Good evening! Welcome to tonight's @OTalk_ discussing the topic of #occupationaltherapy and compulsive hoarding. Please say hello and introduce yourself. The questions have been designed to be open to everyone, those with or without experience in this setting. #OTalk https://t.co/Rex8Lux3MY
Orla @orlatheot
Hi #OTalk, my name is Orla. I am studying my #OccupationalTherapy MSc at @teesOTuni. My dissertation has received ethical approval (today!) to interview occupational therapists on their perceived role of occupational therapy in compulsive hoarding.

Bill Wong, OTD, OTR/L @BillWongOT
@orlatheot @OTalk_ I see a bit from my dad... as he does collect quite a bit of stationaries. And since my family and I got to the US, we have been buying a good bit of stuff off rummage sales. Aside from finance, sometimes that can also impact how we use the home space occupationally. #otalk
Karen Ivey @KarenIvey
my experience is that people reduce their occupations and ADLs #OTalk

@gra_carley Oh really? In question 2 you can share your experience ;) #OTalk
Becky Cooper @beckycooper___

Bill Wong, OTD, OTR/L @BillWongOT
@gra_carley I think in US, I would not be surprised if people work in home health setting for elderly will encounter a case or 2 of such. #otalk
Sarah Carter @ot_saz
@orlatheot @OTalk_ I think hoarding could in theory be an occupation. However also influence occupational balance significantly & actually mean that people have restricted access to their env & other occupations. Alienating/depriving them from other opps & potential. #OTalk
Bill Wong, OTD, OTR/L @BillWongOT
RT @Lauren_OT: @orlatheot @OTalk_ Environment could be hazardous, smaller physical space to utilise resources needed to complete ADL’s. #OT
Lucy gordon @Lucygordon83
@BillWongOT @orlatheot @OTalk_ I have no experience in this area. However I am thinking it may affect relationships, an individual may become socially isolated because of how they feel or how others may feel about them. It may cause occupational disruption and possible safety issues. #OTalk
Orla @orlatheot

@OTalk_ So sorry #OTalk
Orla @orlatheot
@Lauren_OT @OTalk_ Yes, 1 in 4 people that die in house fires were hoarders according to @BBCTheOneShow this evening. A shocking stat #OTalk
Keir Harding @Keirwales

@elenaluck_OT Yes Elena, there is a big stigma. People are portrayed on reality tv shows as lazy and dirty which doesn't help public perception #OTalk
Rachel OT 🌻 @_rachelOT
@orlatheot @OTalk_ From limited experience of this with SU’s, both appear unaware/in denial of the hoarding being an issue. It does make it harder to engage in the home due to space, but it doesn’t seem to limit them socially, they will still invite people over without apparent concern. #otalk

@orlatheot @OTalk_ Hi All 😄👋🏻I’m a soon to be graduate at #YSJOT and just completed my final placement in adult community rehab. Interested in tonight’s topic from working with a couple of clients that hoarded in my pre OT days in home care. Looking forward to hearing everyone’s thoughts #OTalk
Sarah Carter @ot_saz
@BillWongOT @orlatheot @OTalk_ Likely a psychological reason. So that in itself will influence. But then then there are possible habitual, motivational & logistical difficulties that arise from the hoarding behaviour itself. #OTalk
Bill Wong, OTD, OTR/L @BillWongOT
@HessionVicky @orlatheot @OTalk_ I think that is what the US TV show intervention has. #otalk
Becky Cooper @beckycooper___
@orlatheot @OTalk_ In the community, we have a service we can refer on to that will assist a client with hoarding if they consent to this. I'm hoping to feel more equipped to initiate these conversations and perhaps offer suggestions and support the client through the process myself #OTalk

Sarah Carter @ot_saz
@orlatheot Presumably not only things that are good for us are classed as occupations. If meaning is important. Quality of life - the person could potentially see the hoarding as improving their life? #OTalk
Keir Harding @Keirwales
@orlatheot @OTalk_ Interestingly, with the people I've worked with their hoarding has only ever been a problem for others #OTalk
Carolina Cordero 🌈 @colourful_ot
@Keirwales @ot_saz @orlatheot @OTalk_ I was just thinking that re: Q2 about feeling equipped to work collaboratively with people who compulsively hoard. I'd feel uncomfortable about the ethics of going into someone's home and telling them to throw out things that are important to them against their will #OTalk
Elena Luck @elenaluck_OT
#OTalk I haven't had experience as such. Although I have covered hoarding under the 'self neglect' category in my safeguarding training which highlighted the importance of trying to re-engage with service users at a later stage if they decline support initially and have capacity https://t.co/xm4QM03qup

Sarah Carter @ot_saz
@Keirwales @orlatheot @OTalk_ Definitely. The TV shows we see tend to show people who want to change behaviour but struggle to act. This can't always be the case. People must gain something from hoarding. Hence earlier Q about capacity to choose/adult safeguarding. #OTalk
Rachel OT 🌻 @_rachelOT
@orlatheot @OTalk_ I have worked closely with two people who compulsively hoard, both individuals have autism. We have certainly struggled to get support in this area, luckily they live in residential care where the risk of harm to the individual from their hoarding/self neglect is lower. #otalk

Becky Cooper @beckycooper___
@colourful_ot @Keirwales @ot_saz @orlatheot @OTalk_ That is a great point - we need to feel equipped about how to initiate these conversations in the most appropriate way. Our trust does offer a 'having difficult conversations' course which I'm planning on attending to help me feel more equipped #OTalk
grainne carley @gra_carley
On home visits in inpatient physical health pre discharge we've requested de cluttering as 'necessary' for safety around the home, but I think its may be a case of :covering yourself' & doesn't consider the persons wishes #OTalk
Elena Luck @elenaluck_OT
I also think our skill of therapeutic use of self is extra important when working with this client group. Relationship building first rather than immediately trying to resolve hoarding issues #OTalk https://t.co/xm4QM03qup
Bill Wong, OTD, OTR/L @BillWongOT
@RRule_OT @orlatheot @OTalk_ If it is autism, can definitely be related to their obsessions! #otalk
Keir Harding @Keirwales
@colourful_ot @ot_saz @orlatheot @OTalk_ #otalk me too. The distress criteria implies people want to change something but without that desire what right do we have to tell people how to live (if it's not harming others).
Emma Robinson @EmmaRobinsonOT
@orlatheot @OTalk_ I worked with a gentleman with Diogenes Syndrome (essentially hoarding & squalor) he was unable to complete D/ADLs due to access around his home, poor relations with neighbours due to smell/appearance & reduced health due to unmanaged meds and lifestyle #OTalk
Carolina Cordero 🌈 @colourful_ot
@gra_carley @Keirwales @beckycooper___ @KarenIvey Apparently hoarding is common amongst people who have previously been homeless for a similar reason https://t.co/57vGSo3KBM #OTalk

#OccupationalTherapy people, I would recommend hosting an #OTalk Twitter chat. A great way to get opinions on topics you are working on or struggling with. Happy to answer any questions anyone has about it. Thanks to the @OTalk_ for all their hard work! https://t.co/z8StRH67Ud https://t.co/OP7rzUdhBW

@EmmaOT2B @colourful_ot @orlatheot @OTalk_ Agreed! I think there are often (sometimes very clear) psychosocial reasons for hoarding! A lady I was working with generally was a bit of a collector, but also kept a lot if items that reminded her of her late husband. It had been him that had been keeping her "in check' #OTalk
