Andddd, we're off! Question 1 of our #JHMchat tonight:
Annie Massart @Annie_Massart_
@JHospMedicine T1: My intern year was 2011-12, the 1st year of reduced intern work hours. We had overnight experiences but w/o 28+ hour call. I remember being struck by the night-to-night variability. Some quiet, some nonstop codes & admissions. Some seniors happy to teach, some not. #JHMChat
Rebecca Jaffe, MD @RJmdphilly
Rebecca Jaffe here...PGY11 I think? Chatting and watching startrek and untangling knitting tonight... #jhmchat
Avital Y. O'Glasser, MD FACP SFHM (she/her) @aoglasser
@JHospMedicine #JHMChat intern year at @OHSUIMRes the year after Twitter was created: 2007-2008! night were hard, but could still be fulfilling. A lot also depended on the frequency of overnight. Q5 "doable", Q3 felt physically impossible
Shaurya Taran @shauryataran
@JHospMedicine #JHMchat There is a wide breadth of cases. Lots of "bread and butter" medicine but also some very esoteric presentations of rare diseases, side effects of novel medications, etc
Vinny Arora MD MAPP @FutureDocs
T1 what stands out to me being in call were the days we ate dinner together and helped each other out. #jhmchat plus free cake in doctors dining room at 10p when I was intern :)
Anika Kumar, MD (she/her) @freckledpedidoc
@JHospMedicine A1. I was an intern in 2010. I think I remember most the challenging NICU cases I saw. I also remember the confidence that the Medical Director of the NICU, the fellow and my senior had in my ability to safely care for some of our sickest babies. #JHMChat

@JHospMedicine Pediatric intern year, 2013-2014. My favorite part about call nights were the bonds formed b/w interns and seniors when juggling acutely sick patients and admissions. As a 3rd year senior, I covered 5 different teams at night for 3 rotations, and relied on my interns. #JHMchat
Shaurya Taran @shauryataran

@FutureDocs #JHMchat we had midnight curry--the cafeteria was open 24/7, and the hot made from scratch curry was served for those on night shifts I know a lot of OHSU folks remember it fondly
Dr. Tsega @drtsega
Q1: I was an intern in 2011 and remember the camaraderie being on nights (my co-intern calming me down as I freaked out about managing my first instance of chest pain), and feeling like there was always someone to help. #JHMChat
Jon Lim, MD 🏳️‍🌈 @JonLimMD
A1: 2015-2016. lots of learning, running around, pages and phone calls. I think I once counted >100-120 pages during a night float stint in the winter. Also remember post-call brunch and trying to do "family dinner" at BLSMC when we had 5 residents in house each night. #JHMChat

I remember doing a 24+6-hour call in our MRICU and falling asleep on my hand-written H&Ps (*gasp*). I woke up to my attending trying to slip them back under me (already completed by her). #JHMChat
Allan Detsky MD PhD CM @ADetsky
@FutureDocs I loved our 10 o’clock meals . When I was chief of medicine I tried to do them at my hospital. Didn’t work because we just couldn’t get people to come at the same time. #JHMCHat
Avital Y. O'Glasser, MD FACP SFHM (she/her) @aoglasser
@JonLimMD #JHMchat yes all around--I think i tried to count # pages once and lost track and the amount of walking--the main medicine wards were clear across campus from the shiny, new wards patients overflowed to. I think I once estimated 8 miles one night (before I had a fitbit)

@JHospMedicine A1: 2011-2012. It was so busy and so acute. I think I sent a patient to the ICU every night with at least 5 other rapids. Had extremely supportive seniors and nurses. Felt like I could take on anything after that. #jhmchat

Adam Rodman @AdamRodmanMD
@aoglasser @FutureDocs @ADetsky @freckledpedidoc @ETSshow @JHospMedicine @AndreMansoor I did, yeah! (also hi #jhmchat!) "Pimping" is a fairly recent term (probably second half of 20th century) despite claims to the contrary. It's an icky word, but none of the alternatives have really caught. The best I've seen in "malignant questioning" from @AmulyaNagarurMD
Shaurya Taran @shauryataran
@roc_night Keeping it short and relevant is key. Hard to sustain attention for more than 2-3 minutes when so many other tasks need your attention. #JHMChat
Ben Chin-Yee @BenChinYee
@ShreyaTrivediMD @SocietyHospMed @JHospMedicine @ADetsky @shauryataran Sounds like a cool idea. Would love a more systematic way to follow up on cases/overnight decisions. As SMR always tried to touch base with reviewing staff or admitting intern the next morning to see if they agreed with management #JHMchat

@JHospMedicine T4: I hear the addition of nocturnists has added both formal and on-the-fly teaching for the night team...right @roc_night? However, our R3 still accepts patients from the ED so they maintain that educational, autonomous experience...with nocturnist there for back-up. #JHMChat

I think what I’ve learned about the changes in Peds residency Call is that many programs are balancing duty hours, education & patient safety with increased patient complexity & census (with fewer community hospitals admitting kids... but that’s another discussion) #JHMChat

@AdamRodmanMD @aoglasser @FutureDocs @freckledpedidoc @ETSshow @JHospMedicine @AndreMansoor @AmulyaNagarurMD Malignant questioning is ok, but one word would be better.
