RT @physiotalk: Good evening and welcome to tonights #physiotalk on sleep - the forgotten factor in health?
physiotalk @physiotalk
We'll start as usual with saying hello - let us know who you are and where you are tweeting from! #physiotalk
Janet Thomas @JanetThomas47
Sitting on the @physiotalk account tonight from a not quite a sunny as it was Edinburgh #physiotalk
RunningPhysio @tomgoom
@physiotalk Good evening, Tom here tweeting from sunny Brighton #physiotalk
Helen Harte @Hartey_PT
@physiotalk Evening #physiotalk I'm Helen- postgrad student at Herts with an interest in pain, communication and leadership
physiotalk @physiotalk
@tomgoom Well my first fail - typing #physiotalk !!
Bhanu Ramaswamy OBE @BhanuPhysio
@physiotalk Just awoken from a kip and ready for #physiotalk @physiotalk 😉
Janet Thomas @JanetThomas47
@BhanuPhysio @physiotalk Or was it a power nap?? #physiotalk
physiotalk @physiotalk
Lets get going with our opening question #physiotalk
physiotalk @physiotalk
Do you consider sleep habits during a routine physiotherapy assessment? #physiotalk
Stephanie Mansell @skmansell
#physiotalk I'm here! with @lauren_ged
physiotalk @physiotalk
A reminder to all taking part in the chat to include the hashtag in all your tweets So that's #physiotalk in ALL your tweets tonight please
physiotalk @physiotalk
@skmansell @Lauren_ged Thanks so much for joining it - looking forwards to your contributions #physiotalk
Helen Harte @Hartey_PT
@physiotalk Yes when dealing with chronic pain issues. The psychological effects of sleep deprivation can be monumental. No if it's a ankle sprain #physiotalk
Janet Thomas @JanetThomas47
I must admit to not asking - unless someone specifically mentions an issue with sleep #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
@JanetThomas47 @physiotalk Heard it called all sorts like 40 winks, dovering (mum's fave term from a radio show), snooze, quick-drop-off etc #physiotalk
Stephanie Mansell @skmansell
I do, but i'm sleep & vent. Also really important for resp pts with BPD #physiotalk
RunningPhysio @tomgoom
@physiotalk Yes, I’ve made it a routine question as it influences so much! #physiotalk
physiotalk @physiotalk
@tomgoom So - because of what?? Linked to @Hartey_PainPT comment about not asking if Ax ank sprain - would you?? #physiotalk
physiotalk @physiotalk
@skmansell BPD?? Sorry (I can only think bipolar??) #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
#physiotalk Sleep is so important @physiotalk , and working with an older population, it scares me to see how many are put on 'sleeping tablets' and not reviewed. Huge risk factor to falls at night when knocked out but still needing toilet
Amy Black @amy_scotland
@physiotalk Yes very frequently in community paediatrics, especially in new patients under 1 #physiotalk
Pa Carroll @pacarroll_
@physiotalk Don’t consider it in every assessment but if I get an impression it may be relevant I ask about it. Ask more in persistent pain presentations but appreciate role in healing of more acute presentations #physiotalk
Stephanie Mansell @skmansell
@physiotalk @tomgoom @Hartey_PainPT dont you think it would give you osme idea of pain control? And guide your advice on pain mx even if acute? Prevent it becoming chronic right? #physiotalk
Stephanie Mansell @skmansell
@physiotalk #physiotalk Breathing pattern disorders @Lauren_ged is an expert!
physiotalk @physiotalk
@skmansell @Lauren_ged Aaah Ha! See - I'm certainly not a resp physio.... #physiotalk
Dale Whelehan @Daleyfurter
Won’t be joining tonight - but would definitely recommend reading “Why we sleep” by Matthew Walker - really made me realise the importance of sleep in conjunction with healthy eating/physical activity. Here’s a great summary! #physiotalk https://t.co/SJ6cDwdxME
physiotalk @physiotalk
Thank you! #physiotalk https://t.co/ixXFvhPZWP
Bhanu Ramaswamy OBE @BhanuPhysio
Coincidentally to this #physiotalk, I just happened to be reading the Matthew Walker book 'Why we sleep'. Some really interesting information and research into the science of sleep and dreams
Rebecca Scholes @becks09071
@physiotalk Can beat a good snooze #physiotalk
physiotalk @physiotalk
@BhanuPhysio I think its a must read from your comments and @Daleyfurter as well! #physiotalk
RunningPhysio @tomgoom
@physiotalk @Hartey_PainPT Sleep is our most effective form of recovery and an important coping strategy for both physical and emotional stresses. In athletes sleep extension improves performance and loss of sleep may be linked to injury #sleeptowin #physiotalk https://t.co/BpD7QouTdd
Stephanie Mansell @skmansell
@BhanuPhysio #physiotalk I'm part way through, i like to keep abreast of the media advice, there's been lots on the telly recently, which is good for raising awareness as some conditions massively under recognised
physiotalk @physiotalk
So - lets get a little more specific with our next question... #physiotalk
Helen Harte @Hartey_PT
@skmansell @physiotalk @tomgoom If improvement was slow, I would dig deeper and explore sleep. I guess it depends on patient presentation, expectation and presence of risk factors for chronicity. Obvs if patient raises during initial subjective, I would pursue questioning #physiotalk

Bhanu Ramaswamy OBE @BhanuPhysio
@Daleyfurter Shame you can't join in #physiotalk tonight @physiotalk as this is the book I am just reading now
Pa Carroll @pacarroll_
@tomgoom @physiotalk How do you phrase the question Tom? Are you looking for anything in particular or just a persons perception of whether their sleep quality / duration is sufficient? #physiotalk
Stephanie Mansell @skmansell
@becks09071 @physiotalk #physiotalk terrible sleep hygiene!
physiotalk @physiotalk

Stephanie Mansell @skmansell
@physiotalk #physiotalk so many things! I use a sleep disorders questionnaire is 12 pages long!
physiotalk @physiotalk
@skmansell Ahhh - that's quite a lot then! So you have a reference at all? #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
@skmansell I love the fact the author says it is the only time he would never be upset if the reader falls asleep when reading his stuff #physiotalk 🤣
Ramon Holland @RamonHolland
@physiotalk #physiotalk I find it useful to ask if they regularly dream, will give an understanding of duration and quality without directly asking them for hours slept
Pa Carroll @pacarroll_
@physiotalk Typically ask about duration of sleep and whether they have a consistent sleep / wake cycle #physiotalk
physiotalk @physiotalk
@RamonHolland That's a really interesting slant! Any one else about dreams? #physiotalk

Cathy Thomson @cathythomsonPT
@physiotalk Really nice blog I read earlier today re sleep questioning #physiotalk https://t.co/APr0vFF41h
Stephanie Mansell @skmansell
#physiotalk sleep wake cycle, sleep opportunity, sleep hygiene, evidnece of sleep medicine diorders
physiotalk @physiotalk
@pacarroll_ Anyone using wearables? I know I could use mine to look at sleep duration and depth but I don't really bother. Accurate? Useful? #physiotalk
RunningPhysio @tomgoom
@physiotalk I ask how many hours they typically sleep per night, if they feel refreshed in the morning or have episodes of sleepiness during the day. #physiotalk
physiotalk @physiotalk
@tomgoom Are you looking for a set amount of sleep - or perhaps more if it has changed recently? #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
@physiotalk #physiotalk Up until last year, it was a general question I have always asked, but in relationship to how fresh/rested the person feels so I can gauge their concentration levels for our session. I knew, but did not pay enough attention to health impact of lack of sleep.
Helen Harte @Hartey_PT
@physiotalk I ask about whether sleep is restorative, normal sleep habits, medication use, and (personally can't bear the term) sleep hygiene. #physiotalk
Janet Thomas @JanetThomas47
@BhanuPhysio @physiotalk I think that's where I'm at - if someone says they have had a poor night I think more about the session to come! I think tonight will be changing my Ax and thoughts...#physiotalk
Stephanie Mansell @skmansell
@physiotalk @pacarroll_ #physiotalk they're massively inaccurate, they tend to over estimate stage 3 and REM
physiotalk @physiotalk
@Hartey_PainPT Do you have a term you prefer? #physiotalk
Pa Carroll @pacarroll_
@physiotalk It’s not something I have delved into. Could be useful with the right patient group. Accessible tech that can help objectively measure is always worth considering. Anyone else use this? #physiotalk

Stephanie Mansell @skmansell
@physiotalk @RamonHolland do they act out their dreams? Are they nightmare/night terrors? #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
@physiotalk #physiotalk Now however, I try and discuss their sleep patterns, what wakes/ keeps them awake, if on sleeping tablets, ask when these were reviewed and what started for. BUT I don't always refer them back somewhere - I suggest they seek a GP review. Need to be more proactive
Helen Harte @Hartey_PT
@physiotalk I Just talk about habits. For me "hygiene" is too ambiguous and jargon-y #physiotalk
physiotalk @physiotalk
@skmansell @pacarroll_ Thanks - I did wonder (which is probably why I've never really looked at mine!) #physiotalk
Stephanie Mansell @skmansell
@pacarroll_ @physiotalk we use the actiwatch over two weeks with a sleep diary (the one from the ACSM, no point reinventing the wheel) #physiotalk
RunningPhysio @tomgoom
@physiotalk Most papers I’ve read suggest 7 to 9 hours per night. More when increasing training. #physiotalk
Ramon Holland @RamonHolland
@skmansell @physiotalk #physiotalk no not in the slightest, not in the popn I work with. As dream state only occurring in certain phase of sleep cycle, it provides info as to sleep timing and duration/quality. Helps get accuracy of info given and not answer ‘I want to hear’ from my players
Bhanu Ramaswamy OBE @BhanuPhysio
@physiotalk It's good to see some nice info graphics on #physiotalk sleep benefits https://t.co/gu5sg8QhIb

RT @BhanuPhysio: @physiotalk It's good to see some nice info graphics on #physiotalk sleep benefits https://t.co/gu5sg8QhIb
physiotalk @physiotalk
Onto our next question - getting a little more specific here... #physiotalk
physiotalk @physiotalk
Pain and sleep - what do you understand about the link between the two #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
#physiotalk @physiotalk and there's more..... https://t.co/lQmHJQqqvU

physiotalk @physiotalk
RT @BhanuPhysio: #physiotalk @physiotalk and there's more..... https://t.co/lQmHJQqqvU
Stephanie Mansell @skmansell
@physiotalk @pacarroll_ #physiotalk it measures light levels & activity levels so has been used in clinical practice for sleep clinics but also research for activity levels
Janet Thomas @JanetThomas47
@BhanuPhysio @physiotalk How do you know how many dream per night you have had! I cant remember mine... #physiotalk
physiotalk @physiotalk
RT @physiotalk: Pain and sleep - what do you understand about the link between the two #physiotalk
Stephanie Mansell @skmansell
@JanetThomas47 @BhanuPhysio @physiotalk #physiotalk theres not a normal number of dreams, but there is a rough normal percentage of REM sleep which is when dreams occur, thats usually after about the first 90 mins and are more frequent in the early hours
physiotalk @physiotalk
I think we all recognise the pain means less sleep but I was struck when preparing for tonight about the chronic sleep debt for those in persistent pain #physiotalk
Stephanie Mansell @skmansell
@physiotalk #physiotalk and the problem is that the meds affect sleep quality and can interfer with sleep patterns/stages as well as leaving nasty side effects. Pain meds can also induce central sleep apnoeas and nocturnal hypoventilation

@physiotalk Pain can limit/reduce sleep & lack of sleep can reduce tolerance to pain #physiotalk
Bhanu Ramaswamy OBE @BhanuPhysio
@JanetThomas47 @physiotalk #physiotalk I agree - There was a huge fad a decade or so ago to keep paper and pen by the bed and write your dreams as soon as you awoke; I remember dreaming, but rarely remember details or the colours
Helen Harte @Hartey_PT
@physiotalk Thank you! I forgot.... 🙈 #physiotalk
physiotalk @physiotalk
@Hartey_PainPT Better quality sleep? #physiotalk
Emmanuel J Ng'wiza @ENgwiza
@physiotalk Sometimes pain affects the quality of sleep. #physiotalk
Pa Carroll @pacarroll_
@Hartey_PainPT @physiotalk Agreed...what kind of strategies do you use to try and facilitate better sleep quality? An area I feel quite limited in once I have given basic advice #physiotalk
Helen Harte @Hartey_PT
@physiotalk That's the goal #physiotalk
physiotalk @physiotalk
@Hartey_PainPT So - what is better quality sleep? More rested / refreshed? #physiotalk
Stephanie Mansell @skmansell
@BhanuPhysio @JanetThomas47 @physiotalk #physiotalk REM sleep is where your brain does it's processing and transfer of short term to long term memory There's not much need to interpret your dreams clinically unless you are acting them out (REM behaviour disorder) or nightmares (parasomnias)
Janet Thomas @JanetThomas47
Just musing... Any thoughts about sleep walking in our patients... #physiotalk
Helen Harte @Hartey_PT
@physiotalk For my population, yes- more able to engage in their social world. That would mean increase in refreshing sleep, not merely hours spent in bed #physiotalk

@physiotalk #physiotalk Subjective reporting that the person slept better for advice e.g. Change in exercise habits, sleep hygiene or sometimes routine; or equipment so they get out of bed easier to get to bathroom so less disrupted if previously the struggle to move woke them

@tomgoom @physiotalk @pacarroll_ @Hartey_PainPT #physiotalk I often advise the person keeps 2 or 3 different sized pillows to hand e.g. For under knees, under a shoulder, to prop their back - doesn't matter where as long as they are comfortable position to allow sleep. They can move them as necessary during the night
