Demo Day: Merck | Heritage Provider Network Innovation Challenge

Merck and the Heritage Provider Network have identified care plan adherence in chronic disease as an important lever for change in the triple aim of reducing costs in healthcare while improving quality and outcomes. At Demo Day, the Challenge's semi-finalists will present how they plan to change the way patients with diabetes and/or heart disease engage with and adhere to their comprehensive care plan, ultimately leading to a better quality of life.
