@from_bedside A1 To be honest I don't know if we consider it open or not or what our actual policy is or how new it is. We say 2 visitors and try to limit to no visitors during shift change for 1 hr (ie 7-8am). Sign outside unit. All up to bedside nurse discretion though. #TweetRN

@from_bedside A1: When I was in ICU, last in 2017, we did NOT have open visitation and visitation hours and rules were clearly posted everywhere. Families were asked to step out during rounding/reporting, and during ADLs, baths, and things like catheter changes. #TweetRN

@from_bedside A1: in the ICU's we limit to 4 visitors during the day 0700-2000 roughly. But after 8 only 1 person allowed in room overnight. No kids under 14 in the ICU's. Flu restrictions are different and in the case of someone passing exceptions are always made. #tweetrn

@Bacon_n_Leggs Yep open visitation takes a lot of education on the nurses part for sure and when it is harming the patient or ability for nurse to provide care (which is harm to the patient, in my opinion) it can be frustrating to enforce. #TweetRN

@from_bedside A2: We were able to have a doctor's order limiting visitation, but the nurse wasn't "allowed" to do it without the order. Which, by the way, was extremely frustrating because let's be real here... who's really spending time in the situation? #TweetRN

@from_bedside A2 part 2: I will say that having visitors at the bedside was beneficial for the nurse at times because there would be another set of eyes on the patient, or they'd assist with things like feeding. #TweetRN

@reepRN A1 We spent much time and education on how to navigate open visiting. We did not have any issues for the most part making sure patient and family understood the guidelines.Our physicians were involved in this process.#TweetRN

@from_bedside In neuro if our patients need sensory limitation I'll ask the family to be quiet and if they can't follow the rules they are asked to leave. As I mentioned before comfort care patients all rules are pretty much gone as many visitors that can safely fit in the room #tweetrn

If you clear up expectations and boundaries early, then easier to implement and handle. I just have never worked somewhere that had consistency amongst staff regarding visitation. #TweetRN

@from_bedside A2P2: RNs have the ability. I actually had to kick someone out for disturbance and for not allowing a healing environment for my patient. (Verbally abusive) I asked him to leave the room and I expressed my concerns to the patient. He was then asked to leave. #tweetRN

@LttleMouse93 @OnlyintheICU @from_bedside #TweetRN We felt it was citical in the ICUs when possible. Patients are so over stimulated. We also tried to have all patients sleep for at least 5 hours at night uninterrupted in the ICU and at least 6 on med-surg.

@from_bedside A3 Too many visitors came get in the way in rooms and interfere with pt care, visitors can keep patients awake who need sleep esp. if they are being awoken q1hr, in neuro can be ICP issues. At my hospital any visitor issues we call Security to handle the situation #tweetrn

@from_bedside A3 It can be a lot of workload to answer the door (we're a locked unit), see who they want to visit, contact nurse to see whether ok to let visitors in. Sometimes we have a unit clerk sitting at front but they are doing other things sometimes and we don't always have one #TweetRN

@from_bedside A3 Sometimes family can be disruptive to plan of care or nurse workflow. Really disruptive or abusive visitors, we can call different behavioral codes overhead to help deescalate. If really abusive (ie threatening staff) they can be banned from the hospital #TweetRN

@cjchiu @ETSshow @from_bedside The whole newborn nursery thing is bizzare. My oldest was in the NICU for 2 days. And there were all these strict rules. Then suddenly he didn’t need the NICU and they just handed him to us and let us drive off and go home. It was jarring. #tweetrn

@from_bedside Our hospital will bring charge/manager to discuss repeated disruptive behavior. If bahaviot continues SW will have visitor sign social contract outlining expectations for behavior with clear repercussions if outlined behavior is not followed. #a3 #tweetRN

@from_bedside A3: One word: SECURITY! Our security officers were a WONDERFUL resource, and were great at de-escalating situations as well. They'd often be able to avoid having to toss visitors out altogether, thus upsetting the patient less. #TweetRN

@from_bedside Q2: Family is helpful to my patients where English is not their first language. Families benefit from seeing how well the patient may not be progressing, for example, with PT. I'm thankful for families that are helpful in de-escalating cognitively impaired patients. #TweetRN

@from_bedside We had an elderly spouse of a patient that was incredibly unsteady on his feet. He would not listen to pleas for him not to attempt to walk around the dark room (he was staying overnight). It was like taking care of an additional patient. He eventually fell in the room. #TweetRN

@from_bedside A4 We've had issue lately w/ staff telling family they cannot stay overnight. This was supported by one of our managers. Then director got wind of it and emphasized evidence of open visitation. It's up in the air now with last recommendation for unit council to address #TweetRN

@from_bedside A1: visitation in dialysis unit is somewhat prohibited (some can get away with it).. our patients would come in for their tx for about 2-4 hours at a time, & we usually just ask family members to either wait in the patient’s room or the waiting room across our unit. #TweetRN

@from_bedside A4: Most of the families I deal with want to speak with a doctor. Open visitation meets that need. However, families tend to forget I have other patients to look after & can be demanding with a delay in treatment, i.e. toileting, pain meds. #TweetRN

@from_bedside A2: Neuro it’s a mixed bag for pt’s imho families can help encourage, improve compliance w/ exams etc, but often over stim or frustrate pt & place excess demands on RN. Allows 4 Edu of fam, reduce fear 4 families. RNs May limit visitors for care/procedure reasons. #TweetRN

@from_bedside A4: We have daily CQI huddles...Continuous Quality improvement. It allows staff to identify unit specific issues and provide solutions. Some solutions can be solved by staff, others go to senior leadership. #TweetRN

@from_bedside A4: Being a mom having a child currently in the hospital/PICU, & still work as an RN, I understand how a fam member would feel when certain tx/procedures are being done. I support PFCC & this way pts & fams are more involved in their care for a better outcome! 🙌🏼 #TweetRN
