Chelsea Friend @kiwifreund T1: Mental healthcare and physical healthcare are intertwined, not inseparable. Sometimes symptoms are attributed to physical imbalance, but they are actually caused by mental health imbalances that are not being addressed. #IrishMed
ElizabethKelly, Ph.D @Elizabe85727641 T1 MH impt in primary care because mind & body are connected, bodily systems work best when in a state of homeostasis. #irishmed

Marie Scully @MarieOScully Hi. GP in Navan dropping in. #irishmed
Rie Lopez, MPH (she/her) @RieOfLetters T1: Primary care is often the first contact point for an individual struggling with #mentalhealth. In the US, the primary care provider is the one who issues referrals to a specialist, if needed, for insurance purposes. #irishmed
Barby Ingle Official @BarbyIngle Great minds.... !!!!!! #irishmed https://t.co/5BdUCA5cnw
Matthew Loxton is also on Mastodon @mloxton Missing #IrishMed bc fixing spreadsheets Bah Humbug
Marie Scully @MarieOScully T1. We see LOADS of mental health issues in primary care - and deal with most of it ourselves. So it’s v important to be reasonably good at dealing with it. #irishmed

Triona Murphy @Murpht01 #irishmed Evening all! Will be doing mostly listening tonight! Wondering are there any examples of Mental Health care at primary level though? Have personal experience of trying to access care and facing long long delays..unless you access via the voluntary sector!
Chuck Webster MD, MS AI, MS Systems Engineering @wareFLO I KNEW IT!!!! #IrishMed https://t.co/xf1dObSZqE
Chelsea Friend @kiwifreund T1: That is the key - to be GOOD at it. Medical sexism abounds (not every woman is crazy or depressed instead of truly physically ill) and not every oral medication is appropriate for every patient. #IrishMed https://t.co/8ijCJB2NSv
TalentCoop @TalentCoop @drlfarrell It's the first brave step in acknowledging illness, restoring foundation of mental wellness & validation of patient. #irishmed T1

Rie Lopez, MPH (she/her) @RieOfLetters Not only is a longer-standing relationship important in understanding patient needs, but it can increase the likelihood of a patient feeling comfortable enough to discuss #mentalhealth challenges. TalentCoop @TalentCoop RT @Elizabe85727641: T1 MH impt in primary care because mind & body are connected, bodily systems work best when in a state of homeostasis. #irishmed
Chuck Webster MD, MS AI, MS Systems Engineering @wareFLO Case in point, remember (not literally, of course, but from medical history) so-called "female hysteria"? #Wikipedia https://t.co/0ZWZHMlgyz #IrishMed https://t.co/CNQmkNqWGU
ElizabethKelly, Ph.D @Elizabe85727641 T2 MH myths include power of the devil, bad spirits, punishment for sin, treatment myths promote these myths via pull self up by boot straps, confess your sins, believe, ask for forgiveness. #irishmed
Rie Lopez, MPH (she/her) @RieOfLetters T2: There are so many myths depending on population. The primary ones in US view #mentalhealth as a weakness/a personal failing. That if one "tries hard enough" they will get better. Some believe they deserve to suffer or it is punishment. #irishmed
Chelsea Friend @kiwifreund T2: That all pills are the answer for everyone; I'd love to see a push for genetic testing/medication metabolism, so we can more appropriately match treatments to patients as they are needed. #IrishMed
Matthew Loxton is also on Mastodon @mloxton T2 The biggest and most evily pernicious myth is that MH is a matter of moral turpitude or laziness - that just "trying harder" will solve any MH illness #IrishMed
April GQG @GpGQueen A2 hello I am a pt advocate new to #IrishMed chat. I see this often especially in #Gastroparesis pts = closed off pts w/drs & lack of proper care. Thx @BarbyIngle for stating such stigmas https://t.co/IFzTPrP8bK

Barby Ingle Official @BarbyIngle @dualireland As a chronic pt, I am fine with it being said either way, as long as the provider is trying to help. #irishmed
Rie Lopez, MPH (she/her) @RieOfLetters T2 (b): We can combat myths through tailored population-based education, including #mentalhealth curriculum in the school system. Primary care providers can also provide point-of-contact education. We all need to talk about it to combat stigma! #irishmed
TalentCoop @TalentCoop Dare I say it 'all in the mind'!! Not real, we all get a bit down, attention seeking, etc etc #irishmed Thankfully publicity is creating change but still way to go. https://t.co/EVAI7SRKMd
MEQAPI @meqapi T1 dunno, but collocating PC and MH can lead to real treatment breakthrough and vastly increase access and reducing missed opportunities #IrishMed
Chuck Webster MD, MS AI, MS Systems Engineering @wareFLO Yesterday, during #socialVR chat, part of discussion centered on how (at least these introverted programmers) they thought they were more comfortable discussing sensitive topics (well, actually, most anything) in VR where they could don an avatar & speak to an avatar! #IrishMed https://t.co/2i2xK7ZDtR
Triona Murphy @Murpht01 @DerekJGallagher Oh my GP is a fabulous support with the everyday trials and tribulations! Would consider him a friend..but thats not Mental Health care provision! Thats him being a human being and a care giver(??) #irishmed
Marie Scully @MarieOScully @katebielinski Hey Kate!! I’m on #IrishMed too!
Douglas Tynan @DougTynan T2 that mental health difficulties are due to a 'chemical imbalance'. All mental health disorders are multi factorial, rarely is there a single cause. #irishmed https://t.co/jCqU2FellK
Chelsea Friend @kiwifreund @wareFLO And we don't even need Wikipedia as a reference - you have me and @BarbyIngle to start. Nearly every woman has had delayed care because she has been told she has been crazy/depressed/not tough enough, even by female docs. #IrishMed
Chuck Webster MD, MS AI, MS Systems Engineering @wareFLO Was about to say up there with leeches, except I think that leeches still have some medical uses (their saliva is an exceptional anti-coagulant apparently) #IrishMed https://t.co/uk9gdIt5Xk

Barby Ingle Official @BarbyIngle A3: Knowing that we can tx more than 1 issue at a time n a pt w good care plan/pathway is important. I use a team care approach and this has helped me bet better access to timely care. I wrote about this in my book The Pain Code. #irishmed https://t.co/G9vivpcvJQ
Rie Lopez, MPH (she/her) @RieOfLetters T2: This bears repeating and retweeting, especially given the current predominant views regarding #GunViolence and #mentalhealth. Individuals with MH challenges/diagnosis are more likely to be victims of violence than perpetrators. Stop using MH as a 'go-to'! #IrishMed https://t.co/V9JRUfEUXO

Matthew Loxton is also on Mastodon @mloxton T3 a good care pathway reduces missed opptys. High % of suicides saw their GP within 30d of death, translates to large missed oppty #IrishMed
Barby Ingle Official @BarbyIngle how about, "dont be a wussy"??? #irishmed https://t.co/DLJUZws01K
Triona Murphy @Murpht01 @organiclemon Still listening😉but for acute episodes?? Lots of evidence of over prescribing on this side of the channel! Not everything can be dealt with at primary level after all PLUS increasing difficult to get appts for GPs here #irishmed We may be discussing "ideals". Far from that here!
Rie Lopez, MPH (she/her) @RieOfLetters @wareFLO This is a fascinating point! Sometimes, that certain level of anonymity allows people to open up. April GQG @GpGQueen @ArGcearta @DrTerryLynch T2 Most pts with chronic illness suffer from mental illness. I hv had to hv misdiagnoses removed from my chart 2x by an arnp not in specialty field over 10 Yrs span. I wasn't even informed. I read my chart #IrishMed
TalentCoop @TalentCoop #irishmed T2 cntd... got to get off chest... results in stigma and then meds are criticised which further stigmatises. Can you imagine another illness where treatment is criticised so ? #irishmed
TalentCoop @TalentCoop Amen to that!! #irishmed https://t.co/vQfs4ql2iZ
Chuck Webster MD, MS AI, MS Systems Engineering @wareFLO I'll bet there is a way to state this in terms of workflow & then perhaps use workflow tech (engines, editors, analytics, etc) to create more transparent & systematically improvable healthcare workflows w/r/t suicide, what do you think? @mloxton? @StuckonSW? #IrishMed #NatCon18 https://t.co/FlEzoCJ04n
ElizabethKelly, Ph.D @Elizabe85727641 Being stateside/living in Texas reluctant to recommend a care pathway. Community resources include Parents for Health & al-a-non, both support groups where people help each other, talk about MH experiences & share experiences with HC/MH professionals. #irishmed
Rie Lopez, MPH (she/her) @RieOfLetters @drlfarrell @FoxNews Oh yes...the arm-chair diagnosis is stunningly transparent from...certain fox-like sources. Don't even get me going here Liam! Seems now is a good time to practice deep-breathing. #mentalhealth #IrishMed

Rie Lopez, MPH (she/her) @RieOfLetters @TalentCoop There aren't many illnesses that face this level of criticism. Although, I would suggest that overweight is similarly stigmatized, as it is also seen as a willpower/personal failing illness at the root. #irishmed
TalentCoop @TalentCoop @drlfarrell T3 It gives continuity but that's rare! So frightening gaps happen for people leaving them vulnerable and unsupported. Advocacy can be a constant which ensures continuity of care, even if it's just conversation, letting person know they matter and are cared about, heard #irishmed

Chelsea Friend @kiwifreund T4: For a while, my therapist was pushing me to go on meds. I knew from past experience that tricyclics have zero affect. After about six months she stopped asking. Later I had genetic testing done & it confirmed I don't metabolize correctly. #IrishMed

Barby Ingle Official @BarbyIngle Yes, the trauma of being over, under, miss TX'ed as a PT can bring on PTSD & depression. This is about care & trust in a system, after disease/condition has already changed our life & we're traumatized by health system & lack of access 2 proper & timely care by system. #irishmed https://t.co/SimBOBaLbP
Triona Murphy @Murpht01 @annsinead @AodhanC @organiclemon Ara now Ann...don't be daft!!Educational disadvantage well recognised and supported in Ireland...but NOT Health. For the thousandth time on twitter I ask WHY?? #irishmed

Triona Murphy @Murpht01 @dualireland @annsinead @AodhanC @organiclemon "supporting" mental health is different than dealing with mental health issues/crisis...but you know that Dual!! #irishmed
TalentCoop @TalentCoop @organiclemon Yes and patients independently but not all help is equal! Needs to meet standard and be included within formal care plan. #irishmed
Clodagh de Jode @realdoc50 T5 unfortunately medication is often our only option and usually, in my experience,it is not a very successful one #IrishMed
Adam Raymakers @AdamR_PhD @drlfarrell T5. That's a good point. Health services provision (and compensation) needs to evolve and appreciate the difference between mental health and (perhaps more obvious) physical health. #irishmed
Adam Raymakers @AdamR_PhD @drlfarrell T5. You're right. There is evidence to suggest that there is a positive association between club/community involvement, and individuals' self-rated overall and specifically their mental health. #irishmed
Clodagh de Jode @realdoc50 T5 when the triggers are poverty, childhood trauma, unemployment, poor housing it is difficult to see what we can offer except sympathy #IrishMed
TalentCoop @TalentCoop @ArGcearta @drlfarrell @PietaHouse Yes agree... overwhelm also needs factoring in, sheer numbers. Need for more good therapists. #irishmed
Triona Murphy @Murpht01 @drlfarrell Thanks Liam. Course our community Mental health Nurse colleagues could be the resource that Primary care needs.....but there are not enough of them either!! #irishmed
TalentCoop @TalentCoop @DerekJGallagher @drlfarrell Yes thats why I'd like inclusion within formal care plan. #irishmed
Rie Lopez, MPH (she/her) @RieOfLetters T5: I manage my #mentalhealth challenges through managing my schedule, meditation, mindfulness practice, coloring, having a group of supportive friends and family, eating well and getting daily movement. I also take all prescribed medication as indicated. #irishmed
Chuck Webster MD, MS AI, MS Systems Engineering @wareFLO BTW, my other (non-workflow) hat besides VR is building stuff (little robots kids find very funny). I find #Making excellent mental therapy (hmm, #makerspace therapy?). Recently discovered the #MakersHour 2 hours before #IrishMed so maybe see you next week https://t.co/rA6LYYGTMR

Rie Lopez, MPH (she/her) @RieOfLetters CT: #mentalhealth continues to be an essential and timely topic. There is so much to discuss and so little time! There is so much I missed, but I can't wait to catch up! I do have hope, as we are all primed to make a difference. #irishmed

Sean Erreger, LCSW @StuckonSW RT @mloxton: @wareFLO @StuckonSW I totally agree, and have been hunting for appropriate data to see what process mining might reveal about the pathways common to survival, thriving, and death #irishmed
Sean Erreger, LCSW @StuckonSW RT @wareFLO: I'll bet there is a way to state this in terms of workflow & then perhaps use workflow tech (engines, editors, analytics, etc) to create more transparent & systematically improvable healthcare workflows w/r/t suicide, what do you think? @mloxton? @StuckonSW? #IrishMed #NatCon18 https://t.co/FlEzoCJ04n
