Diverse Alzheimers @DiverseAlz
Welcome to #Diversealz here to chat about ways to meet the needs of people with dementia. It's all about person centred care/knowing the person but also about wishes
Diverse Alzheimers @DiverseAlz
We have chosen tonight's topic on a whim but think it a pertinent one as there may be a situation whereby what you think a person with dementia may want is not what they need nor what they want...#diversealz
Diverse Alzheimers @DiverseAlz
We have our one word icebreaker for anyone waiting to participate or tuning in but willing to give a one worder #Diversealz It follows next tweet...
Diverse Alzheimers @DiverseAlz
Question 1 our icebreaker: please give us your one word relating to the theme of meeting the needs of people with dementia on their terms. #diversealz ours is enjoyment based on day bought wonderful culinary delights to make and all person wanted was something simple.
Diverse Alzheimers @DiverseAlz
We've asked you to give us one word for our theme today - meeting the needs of people with dementia on their terms. Our word is "enjoy/enjoyment" things that give them joy and sometimes it can be so simple, too simple for your liking but is about you or the person? #diversealz
Diverse Alzheimers @DiverseAlz
Good to see some of the answers coming up for our icebreakers. From @wendymo94921768 we have "Ronnie" reference to gentleman, who is is and who is was. We say this is so important and yes, we're going to ask you in course of conversation #diversealz

Diverse Alzheimers @DiverseAlz
Respect via @helenfindlay welcome with thanks. Respect came into the backstory that led to this conversation. Though it started with food and wanting to give the best meal...it was the simplicity of one of the ingredients that mattered most to the PWD so went along #diversealz
Diverse Alzheimers @DiverseAlz
Empathy via @TommyTommytee18. We've tweeted about this one word as a whole #diversealz conversation. We will try to revisit before the end of the chat.

Kindness from @LynnGamble20. Indeed question 2 is inspired by @TommyTommytee18 @LynnGamble20 Wait and see! #diversealz

Question 2: Kindness and empathy....should this be at the back of our minds when thinking about the wishes of people with dementia. Note the words wishes and not needs. This is a conversation about responding to what one would like #diversealz

Diverse Alzheimers @DiverseAlz
@EstherWRJ welcome to #Diversealz chat. We chat every Thursday 8pmBST on a range of issues around dementia with reference to all the dementias not specifically Alzheimer's. We're the only chat with a regular focus on diversity issues but #diversealz-ers will fill you in.
Diverse Alzheimers @DiverseAlz
@EstherWRJ to follow and join in chat just add #diversealz in your sentence as a tag so that we can see what you are saying and respond. Tonight we chat about meeting the desires/wishes of people with dementia. What they want as opposed to what we want for them

Question 3: At times, is it necessary to put aside your thoughts on what you feel a person with dementia needs? #diversealz Remember our topic.

@DiverseAlz Today my mother's PSW cancelled. It's a gloomy rainy day and mum would have liked to stay in bed all day. We did a compromise, and I got her up, but she lounging around in her nightie & dressing gown. Pleased with that.#personcenteredcare #DiverseAlz

RT @estherWRJ there’s something about respecting a person’s autonomy and not imposing our own sense of what someone’s ‘best interests’ are #diversealz
Diverse Alzheimers @DiverseAlz
@EstherWRJ @helenfindlay Yes. You are doing this perfectly. RE: the food it was a meal of seafood and fish and yet one part of the ingredients that the person with dementia wanted was not what had in mind but gave in as it meant so much and there was great enjoyment. Is about compromise too #diversealz
Caroline Bartle @3Uknz
@DiverseAlz I’m starting to think radically there is no such thing as person centred care in dementia. Not because it isn’t possible but because it isn’t appropriate model in later stages. Because identities are co constructed and evolving in later stages #DiverseAlz #thinkonyourfeet
Diverse Alzheimers @DiverseAlz
Compromise also via @journeyofcareM and mum who in end "lounged" around in nightie and dressing gown in response to rainy day among other things. All pleased about that. Course other side could have been must get dressed etc but think about lounging. Relaxing etc. #diversealz
Caroline @journeyofcareM
@DiverseAlz A.3 Sure, it is, but what most often happens is the need is still there, but the timing isn't right. So, a wait and try again later approach usually works. #DiverseAlz
Diverse Alzheimers @DiverseAlz
@EstherWRJ @helenfindlay Home setting though could be translated to many others am sure. Well, in care home setting might have to be amended to getting a meal one would enjoy or adapting #diversealz

well, @sandras_smith has named a dementia for us FTD, so let's timetable question 4 around the dementias, not that we hadn't been speaking about dementia! #diversealz
Diverse Alzheimers @DiverseAlz
Question 4: thinking about the dementias, we can mollycoddle people with dementia. Fact. So how can we be more aware of their wishes? Is it about learning to speak dementia relating to the individual? #diversealz

Want to get Question 5 in before forget as it is about stages of dementia and also interested in jargon for want of a better word #diversealz
nursesKidONE @nursesKidONE
@3SpiritUKNZ @DiverseAlz “Because identities are co constructed and evolving in later stages” What does this mean? I felt that supporting my mother’s identity was a constant battle against all of the people who preferred to disregard her identity because it was more convenient for them. #diversealz
Caroline @journeyofcareM
@3SpiritUKNZ @DiverseAlz I'm wondering why you do not think it appropriate in the later stages. Humans can continue to communicate even under dire circumstances & #pwd are no different. Being attuned to nuances, like person centered care, could guide one in their care to the end. #DiverseAlz
Diverse Alzheimers @DiverseAlz
Question 5 how should one cope with mid to late stages of dementia. Do choices still apply? #DiverseAlz

@DiverseAlz A.4 This question made me think of a tweet where a dementia care giver took issue with mollycoddling/something of the sort & instead challenged the #pwd vigorously. Found it enlivened them. Not good overall, I'm sure, but not always harmful. #DiverseAlz

@DiverseAlz A.1 My mum is in the mid/late stage (so hard to define) but put a plate of food in front of her and she can still make a choice of what she likes or not. She might not be able to articulate a preference with reasons why, but she can still express herself very well. #DiverseAlz
Diverse Alzheimers @DiverseAlz
Question 6: how can we encourage people to stop making assumptions about people with dementia. @TommyTommytee18 mentioned that many people do not understand the needs of people with dementia or how to care/support them #DiverseAlz
nursesKidONE @nursesKidONE
Is it ‘co-construction’ when limitations/restrictions are unilaterally forced on a person? Like places that don’t employ adequate staff numbers to escort residents to bathroom, eg, one (& probably both) hospitals to which my mom was admitted, w/ execs paid millions. #DiverseAlz
