@physiotalk Most of our patients know and want to go home as soon as possible, and as therapists I feel we try and facilitate our patients home in a timely manner. Not all the time is this achievable #physiotalk

@physiotalk Start MDT d/c planning from point of admission (or pre-admission if possible). Gather info on what patient wants, social challenges, fears and concerns, areas for optimism etc as well as physical assessment. Set the MDT and patient a SMART goal for d/c as a challenge #physiotalk

@physiotalk It can be difficult to balance patient wishes, family wishes and patient safety. Don’t be afraid to disagree with other professionals if you truly feel the decision being made is wrong. Get your history, sort your patients goals, reason the decision, discuss with MDT. #physiotalk

@JoFallsPhysio @physiotalk If the patients want them involved then yes. Include in pre-admission assessment if possible. Aim to assess them day one and meet family/carers at visiting times. If not then call. Give them a role to play in the process. The MDT isn’t just the staff on the ward #physiotalk

@physiotalk I personally feel like capacity is a bit of a fluffy area and something as a junior, I would welcome more teaching about. But other issue is different professional opinions about where the best discharge destination for a patient is. #physiotalk

@physiotalk Space issues: work around it and adapt. It’s the patients own home & we can suggest that less clutter would make them safer etc but they also can choose (if they have capacity) to make unwise decisions. Often kitchens are good places for exercise #Physiotalk

@physiotalk I think with most barriers it’s about picking up the phone and explaining! I often get questioned why I need an assessment at home or why I need a favour to support a discharge and am very rarely disappointed by the response even if outside the criteria! #physiotalk #teamwork

@physiotalk Not normally...there are blocks of flats I avoid after dark though being completely honest. And today I was advised to take a male.colleague to a home after 80 year old husband was aggressive towards a male carer...he was delightful to us. But normally I don't worry #physiotalk

@physiotalk I think there does need to be more crossover between acute and community but hard when most trusts are desperate! I get frustrated at times (probably unfairly) when a patient gets sent to ED by a therapist with no acute medical need but know sometimes no other option #physiotalk

@physiotalk My most difficult parts came with crises calls on a Friday afternoon and the difficulty getting hold of people/equipment/services before the weekend. Only managed really with true 7 day working from all services/teams and adapting to manage overnight if possible #physiotalk

@ellena_farrow @physiotalk Definitely!! We all recognise that assessing mental capacity is everyone’s responsibility but the decisions that are being assessed are of such high risk at times this can be a lot to ask of less experiences colleagues #physiotalk

@JoFallsPhysio @physiotalk This....a colleague had an off colour patient on Friday pm.. GP surgery not answering. Eventually got through and was told.would triage call by 5pm...no call so 111...ended up ambulance to ED when a gp review may have solved issue #physiotalk

Long even corridor and physio steps v diff to lumpy carpet and spiral staircase...and then there's the tricky little threshold to.negotiate into the bungalow they didn't need steps for....#physiotalk

@physiotalk Picking up the phone, coming and seeing how each other work (ideal world with perfect staffing numbers), joint education, collaboration on outcome measures, patient feedback so many ideas...but most of all learning how each other work #physiotalk

@physiotalk A guarantee of a very thorough induction period with joint sessions, regular supervision, regular teaching. The ability to talk to a current band 5 in community work prior to starting or applying for jobs may help to ease anxieties too. #physiotalk

#physiotalk . In Bonteheuwel 5 year-old Damian & his other-mother loved to chat while she cooked. We sat him on one of my 200mm foam blocks to lift him to her level but he still needed foot support. Gran opened the old cutlery drawer @ his feet & popped his feet in there Perfect!

Patient despite "feeling better" had actually not improved ROM and was able to query with chiro and patient and therapist then know where to focus - outcomes based treatment
