@RoxanaDaneshjou I suggest my patients at high risk for #melanoma take #vitD. I’ve mentioned nicotinamide to some, but am now thinking of discussing this more regularly for patients with a history of multiple nonmelanoma #skincancers. #DermJC
Roxana Daneshjou MD/PhD @RoxanaDaneshjou
@HarkerDavid I agree that patients on many medications usually don't want to add another. I also have patients who are actually wearing sunscreen and want to know what more they can do. Nicotinamide appears to have a good side effect profile and it's over the counter. #dermjc
Allison Larson, MD @AllisonLarsonMD
@HarkerDavid @RoxanaDaneshjou And the issue with both of these is that the benefit goes away when patients stop taking them, so essentially it’s treatment for life, which many are less inclined to do #DermJC

This is one of the central reasons why I'm hesitant in many elderly patients, particularly those with only a limited skin cancer history. They already take a fist full of pills. #dermJC https://t.co/f8Q6d8Nmh9

@RoxanaDaneshjou I do. The results are consistent across BCCs, SCCs, and AKs. The study looked well-constructed to me and they cite basic science articles that posit a possible mechanism #DermJC

@AllisonLarsonMD The one benefit of nicotinamide to me is if the patient is willing to take it, it has a low side effect profile and is to get over the counter. So it's worth the possible benefits, which appear to be real. #dermjc

@gooserrrrr @HarkerDavid I've seen the flushing issue more with niacin, but not nicotinamide 500 mg BID. Has anyone had issues with flushing with that dose? #dermjc

I don't have a strong reason not to, but the typical patient I see also doesn't have an average of 8 skin cancers in the previous 5 years either. #dermJC https://t.co/AkctxGqRfK

@AdeAdamson I love learning about the different populations that people see. Because I have, on several occasions, biopsied 3+ lesions in one visit - all came back as NMSC. My current biopsy record was 7 lesions (and he had more that we didn't biopsy), all which came back as NMSC! #dermjc

Do you use nicotinamide in any other way in your dermatology practice? We also use it in our bullous disease clinic (in addition to other traditional therapies). #dermjc

@RoxanaDaneshjou @AdeAdamson I have a subset of patients who develop #skincancers regularly, like the light-skinned folks who had 20 years of PUVA for their psoriasis. Based on this paper, I’m going to think more about suggesting nicotinamide to repeat SBCC formers #DermJC

@AdeAdamson ... But niacinamide isn't a sunscreen; it's chemoprophylaxis. I'm missing the sunscreen part. #dermjc
Ade Adamson, MD MPP @AdeAdamson
I'm very wary of recommending vitamins for most things because history has shown they fail in so many clinical trials #dermjc
Allison Larson, MD @AllisonLarsonMD
@AdeAdamson Nope. I can’t tell what’s in this, I don’t think it could possibly protect the skin as effectively as sunscreen, and I suspect adherence will be better with sunscreen than a drink. Plus I’m not a fan of sugared beverages #DermJC
Roxana Daneshjou MD/PhD @RoxanaDaneshjou
@AdeAdamson I feel like calling it "sun protection" is false advertising! What if people start using this and then decide to forgo sunscreen? Also, seems like it would be expensive! #dermjc
Laura J Gardner, MD, MS @ljgardn
@AdeAdamson Definitely not if it tastes like sunscreen... #dermjc
Ade Adamson, MD MPP @AdeAdamson
@RoxanaDaneshjou I think UVO is bogus as a "sunscreen/" I was just saying you could drink UVO as your niacinamide supplement every day. #dermJC

@AdeAdamson This is the problem in dermatology - people creating really expensive products when there are cheap alternatives on the market that are more clinically effective. I imagine OTC niacinamide pills are much cheaper than this "hyped" drink. #dermjc

Allison Larson, MD @AllisonLarsonMD
Is anyone in the #derm community using polypodium leucotomos extract in patients to try and prevent sun damage? #dermjc

No experience with heliocare. I'd put it in the same category with UVO. I don't believe it. #dermJC https://t.co/MrDgXIKCp4

Roxana Daneshjou MD/PhD @RoxanaDaneshjou
@ShervinSays @AdeAdamson Unfortunately important to bear in mind as well that all OTC supplements are not regulated by the FDA, so can't even be 100% certain that the nicotinamide pills your patients get have the proper amount of vitamin. #dermjc
Ade Adamson, MD MPP @AdeAdamson
The issue is target population. I think the benefits of pills would be marginal at best for most patients with average risk. Pills likely better than nothing, and low side effects as you mentioned. #dermJC https://t.co/yJkXX2xvyc

Ade Adamson, MD MPP @AdeAdamson
I believe @gooserrrrr brought this up with heliocare. I think its bogus. The purported mechanism involves believing that ingesting an extract that a plant uses for photo protection somehow does the same for human skin. Really? #dermJC https://t.co/qNfXwX0NIw

@RoxanaDaneshjou Hmmm. I’ve noticed some people love the idea of vitamins- I could see it possibly being easier for certain high risk patients who aren’t great at sunscreen but already take vitamins #dermjc
Roxana Daneshjou MD/PhD @RoxanaDaneshjou
@AdeAdamson @gooserrrrr This is the beauty of evidence-based medicine to me. I feel like dermatology in particular is prone to many OTC "magic bullets" (many expensive), and I hope we can use #dermjc to discuss what's evidence based and what's not (and learn from one another!).
Allison Larson, MD @AllisonLarsonMD
@AdeAdamson @gooserrrrr I suspect the same. I’ve had patients ask about it. I don’t know of any #dermatologists that recommend it at this point #dermjc
Ade Adamson, MD MPP @AdeAdamson
The evidence that sunscreen reduces risk of skin cancer is a mixed bag by the way. That is likely a discussion for another day. #dermjc
