Time for #OTalk . I'm here 🙋‍♀️
RCOTSSWork @RCOT_Work
Welcome to our #OTalk and thank you for joining us. We'll kick off immediately with Q1: How can we facilitate a return to work for a client with poor insight? We are interested to hear how you do this in a way that is honest but preserves their wellbeing and dignity.
Emma Harman @OTEmmaHarman
I’m ready! #OTalk !

RT @RCOT_Work: Looking forward to 8pm for tonight’s #OTalk when we will be asking the #ot community to join our debate on the communication…

@RCOT_Work #OTalk personalised goal setting - being as client centred as possible. Even if it takes a few sessions to build up rapport ... clients best interest will be the key to achieving goals.

Bill Wong, OTD, OTR/L @BillWongOT
@RCOT_Work A1. That's an interesting question. I think first of all, you want to get as much info about the patient's past employment history, as well as performing chart review first. After that, I think motivational interviewing will be key. #otalk
Healthywork Ltd @HealthyworkLtd
@RCOT_Work #oTalk i disciuss issues with a client ans then a client reads their report before release to the referrer/employer so they begin to understand what our suggestions are.

Katherine Swithenbank @katy_maggi
@RCOT_Work #Otalk this is a tricky one, depends on lots of factors such as how much they lack insight, if we think this will improve with time, what their job role is etc. Careful assessment, goal setting, simulation, work based assessment and much more, very complex cases usually.
Bill Wong, OTD, OTR/L @BillWongOT
@RCOT_Work Personally, I seen this type of work as a patient. I have not seen that aspect at all while I was receiving OT services. #otalk
Marissa_OT @Marissa_OT
@RCOT_Work I would say having a discussion with the client prior to meeting the employer is needed. This way you could run through with the client what you had in mind to say to the employer and also make sure everything the client wanted to get across is included #OTalk
Sinéad Hynes @sineadnieidhin
Q1 #otalk If the client allows it then videoing session and watching it back together can be very useful.
Helen Barnes @mehelenbarnes
#OTalk wondering is this could be agreed at the contracting phase at the beginning of the process?
#OTalk @OTalk_
@OTJuliePhillips @RCOT_Work Hi Julie, please use the #OTalk hashtag in your tweets so that people following the chat can find them more easily and so that they're included in the chat transcript afterwards 😊
Bill Wong, OTD, OTR/L @BillWongOT
@mehelenbarnes yes- absolutely. I think it is a good idea to have conversations with the patients in the beginning, but also during the process to remind the patient sometimes. #otalk

Katherine Swithenbank @katy_maggi
@RCOT_Work I haven’t done many work meetings but would always ensure the patient is present and we have discussed/prepared all feedback prior to the meeting #otalk
Catherine McNulty @cathymc9781
#perhpas further exploration about the 'lack of insight' what do you mean by this phrase? and from whose perspective?
Laura Student OT @Laura_Does
@RCOT_Work #OTalk Ensuring the client is involved in all meetings about them and their work would be more client-centered, surely? Then fewer misinterpretations are likely to occur as well and clients have control about what is said/not said.
Marissa_OT @Marissa_OT
@RCOT_Work Also, I think it is important to make the client aware that even the difficult topics need to be discussed but also linking it in to how it may affect them at work if the employer is not aware of the their difficulties in certain areas. #OTalk
Sheena Wight @sheena_OT
@RCOT_Work A1 #OTalk Would want to know more about extent of the poor insight, what had caused it and if it could be likely to change. Careful assessment of other cognitive skills would support the work ie dysexecutive functioning, attention, memory etc. As well as being person centered
#OTalk @OTalk_
RT @Laura_Does: @RCOT_Work #OTalk Ensuring the client is involved in all meetings about them and their work would be more client-centered,…
@mileswashbrook @mileswashbrook
@RCOT_Work #OTalk Used an Access to Medical Records Act agreement with patient/employee to share assessment and treatment details with employer and primary physician.
Juliette Truman @Jt12Truman
I agree this is tricky - it is essential that we work with the person to establish the remit of disclosure #OTalk I believe so much goes on behind the scenes with the individual- I have done some research on this happy to share
Deirdre Heaney @Deeheaney4
@RCOT_Work My service does not include meetings with employer's. My recommendations are written , using a AHP health n work report and given to employee to share with employer. #otalk

RT @RCOT_Work: #OTalk This is all really useful and interesting thank you so far.

I think it’s important to maintain the Individual’s sense of self and remember the impact that HCPs have on this and the messages we send to employers by just being involved #otalk

#OTalk @OTalk_
RT @RCOT_Work: #OTalk Question 3: How do you manage risk when things don’t go to plan?

@RCOT_Work From a patient's perspective, I think you have to understand what the patients' tolerance first. #otalk
Sheena Wight @sheena_OT
@RCOT_Work #OTalk Risks should be fully anticipated and explored at the outside with the patient plus also have discussion / agreement about how to mitigate these ie best and worse case scenarios with alternatives explored.

Marissa_OT @Marissa_OT
@RCOT_Work It's all about positive risk taking. If something doesn't work out, then we need to take a step back, have a look at what we did, where it went wrong and approach the situation differently next time. I think it is also important to have a debrief after as well #OTalk
Laura Student OT @Laura_Does
@RCOT_Work #OTalk Risk assessing key tasks and having a grading plan in place in the first place. This could also allow clients to step-up when they feel comfortable to instead of when expected. Fewer problems occur this way I believe? Not sure about afterwards.
Juliette Truman @Jt12Truman
I agree we need to discuss this with our client and the employer prior to any action nothing gained without positive risk taking #OTalk

Healthywork Ltd @HealthyworkLtd
@RCOT_Work Sometimes the pt has not been given enough info/Referral options, so it is necessary to support them and make recommendations. Dont be devisive. #otalk
Bill Wong, OTD, OTR/L @BillWongOT
@RLucasOT1 @Marissa_OT @RCOT_Work I see that personally. For me to work in my current work setting in OT actually was an example of that. Before I interviewed, I actually had low self confidence because my first OT job flopped badly. So, to go to another setting with no placement experience was scary. #otalk
Marissa_OT @Marissa_OT
@Jt12Truman Definitely! It should be an open dialogue #OTalk
Bill Wong, OTD, OTR/L @BillWongOT
@RLucasOT1 @Marissa_OT @RCOT_Work and speaking of risk- I started as an adjunct faculty at a uni late last year. I chose to go this route instead of full time... while it was positive risk taking, but it was not so reckless at the time (as I have option to not renew contract if I didn't like it). #otalk
Katherine Swithenbank @katy_maggi
@RCOT_Work #OTalk hopefully risks will have been considered before plans/ treatments are carried out and options to keep them safe have been agreed with the patient beforehand. A good assessment and knowledge of your patient would be key to this.
