@AgencyNurse We are supposed to check items because it is a patient safety procedure. Manily blood, IV's, CD's and all sorts of patient checks in theatre like consent forms and allergies. #WeNurses

@WeNurses Having a second person to check some thing should reduce the risk of mistakes. Also if you have to find a second person to check something you are more likely to do a proper check. It could be too easy for some people to be complacent by checking on their own #WeNurses

@AgencyNurse I think this is where the benefit of double checking needs to be balanced against the danger of patients waiting too long. It probably is different for each area. #WeNurses

@AgencyNurse Yes, I think we do. I think as a profession we have it drummed into us that double checking is better, so people assume it is. I do think double checking is good myself, but only as long as both people are paying attention. #WeNurses

@WeNurses Finding someone free to do the double check. Some people dont fully check as they think the other person is checking. Where i work even if double check only one person signs chart so no proof you had it checked unless its a l cd. #wenurses

@WeNurses In some areas finding someone to do the double check. Need to ensure the double check is done properly, not one person half looking at the drug/blood #WeNurses

@WeNurses It is due to safety but some evidence shows that actually it decreases the quality of the check as you subconsciously rely on the other person. It only works of you each check properly #WeNurses

@WeNurses When I am doing something that needs a double check I generally check the item twice or even three times myself before approaching someone else for the double check But as getting things right is important I wouldn't want to eliminate the second checker #WeNurses

@nursingPUNC18 @AgencyNurse Where i work they have trained hcas to be 2nd checkers of medications. This has helped woth patients wait time. But on occasion i have had “lazy” RGNs say get a haca checker when i haved for a 2nd check even though hcas all busy but said RGN not at the time #WeNurses

@WeNurses Good question, there is a difference between 2 independent checks, and 1 check done by 2 people. It should be 2 independent checks I think #WeNurses

@CalvinMoorley @AgencyNurse But we single check a lot of oral drugs which can be just as problematic if given wrongly, not sure why iv drugs are given a higher status in terms of drug errors and therefore are double checked #WeNurses

@alimully @CalvinMoorley @AgencyNurse I think it is the higher likelihood of quick adverse reactions with IV drugs that calls for the double checking. #WeNurses

@faebeth @WeNurses Very true but what are you double checking, the prescription? the dose? the indication? the patient id? Very difficult to do 2 totally independent checks of all these things #WeNurses

The problem with Healthcare? We stop doing something due to evidence, so did we bring it back because of evidence or as a knee jerk reaction to errors? #WeNurses

#WeNurses, in the case of CD's it would mean that the checks are done at the bedside as they should be. In other cases we should remain vigilant and attentive to the process, no distractions.

@alimully @WeNurses I think it depends on what/who you are checking. We have 3 totally independent checks when we check a patient into theatre #WeNurses

Local hospital nurses can give pre filled clexane without it being checked but hospice in same county under same formulary and ccg has to have them checked but no evidence to why. #wenurses

@WeNurses Would depend why they were wrong, we had an incident with clindamycin years ago the vials were poorly labelled which meant the patient got the wrong dose on several occasions - drug company changed the labelling but goes to show errors do happen even with 2 #WeNurses

@maxwele2 @AgencyNurse I think there is something to this...lot of stuff about the distractions/interruptions that contribute, which I agree with, but not the process of 2 RNs & the relational aspects of second checking. We ‘independent check’ all meds but don’t know effect in reducing error #WeNurses

@alimully @WeNurses That can be an issue, sometimes vials can be very similar as well, particularly if the boxes change. #WeNurses

@WeNurses Most evidence is based on medication incidents, so difficult to interpret. The culture needs to be safe - you need to have time to do whatever checks you need to make - the evidence does show the less staff you have the more errors that happen #WeNurses
