Roxana Daneshjou MD/PhD @RoxanaDaneshjou
What a great paper @AMostaghimi - Pyoderma gangrenosum is such a difficult entity; it was great to see a summary of data from this many cases. Was there a particular question you were seeking to answer? #dermjc

In terms of the younger patients w/ IBD what was the breakdown for CD/UC was one more common? And Were most of them controlled or poorly controlled with their IBD? @AMostaghimi @DanielButlerMD #dermjc
Roxana Daneshjou MD/PhD @RoxanaDaneshjou
Note that this paper pointed out the importance of doing the skin biopsy when worried about pyoderma gangrenosum (instead of worrying about the pathergy). #dermjc

@RoxanaDaneshjou Biopsy is important to rule out other diseases. In other work we've evaluated risk of pg after procedures. Low but not 0: 5.5% of procedures, 15.1% of patients. https://t.co/Z5yg8GDcSz #dermjc
Roxana Daneshjou MD/PhD @RoxanaDaneshjou
@DrPatrickRush Yes! 28.1% of PG patients in this paper had pathergy. I'm going to quote that in the future. #dermjc

#dermjc I couldn't tell if most of these patients got their PG diagnosis as an inpatient, or were admitted for complications of PG? Some of the worst PG patients I helped take care of on consults were well known and frequent flyers
Dr. Carolyn Lee, MD/PhD @CarolynLeeMDPhD
@RoxanaDaneshjou I agree - pathergy occured less often than I expected! #dermjc

A question about the cancer diagnoses. Were a lot of these historical and taken out of the PMH section of the chart? If so, the older patients would have had more time to accrue them, hence the association with age #dermjc
Jules Lipoff, MD @juleslipoff
Hey @AMostaghimi - maybe this is a silly question, but why did you pick 65 years old specifically as the demarcation between older and the rest? Is that a standard split? #dermjc

#dermjc cutaneous crohns can often be mistaken for PG - and getting Crohn's under control is key in those cases. In cases where cutaneous crohns was the comorbidity, was PG still the appropriate diagnosis?
Arash Mostaghimi @AMostaghimi
@juleslipoff Hi @juleslipoff it's a great question. We did a sensitivity analysis showing that the association stayed the same whether it was 55, 65, or 75. Chose 65 as standard cutoff for older folks in US. #dermjc

@RoxanaDaneshjou @AMostaghimi @MishaRosenbach @DrMeganNoe @DanielButlerMD Yeah, the low percentage of pathergy, as well as the stark age difference in the associated inflammatory diseases. I would have liked to see them touch on the type of IBD (CD/UC) &any relationship between well or poor symptomatic control of their IBD #dermjc

@aaron_drucker Hi @aaron_drucker spoken like a true epidemiologist. The cancer data was for either known active cancer or cancer dx in context of w/u for #pyoderma. UR correct that somebody may have had MGUS or something similar for a while beforehand so misattribution bias possible. #dermjc

@DrPatrickRush @RoxanaDaneshjou @MishaRosenbach @DrMeganNoe @DanielButlerMD Hi @DrPatrickRush we couldn't correct for active versus not active IBD given retrospective study, but we did see that Crohns > UC in our population consistently across age. #dermjc https://t.co/2EG3Gook0S

Curious how people fall along lumper-splitter dichotomy when it comes to neutrophilic dermatoses - how much do we really believe that Sweet's is a completely different disease from PG, or are they different aspects of a continuum? How do Sweet's associations vary by age? #dermjc

@CarolynLeeMDPhD Hi @CarolynLeeMDPhD prednisone is fast, easy, and cheap but fantastic RCT from @CebdNottm of cyclosporine versus pred suggests that both good first line options for #pydoerma. https://t.co/ANQletJqQh #dermjc
Arash Mostaghimi @AMostaghimi
@leoshmu Hi @leoshmu this can definitely be challenging to differentiate, but if I recall correctly I don't think any of our cases had cutaneous crohns + PG. If there were some it wasn't a large enough number to move results. #dermjc
Arash Mostaghimi @AMostaghimi
@DrPatrickRush @lisa_iannattone Hi @lisa_iannattone I am obliged to answer my old fellow's questions ;-). We didn't look at activity in this study but definitely occult disease fairly common--I have dx 3 in last 6 months alone. I'd scope even if ROS negative. #dermjc

What is the role of topical steroids and/or topical steroid-sparing agents in treating PG? I inherited a patient with a <1.5 cm bx-proven PG and was not impressed by the response to topicals; however, it zipped right up with prednisone! #dermjc

And now it's all on twitter... :) Will be posting impression stats in the future to show just how far each tweet reaches! #dermjc
