@from_bedside @NorthernCall19 Hey-o! 👋🏻👋🏻👋🏻 Trauma ICU nurse here! 👍🏻 #TweetRN

@from_bedside @NorthernCall19 Hey! NP in advanced heart failure but with a RN background in trauma ICU. Love the nursing profession and how dynamic it is! #TweetRN

Join me!
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside A1: I currently work in CVICU and SOOOO much is outside of our pts’ (and our) control; unfortunately, the way we conduct ourselves by asserting our role as the one in charge (not all ICU nurses are like this, but many of us are) can compound this. #tweetrn
Kirstin Manges Piazza PhD RN @kirstin_manges
@from_bedside A1: This isn’t my area of expertise, so I look forward from learning from everyone! My intuition is that it can very by individual: depression, sadness, anger, mood swings, hypervigilance, anxiety, tearful #tweetrn
People Are Wild Podcast! 😷 @peoplearewild
@from_bedside A1: I'm from the land of the ER. I'm seeing people on a horrible day for them. They are hearing things in other rooms that are concerning or escalating. Sometimes I need to remember that while I'm surrounded by chaos, most people are not. #tweetRN
Justin @NurseJustinRN
@from_bedside A1: There are numerous ways to retraumatize someone in healthcare, including peer bullying and workplace violence. Outward signs are myriad and can include anything from severe anxiety, stress reactions, compassion fatigue, and overall burnout #TweetRN
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside So much agency is taken away by the nature of the situation, even if people aren’t sedated and/or paralyzed. My unit also has high turnover & lower morale; staff burnout-related mannerisms/behavior probably triggers traumatic memories, too :/ #tweetrn
Justin @NurseJustinRN
@peoplearewild @from_bedside Yes and it's scary for those who are not accustomed to it. Great observation! #TweetRN

@from_bedside A2: Not to sound crass but I'm going to, in a vast majority of the ERs I've worked at, they got jack crap for any interventions. #tweetRN
Ashley Blackmon @ABlackmonAPRN
@from_bedside A1, for the nurse it can be seeing a pt experiencing the same thing they had. For a pt can be incorrect verbiage or a too-firm/aggressive physical exam. Easy to get caught up in routine and forget to slow down for the pts sometimes. #TweetRN
Justin @NurseJustinRN
@from_bedside A2: I've seen a ton of education related to this topic in patient care settings. In home care, going into someone's home who has been traumatized or is currently being traumatized takes an awful lot of education and skill to handle the situation #TweetRN
Justin @NurseJustinRN
@NurseGnome @from_bedside I'm so sorry! I can't imagine how traumatizing our jobs can sometimes be. Just the pressure alone can trigger moments of anxiety. #TweetRN
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside A2: as much as possible, I try to tell my sedated pts what I’m going to do and what they’ll feel before I do it. Idk how much they process, but I want to try. I also try to ask awake pts before touching them/doing an intervention, and offer choices about timing cares. #tweetrn
Ashley Blackmon @ABlackmonAPRN
@from_bedside A2: no but I think we should! My pts have a lot of chronic dz and are typically end-of-life. Addressing trauma would be an important factor in holistic tx/healing. #TweetRN
Justin @NurseJustinRN
@NurseGnome @from_bedside Great way to provide the foundation of trauma-informed bedside care! #TweetRN
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside I think it’s important to inject as much agency as possible & opportunity for maintaining/redeveloping self-efficacy. These are both critical elements of trauma recovery, and have been shown to have some protective fx against PTSD after traumatic experiences! #tweetrn

@from_bedside A3: I tend to explain to patients and their family members that they might hear a lot of things happening if we know a trauma or code is coming in. I also make sure to follow up with patients and family members when we've had something happen in the department. #tweetRN
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside A3: esp in ICU, I think protecting sleep & facilitating sleep hygiene whenever possible is an oft-overlooked component of making environments safe for pts with hidden traumas. #tweetrn
Hui-wen (Alina) Sato @alinasato
@from_bedside A1: Once had a pt to whom we were going to give rectal Tylenol. Caregiver hesitatingly pulled me aside + sadly asked us to find another option bc the child had a history of sexual abuse. Signs of trauma: What may seem like unreasonably strong rxns to something minor. #TweetRN
Ashley Blackmon @ABlackmonAPRN
@from_bedside A3: TI training is awesome. Formal/ informal (Googling) helps us be aware of our words and actions, as well as what signs to look for. I never intentionally want to offend or turn-off a pt, but many things I say or do may do just that! Awareness is the 1st step! #TweetRN
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside A3 for colleagues, I think spending time together outside of work/getting to know people as people! Trauma erodes ability to trust; building rapport is essential to building trust. #tweetrn
Hui-wen (Alina) Sato @alinasato
@from_bedside A3: Explaining in specifics (as much as possible) everything we are going to do *before* we do it and why, not when we're already in the middle of it and just saying "It's ok! You're ok! Just relax!" #TweetRN
Catherine Gegaris @cgegaris1
@from_bedside Catherine here,Informed Trauma care is alot about sensitivity during that fast moving golden hour of trauma care.I don't know how many hospitals do training for staff. Should be a Chaplain and grief/social worker present.#TweetRN
Justin @NurseJustinRN
@from_bedside A3: Organizations should provide confidential spaces for patients and their families to engage with providers. Also, include training in their improvement projects. It wouldn't hurt to provide signage that acknowledges the human response to hospital visits and illness #TweetRN

@versedbarbie @reepRN @from_bedside Intrusive thought. One of my biggest fears about the surgery was that if I had any complications & needed to be intubated, that I would *remember* the intubation (given hx of rape). It was hard to discuss w/ surgeon (shame, stigma); I worry for pts sometimes too 2/2 #tweetrn

Justin @NurseJustinRN
@ABlackmonAPRN @ArianaMasGrande @from_bedside How calming and soothing. Just think how patient outcomes, experience, and engagement would look. When we quell fears, we practice a form of medicine that is essential and according to Maslow's vital to our lives #TweetRN
Justin @NurseJustinRN
@NurseGnome @from_bedside It sure is and sometimes it is the best not only for you but for the patients you serve. We can always serve patients better when we are spiritually intact. #TweetRN

@ArianaMasGrande @from_bedside Yet on its own stands alone as a respectable and vital scientific discipline with its own growing body of knowledge specific to the study of the human response to caregiving #TweetRN
Jillian (she/hers), RN, CCRN @NurseGnome
@from_bedside A4: I have the easiest time with empowerment since coaching/cheerleading come naturally to me; I have the hardest time right now with trustworthiness (I’m young & look it, I’m wrestling early burnout & work environment is pretty toxic). #tweetrn
Justin @NurseJustinRN
@ABlackmonAPRN @ArianaMasGrande @from_bedside And it is so important. Nurses never forget this concept and integrate it into all aspects of our interaction with patients. If only we could get executive leadership to understand it the way we so intimately do #TweetRN
Ashley Blackmon @ABlackmonAPRN
@from_bedside I was surprised how often I saw it in family practice. But pts need PCP relationships so badly that all kinds of things would surface. #TweetRN
Justin @NurseJustinRN
@from_bedside I'm also curious to understand the cultural responses to trauma. How might we be informed better here? I know that culturally, we all express our effects of trauma outwardly in different ways. Which means we might frequently miss the signs depending on culture #TweetRN
Justin @NurseJustinRN
@NurseGnome @from_bedside I remember when patients treated me standoffishly when I. was young. It was insulting then. Now I'd love to hear that again, but I completely understand that phase of nursing. #TweetRN

@from_bedside A1. Some outward signs easily started or withdrawn, hyper alertness. Impulsively, withdrawn lack/excessive of appropriate emotion to event. Reassure them they are in a safe space if able move them away from current event that might be triggering their trauma reaction. #TweetRN
Justin @NurseJustinRN
@from_bedside Great point! I think there should be extensive education. I'd love to learn more about the cultures that surround me. Living in a rural area my entire life, I've had to learn more about culture because of the growth of the community which includes cultural growth #TweetRN
Justin @NurseJustinRN
@ResusTonight @from_bedside Incredible. Sometimes necessary for safety, but I've seen it done before, and it breaks my heart every time. It seems so demeaning. #TweetRN

@from_bedside A2: I would love to use them but atm haven’t really been trained in them & have to defere to experiences RNs/Chaplin’s to do that work. It’s an excellent thing to add. RNs are 4 holistic care of pts/families. Trauma permanently changes people & affects generations. #TweetRN
Justin @NurseJustinRN
This is excellent! Take note if you apply 4-point restraints regularly in clinical practice #TweetRN

@from_bedside A3. Communication communication communication....talk, tell people what you are doing why you are doing it, close doors, protect privacy/modesty as much as possible, use people pronouns, use their chosen name, ask before you touch them. Use common language to explain #tweetRN
