaning colleagues? #physiotalk
Jo Kitchen @J0Physio
@physiotalk Another example: Any competent HCP can get pt out of bed safely, but if the pt is more complex/struggles/pt fearful you could hoist to chair as sitting out important or ask Physio to assess. #physiotalk
Physiotherapy Association of Saint Lucia Inc. @pasl_pt
#physiotalk
physiotalk @physiotalk
RT @SamHBooth: @physiotalk Does anyone think that the misinformation about physiotherapy approaches might also be coming from well meaning…
Dale Whelehan @Daleyfurter
@ClaireC_PT @physiotalk Equally same concept applies in physio - attending training courses that cost so much etc. Saying that - I think laparoscopic surgery and the use of surgical simulators has brought surgical intervention forward a lot in recent years #physiotalk
M-J Sharp @hipsterlife85
@IanTyrrell5 @KBNeuroinfo @physiotalk And "gentle chest physio" ! #physiotalk
Aideen Larmer @AideenLarmer
@wolstenholme_d @CherylGrindell @physiotalk I think it's down to the mentality you enter the profession in - I was 'lucky' in some ways to be learning things in uni that were actually being debated/overturned in practice - made me realise that physio will always be changing and to get ready to keep learning! #physiotalk
Claire Campbell @ClaireC_PT
@physiotalk EBP also seems to be common sense? Not very complex or specialised. Don’t have the ref for this #physiotalk https://t.co/rm6p60IAn6
physiotalk @physiotalk
Q4 How can we become better at appraising the new (and relinquishing the outdated)? #physiotalk
Action Potential @DNApolymers
@SamHBooth @physiotalk missing electronic records, fake complain, false history taking, inappropriate assessment, insult & carelessness, cognitive bias, work pressures, misdiagnoses, prank exercise plan & competitive world leads to misinformation in physiotherapy #physiotalk #misinformation
Suzanne Brown 💙 @suzeeyre
@physiotalk And it’s hard to deal with when it comes from a professional working in a “specialist” area! Some patients/families will accept that; others fight for more. I would be a fighter for my family, so I like to think I do it for other people’s family 🤷🏻‍♀️ #physiotalk
physiotalk @physiotalk
@wolstenholme_d @CherylGrindell @AideenPhysio We think you should do a PhD on this subject @wolstenholme_d 😀#physiotalk

Sam Haworth-Booth @SamHBooth
@physiotalk Work on health literacy, get better at shared decision making, watch our language, be honest about our limitations, challenge misinformation in our colleagues (PT& non-PT) as much as we do patients. #physiotalk
Jane Manson @janeylew2
@physiotalk Everyone needs to be on board, EBP needs to be seen as less scary and inaccessible. Need a culture of improvement so that people are ready to accept change #physiotalk
Dale Whelehan @Daleyfurter
@physiotalk Again - build a research culture. Build capacity within the physiotherapy profession with regards to how to critically appraise journal articles and know which articles carry more impact (i.e peer review) #physiotalk

@SamHBooth @physiotalk Yes health literacy is a big one! Sometimes we are spreading myths that we don't even realise just because of misunderstandings - getting patients to explain it back can be a good tool #physiotalk

@physiotalk I think sometimes we lack critical evaluation skills, and also the ability to question our biases, so research is evaluated and we chose to believe research that supports our beliefs rather than question them. This means new ideas often get excluded #physiotalk

@ClaireC_PT @physiotalk Thanks 🙏 but I think many of my pieces may not have been well received. Need consumer input, not just academics naming facts. I think attempts to help the public better assess claims about tests and treatments has the most promise. We can’t control all info #physiotalk
