On the [REDACTED USER] account tonight from Edinburgh. Fairly obviously - I'm a physio! #physiotalk
#OTalk @OTalk_
Hello #PhysioTalk it’s a pleasure to be with you, tonight. On the account tonight [REDACTED USER] I’m a clinical lead OT in a mental health trust, I have some experience of working alongside physios but perhaps not as much as my acute trust and social services colleagues.
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
[REDACTED USER] Hi I’m Holleigh. Occupational Therapist in Palliative care #physiotalk
#OTalk @OTalk_
Don’t forget that # and tonight remember we are using #PhysioTalk
#OTalk @OTalk_
Don’t forget the hashtag in all your tweets #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] Hi I’m Rhona, I’m a physiotherapist in acute medicine in Paisley, near Glasgow #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
[REDACTED USER] My service has newly set this up in the last two weeks, so far I’m loving it, but it’s still very much new to us all! #physiotalk
#OTalk @OTalk_
Question 1 #physiotalk
@_JoeMiddleton
[REDACTED USER] Challenges and opportunities, probably in equal measure!!! #physiotalk
Stella Krain @krain_stella
[REDACTED USER] Hi, I'm Stella Krain OT and joint team leader with my PT colleague Nicky of CHAT (Community Hospital Admissions Avoidance Team). We are based in Rotherham ED/ AMU/Community Our team consists of OT'S, PT's and TSW'S all working generically. #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] I started as a new band 6 in 5 years ago in a medical team that was newly attempting to integrate. Lots of teething problems & anxiety around losing professional identities initially but now I can’t imagine things ever going back! So much more progressive & effective #physiotalk
#OTalk @OTalk_
[REDACTED USER] From my experience of 15 years of working in acute mental health and rehab services, rarely are cross over of roles but, more a referral to physio for advice and help! #physiotalk
Sophie Cresswell @93SophieC
[REDACTED USER] My first experience of a crossover role was in an inpatient developmental paediatric placement- an excellent opportunity to gain knowledge of development and anatomy #physiotalk
#OTalk @OTalk_
Don’t forget the hashtag #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] [REDACTED USER] That’s very similar to how our integration started. We realised as we started to learn to work together how much more fluid and hollistic our assessments could be if we took advantage of each other’s expertise and where the roles blurred! #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] [REDACTED USER] Definitely. Sometimes we have to take a step back and reaffirm those professional lines but the increase to efficiency, communication and effectiveness of intervention with some crossover seems to be well worth the challenges #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
[REDACTED USER] Initially when this started in our service, I had quite a few concerns and even started feeling territorial, which surprised me that I felt this way. But I feel a lot more open to it now we’ve trialled it more #physiotalk
#OTalk @OTalk_
What particular things were you being territorial about? #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] I would concur that areas where PT input is primarily/almost solely respiratory/chest PT in nature OTs may have less of a role there such as ITU/ICU but as these patients progress to other wards the goals become more functional & their input is vital #physiotalk
@_JoeMiddleton
[REDACTED USER] Surely joint working occurs everywhere that occupational therapists and physiotherapists co-exist? However, cross-boundary working might not. #physiotalk
#OTalk @OTalk_
Question 3 #physiotalk
Stella Krain @krain_stella
[REDACTED USER] Working generically in a community therapy team and also in ED / AMU has made us a strong team. We have increased skills, our assessments are more thorough and we share knowledge. However we also know when we need ask for an OT or PT specific detailed assessment. #physiotalk
#OTalk @OTalk_
What do others think? #physiotalk
@_JoeMiddleton
Physiotherapists and Occupational Therapists occupy the same therapeutic ecosystem, but are trained very differently. Understanding that is an important first step. (Although I still don't understand all these models of practice and assessment within OT) #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
[REDACTED USER] Continuity of care, less energy expenditure and duplication for fatigued patients, better time efficiency for staff, up skilled staff, reduced delays, discharge planning continuing even during staff absence/sickness, the benefits are endless #physiotalk
@_JoeMiddleton
Is lack of staff a driver? I'm sure we'd all like to say no, but... #physiotalk
Rhona Burch @BurchRhona
@HolleighLouise [REDACTED USER] I think this is very common. It’s what both professions felt strongly at the start of our service integration but it’s the ‘norm’ now & to work in isolation without consideration of each others goals and roles with patients seems alien and less holistic now! #physiotalk
Mary Rose Mulry @MulryMary

amanda deaves @ADeaves
[REDACTED USER] [REDACTED USER] #physiotalk Whilst I agree with your first comments. The aims and objectives should be the patients / client / service users not the professionals to ensure that their needs are met.
#OTalk @OTalk_
What do you mean by poor staffing? Inability to fill rules? Incompetent staff? Lack of funding? #PhysioTalk
Rhona Burch @BurchRhona
@chelseadagger21 [REDACTED USER] I definitely agree. Nearly 6 years on I don’t think we could ever make a convincing case to go back to the ‘old way’. Practice & the way we deliver it needs to & should be constantly evolving & moving forward...even if it can be scary! #physiotalk
@_JoeMiddleton
If a physiotherapist is compentent in traditional OT skills, are they an 'extended scope practitioner'? (and vice versa) #physiotalk
Stella Krain @krain_stella
@OTalk_ Our OT'S and PT'S carry out the sane assessment, using exactly the same basic paperwork. There are "add-on" assessments that are OT and PT specific if necessary. We definitely cross the boundaries. #physiotalk
#OTalk @OTalk_
The same therapeutic ecosystem? Can you elaborate on this? #PhysioTalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@BurchRhona [REDACTED USER] I’m not too sure what was really driving the territorial-ness. I think it might have been because I started in palliative care for the first time two weeks prior to the integration, so I was already feeling a little lacking so I started to feel a little useless #physiotalk
laura jenions @mrsbungy
[REDACTED USER] I would disagree with the comments about ICU. There is an increasing focus on rehab and chests are no longer treated in isolation. Therefore increasing opportunities for joint working. Also joint working needed to manage delirium within crit care #physiotalk
@_JoeMiddleton
@OTalk_ We share a holistic, broad approach to the care of our patients and are often found working in the same settings, often with the same ultimate purpose and/or function. #physiotalk
Rhona Burch @BurchRhona
@HolleighLouise [REDACTED USER] I love your comment on less duplication for fatigued patients. We tend to think about the less duplication in reference to us professional & the service- such a valid and true point that it benefits these patients massively too! #physiotalk
#OTalk @OTalk_
What do you mean by traditional OT skills? Is the skill not to complete an occupational based assessment? Can this be done by another profession? ( i’m just playing devil‘s advocate) #PhysioTalk
Stella Krain @krain_stella
@clbowdler [REDACTED USER] Yes it is a much more efficient use of staff if they can do one joint assessment. It enables a quick response rather than waiting for an OT and then a PT to assess separately. #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] [REDACTED USER] [REDACTED USER] - sorry! First # I’ve missed 😂 #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@BurchRhona [REDACTED USER] I think it’s so important, in palliative care, we often talk to our patients about fatigue management, but we should practice what we preach and allow them to pace and prioritise! #physiotalk
@_JoeMiddleton
@OTalk_ I mean, sure, if you want to compile the breadth of knowledge and skills held by the OT profession into a single skill ;) As other tweets have mentioned, I would consider a cognitive assessment (within an occupational context) to be an OT specific skill. #physiotalk
#OTalk @OTalk_
I’ve not worked in a physical health setting since being a student. But the same assessment? Are PTs and OT’s not looking for different things? And do you occupational therapist not use standardised occupational therapy assessment, like #MOHO #VDTmoca or others? #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
[REDACTED USER] [REDACTED USER] @Georgia_Physio [REDACTED USER] What do your generic competencies involve? We’re trying to establish some generic competencies in our service #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] [REDACTED USER] [REDACTED USER] @krain_stella [REDACTED USER] Definitely, we make use of the areas where our roles blur but there are still tasks that ‘belong’ to that profession & rightly so: chest PT, kitchen Ax, cognitive Ax, neuro gait re-ed/facilitation, treatment of MSK specific injuries to name but a few #physiotalk
#OTalk @OTalk_
Thanks I understand your point now! #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
[REDACTED USER] I think perhaps the biggest barrier I’ve come across is people’s fear of change and negative thoughts about it all #physiotalk
@_JoeMiddleton
Oh! AN opportunity to plug the [REDACTED USER] therapy compencies for acute medicine! https://t.co/QRDqCS9SE3 #physiotalk
Stella Krain @krain_stella
[REDACTED USER] [REDACTED USER] PT = respiratory or MSK specific. OT = complicated moving and handling / equipment assessment, cognitive assessment for example. #physiotalk
#OTalk @OTalk_
@_JoeMiddleton So do you have any questions about occupational therapy models I might be able to help you learn more. #PhysioTalk
Janet Thomas @JanetThomas47
[REDACTED USER] [REDACTED USER] Well it was an [REDACTED USER] tweet that sparked this chat idea :) #physiotalk
Rhona Burch @BurchRhona
@krain_stella @OTalk_ This resonates with what I remember to be our ‘old way’ of working! Paperwork was colour coded but essentially pages of the same information and duplicated assessments for a lot of interventions #physiotalk
Sophie Austin @_SophAustin
[REDACTED USER] Reluctance from staff to lose identity or share responsibilities, this hinders any progress and development to push forward #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
#physiotalk are there any good resources you recommend for an OT wanting to learn more about the physio side of things?
Fiona Skelton @skelton_fiona

#OTalk @OTalk_
Within mental health settings occupational therapists use standardised occupational therapy assessments, I get the feeling this is not as commonplace in a physical health setting? Thoughts and experiences? #PhysioTalk
@_JoeMiddleton
@OTalk_ What, How and Why? Like, what are they? How do they work? Why do the exist? Do any other healthcare professions use such defined models? (I'm seeming to recall that mental health practitioners do?) #physiotalk

Rhona Burch @BurchRhona
[REDACTED USER] [REDACTED USER] [REDACTED USER] @Georgia_Physio As [REDACTED USER] highlighted in another response it’s something we try to be very mindful of with both professions’ rotational staff. We have an integrated working workshop to firm those boundaries & encourage junior staff to remain ‘true to their profession & scope’ #physiotalk
Mary Rose Mulry @MulryMary
#otalk #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@BurchRhona [REDACTED USER] [REDACTED USER] [REDACTED USER] @Georgia_Physio [REDACTED USER] Very good point. I’m rotational, and when I initially started in this integrated role, I started to worry that I wouldn’t get the OT skills I wanted out of this new rotation, but as the integration has happened my worries have dissipated somewhat #physiotalk #nearlyputotalk
#OTalk @OTalk_
@_JoeMiddleton [REDACTED USER] Why - because occupational therapy is a complex intervention. Understanding the impact on occupations, depends on many factors, these models allow us to work out a persons occupational performance abilities (functioning) using standardised tests and assessments. #PhysioTalk
@_JoeMiddleton
[REDACTED USER] Competencies and training are the easy bits! Embedding the new way of working and getting through all the bumps in the road, now that it is tough bit #physiotalk
#OTalk @OTalk_
@_JoeMiddleton [REDACTED USER] Many other professionals would have standardised assessments, psychologists have many #PhysioTalk
Stella Krain @krain_stella
[REDACTED USER] [REDACTED USER] Yes we also work closely with Frailty in ED/AMU. We have a daily MDT with Frailty and social work. Team working is so important as well as the generic OT/ PT role. #physiotalk
#OTalk @OTalk_
Don’t forget those hashtags people. Otherwise others won’t see what you’ve said #PhysioTalk
Janet Thomas @JanetThomas47
@ChloeDicksonOT [REDACTED USER] Just another musing Do you think this difficulty is increased if we are wearing the same uniform? In Scotland we wear same colour and only defined by small embroidered name. #physiotalk
Barbara Ann Smith @Babsyslapsy
[REDACTED USER] Our service initially had a minor crossover between Phyio, OT and Nursing. We all carried our pre rehab assessment ISWT/6MWT, spirometry and PR education@. #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@ChloeDicksonOT [REDACTED USER] This is something I’ve worried about, esp as OT is a profession people don’t always understand and we work hard to promote our profession and get our roles known among the gen public, but this may be confusing things slightly #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] Staff need to understand what is being asked of them. Crossover & collaborative working isn’t about ‘becoming’ the other profession it’s about being mindful of what the other profession will need in terms of information & assessing tasks that are ‘needed’ #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
RT @BurchRhona: [REDACTED USER] Staff need to understand what is being asked of them. Crossover & collaborative working isn’t about ‘becoming’…
#OTalk @OTalk_
[REDACTED USER] Interesting I would still set goals, but my standardised occupational therapy assessment allows me to understand the persons occupational performance, and therefore how to target the goalsetting? #PhysioTalk depending on their functioning level.
Stella Krain @krain_stella
@ScottBuxton_1 [REDACTED USER] [REDACTED USER] @OTalk_ Even though our therapists are generic, they also have good supervision with a supervisor from the same profession. There are also specific OT and PT meetings and we are emailed profession-specific info. I have kept my OT identity. #physiotalk
Rhona Burch @BurchRhona
@ChloeDicksonOT [REDACTED USER] I think in some respects it might not matter. If you can explain to patients we are all part of the therapy team/rehab team & have created shared therapy goals with the patient does it matter who carries out those particular cross over tasks? #physiotalk
#OTalk @OTalk_
@skelton_fiona [REDACTED USER] Do they all complete the exact same thing, A part of one assessment. I the occupational therapist would look at occupational performance and functioning ability? #PhysioTalk
@_JoeMiddleton
Your first challenge is to get the wider healthcare community to understand what the 'Occupation' in Occupational Therapy means. (and not be referred to as occupational health). #physiotalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@BurchRhona @ChloeDicksonOT [REDACTED USER] Very true. Do people introduce them self in that way in your team? In ours I think we all introduce ourselves as OT/PT and don’t necessarily explain that we are doing blurred roles #physiotalk
#OTalk @OTalk_
[REDACTED USER] I’m very good at spotting those that don’t use the hashtag. During #OTalk we have a naughty step. For such things. #PhysioTalk
Rhona Burch @BurchRhona
@HolleighLouise @ChloeDicksonOT [REDACTED USER] We would likely say along the lines of “hello my name is Rhona and I’m one of the physiotherapists working on the rehab team for the ward, this is Kirsten [REDACTED USER], she’s one of the occupational therapists here. We/I was hoping we could...etc #physiotalk
#OTalk @OTalk_
In occupational therapy, occupations refer to the everyday activities that people do as individuals, in families and with communities to occupy time and bring meaning and purpose to life. Occupations include things people need to, want to and are expected to do. #PhysioTalk
#OTalk @OTalk_
Physiotherapy helps to restore movement and function when someone is affected by injury, illness or disability. It can also help to reduce your risk of injury or illness in the future. It takes a holistic approach that involves the patient directly in their own care. #PhysioTalk
#OTalk @OTalk_
They are very different professions, how would others define the difference? #PhysioTalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@BurchRhona @ChloeDicksonOT [REDACTED USER] [REDACTED USER] My OT heart loves that. Might suggest something similar to my team 😊 at the moment our team is basically “hello I’m x, I’m x profession, let’s look at your equipment and mobility” #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] I think some ‘tribal-ness’ is good! We all worked very hard to become the profession we chose & we should protect & promote it as much as possible but that’s not what crossover is- it’s sharing the tasks that possibly neither tribes perceives to exclusively ‘own’ #physiotalk
Mary Rose Mulry @MulryMary
[REDACTED USER] Respect for each profession and education on the differences between OT and PT would help! A clear model of practice is needed to maximize the positive effectiveness of these roles. #physiotalk #otalk
Holleigh Bryan 💚🏴󠁧󠁢󠁷󠁬󠁳󠁿 @HolleighLouise
@ScottBuxton_1 @BurchRhona @ChloeDicksonOT [REDACTED USER] I agree, I would introduce myself as an OT, but I feel maybe there should be a discussion to pts about the blurred roles, so that we’re not backtracking on the work done by RCOT to increase public awareness of what OT is #physiotalk
Rhona Burch @BurchRhona
[REDACTED USER] [REDACTED USER] A lovely tweet 😊 Respect is paramount- for your own profession & the other. We can share some skills & tasks without devaluing those that make us the professional we trained to be. We should enjoy the learning & be able to feel safe & defined in our roles #physiotalk
Stella Krain @krain_stella
[REDACTED USER] [REDACTED USER] In our CHAT team we draw on the experience and knowledge that our OTs / PTs have gained from working in specific areas beforehand. It is helpful if B5's can gain experience and be rooted in their profession. They then have more to bring to a generic /integrated role. #physiotalk

[REDACTED USER] Easy to go on courses, but it is important to have supervision and check competencies. #physiotalk
Rhona Burch @BurchRhona
@ScottBuxton_1 @HolleighLouise @ChloeDicksonOT [REDACTED USER] I agree re defining who you are to the pt & would always say I am a PT but I’m more than happy to say I’m part of the therapy/rehab team. I like our patients to know they have a whole team looking after them & feel reassured that we talk & have their interests at ❤️ #physiotalk

@ChloeDicksonOT @HolleighLouise @ScottBuxton_1 [REDACTED USER] I think then it’s the job of us all to promote & share our own & each others’ roles. We are trying hard to raise the profile of all AHPs just now with @NHSGGCClydeAHP as together is when we are strongest & work best for the patients. #GGCAHP #oneteam #physiotalk
