physiotalk @physiotalk
Ok - lets get going with question one.... #physiotalk
Ruth ten Hove @RtHphysio
Hi Ruth @RtHphysio hello everyone #physiotalk
physiotalk @physiotalk
Just to have a look first at how wide #FirstContactPhysio is … Does your local area have a FCP service? #physiotalk
physiotalk @physiotalk
And if it does, can you tell us a little about it! #physiotalk
Laura Finucane @LaurafinucaneB
@physiotalk we are currently running a pilot #FirstContactPhysio #physiotalk
Eily @eilymcc
@physiotalk Good morning from Melbourne, on my way to work on the train 🌎 #physiotalk
Scott Buxton @ScottBuxton_1
@_lewadam @physiotalk Don't just lurk! Your questions & contributions will be just as valuable as everyone else! #physiotalk
Mari @mariand84
@physiotalk In Sweden many patients go to a physiotherapist first if problems with the muskuloskeletal system #physiotalk
physiotalk @physiotalk
@LaurafinucaneB Can you give us any details - just started?? #physiotalk
Amanda Hensman-Crook @AmandaHensman
@LaurafinucaneB @physiotalk I do a FCP service at 2 practices and West Cumbria is a pilot site #physiotalk
Laura Finucane @LaurafinucaneB
we have 3 FCP's working in 2 GP practices 4 sessions per week , 20 minute appointments #FirstContactPhysio #physiotalk
Cathy Thomson @cathythomsonPT
@physiotalk I work at Guildford. Currently looking into developing FCP #physiotalk
physiotalk @physiotalk
@mariand84 And is that for diagnosis, investigations as well as treatment? #physiotalk
Laura Finucane @LaurafinucaneB
@AmandaHensman @physiotalk evening! #physiotalk #FirstContactPhysio
Lewis Lovegrove @_lewadam
@ScottBuxton_1 @physiotalk Thank you Scott. I am sweeping for @physiotalk as well - so will engage where possible. #physiotalk
Scott Buxton @ScottBuxton_1
@physiotalk Just to help quantify things... how big is a local area? I'm aware of services in the county / neighbouring counties but am not near enough to know much about it #physiotalk
physiotalk @physiotalk
@ScottBuxton_1 Self defined! :) #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk No idea! It difficult to know who is pushing this agenda (not that I don’t think it’s a good agenda) is it administrators seeing the benefit of physio therapy or is it driven by physiotherapists wanting to do more! #physiotalk
Melissa McDonnell @Mel_McDonnell7
@physiotalk We already run a predominantly self-referral approach without seeing a GP in our area but are just starting to trial an FCP service #physiotalk
physiotalk @physiotalk
@ScottBuxton_1 In your NHS trust or board area perhaps - if that helps... #physiotalk
Scott Buxton @ScottBuxton_1
@LaurafinucaneB Laura, how long is each session typically? In my head i make it an AM or PM session of 4 hours? #physiotalk
Laura Finucane @LaurafinucaneB
@physiotalk running for a month seeing a variety of MSK conditions but dealing with the wider issues as well, some serious pathology picked up too #physiotalk #FirstContactPhysio
physiotalk @physiotalk
@sallybanerjee #physiotalk
physiotalk @physiotalk
@fiona_rough #physiotalk
physiotalk @physiotalk
So it seems like mainly at a pilot or 'just getting started' stage for many... #physiotalk
Eily @eilymcc
@physiotalk Within private practice, a physio will often be working as a primary contact. In Australia, a significant proportion of the physio community work in private practice and are often first to meet the patient #physiotalk
physiotalk @physiotalk
Just reminder to all to use the hashtag please! #physiotalk

Laura Finucane @LaurafinucaneB
@ScottBuxton_1 yes session is 4 wish hours an am or pm #physiotalk #FirstContactPhysio
Mari @mariand84
@physiotalk Diagnosis and treatment #physiotalk
physiotalk @physiotalk
@_JoeMiddleton I think that is a key question - anyone have an answer for Joe? #physiotalk
Melissa McDonnell @Mel_McDonnell7
@_JoeMiddleton The FCP is in place within the GP practices so that if a patient calls to book an appt with a GP for an MSK related issue they’re instead booked with a physio. Useful for those that struggle to acces self-referral. #physiotalk
Scott Buxton @ScottBuxton_1
@eilymcc @physiotalk It's really interesting hearing this as I think it clarifies the issues with clarity around the name of FCP. #physiotalk
Eily @eilymcc
@physiotalk Advanced practice is a growing area in Australia too, within the public health sector. Physio may be the first to see a patient in ED setting. Also physios working alongside consultants in OAHKS and neurosurg clinics... Though on referral from GP potentially? #physiotalk
Katie Knapton @physiokatiek
@physiotalk #physiotalk what outcome measures are people using to measure success/satisfaction levels
Laura Finucane @LaurafinucaneB
@AmandaHensman @physiotalk we used @AmandaHensman to mentor our FCPs before they got going which was a great insight to what they were letting themselves in for! #physiotalk #FirstContactPhysio
physiotalk @physiotalk
@physionewlands #physiotalk
physiotalk @physiotalk
So - onto our next question... #physiotalk
Euan McComiskie @EMAHPInfo
@physiotalk Hi all. Joining from Edinburgh #physiotalk
Amanda Hensman-Crook @AmandaHensman
@LaurafinucaneB @physiotalk Mentorship vital for new services #physiotalk

Q: What are the benefits of providing a FCP service? #physiotalk

RT @LaurafinucaneB: @AmandaHensman @physiotalk we used @AmandaHensman to mentor our FCPs before they got going which was a great insight to what they were letting themselves in for! #physiotalk #FirstContactPhysio
physiotalk @physiotalk
@bike2run @_JoeMiddleton #physiotalk
Scott Buxton @ScottBuxton_1
@physiotalk @_JoeMiddleton In my mind a FCP is an alternative to a GP as in the expectation of the patient is that they receive the same standard of care and help to same accountability. #physiotalk
C McManus @bike2run
@physiotalk @_JoeMiddleton #physiotalk my bad
physiotalk @physiotalk
#physiotalk
Laura Finucane @LaurafinucaneB
@eilymcc @physiotalk I think it is important to be clear about the different between first contact physio and first contact practitioner, this is often perceived as the same and it isn't #physiotalk #FirstContactPhysio
Joe Middleton @_JoeMiddleton
So, if I saw a FCP physiotherapist, they would potentially have to refer me to a physiotherapy service for ongoing management? #physiotalk
Amanda Hensman-Crook @AmandaHensman
@physiotalk Pts get seen by the right person right place first time, rewarding job, chance to be part of primary care MDT, part of the community, chance to upskill to Advanced practice. What not to like? #physiotalk
KB @KBNeuroinfo
@physiotalk Care navigation, seeing the right person at the right time. Appropriate referral and signposting #physiotalk
Katie Knapton @physiokatiek
@physiotalk I can see it freeing up GP time far better for MSK presentation #physiotalk
physiotalk @physiotalk
@LaurafinucaneB @eilymcc Can you explain? The CSP do seem to say that a FC practitioner if a physio, should be called a #FirstContactPhysio Or that's how I read their guidance! #physiotalk
physiotalk @physiotalk
RT @AmandaHensman: @physiotalk Pts get seen by the right person right place first time, rewarding job, chance to be part of primary care MDT, part of the community, chance to upskill to Advanced practice. What not to like? #physiotalk
Euan McComiskie @EMAHPInfo
@physiotalk @_JoeMiddleton #FirstContactPhysio is more than just quick access to physio. Depending on individual training it can offer injection, prescribing, direct referral to orthopaedics, neurology or other services and more as well as physio advice. It adds to the offer of primary care #physiotalk
Laura Finucane @LaurafinucaneB
@_JoeMiddleton or not, the idea is to try to get the patients to self manage where appropriate #physiotalk #FirstContactPhysio
Scott Buxton @ScottBuxton_1
@physiotalk @LaurafinucaneB @eilymcc Excellent question. To build on this can a physio (or FCP) be called an FCP if they work in a setting where GPs also work - take an urgent treatment centre as an example. #physiotalk
Natalie Beswetherick (She/her) @natbeswetherick
Having visited @AmandaHensman practice in Cumbria I saw all these benefits for patients #PhysioTalk #FirstContactPhysio
Ian Tyrrell @IanTyrrell5
@ScottBuxton_1 @physiotalk @_JoeMiddleton Is the FCP title just a bit of a smokescreen. Paramedics, nurses and other AHP’s see people in GP surgeries. Does the FCP need to focus on MSK or do they do everything. If so who streams them the msk issues (which may not be msk issues) it’s that what confuses me. #physiotalk
Fiona Rough @fiona_rough
@physiotalk #physiotalk #firstcontactphysio Shift Capacity from GPs. MSK patients seen by right person at the right time. Our data also demonstrates reduced rates for imaging, medication, and onward referral rates to secondary care services.

Corinne Birch @PhysioCorinne
@physiokatiek @physiotalk #physiotalk hi Katie outcome measures are tricky as most patients are only seen once (75% patients managed in one appt). At the moment pilot data is being collected to see how patients are managed. We have been using PSQs which show consistent excellent satisfaction
Scott Buxton @ScottBuxton_1
@IanTyrrell5 @physiotalk @_JoeMiddleton Nail. On. Head. This has been my issue all along! I am fearful that #FCP is being used as a branding exercise and potentially neglecting other areas of physio practice which can be the foundation within these roles. #physiotalk
Cathy Thomson @cathythomsonPT
@LaurafinucaneB @eilymcc @physiotalk Common misconception IME. Important to get management on board with FCP set-up with required level of clinical experience & training #physiotalk

Simon Ingram @simon_ingram13
@Mel_McDonnell7 @_JoeMiddleton FCP in GP practice also means working as part of the primary care team. Enables upskilling across professions and better understanding of complex care pathways #physiotalk #firstcontactphysio
Amanda Hensman-Crook @AmandaHensman
@IanTyrrell5 @physiotalk @ScottBuxton_1 @_JoeMiddleton FCP advanced practice role, we also have an FCP paramedic, dieticians are developing their skills, also have developed FCP frailty not just MSK #physiotalk
Corinne Birch @PhysioCorinne
@AmandaHensman @LaurafinucaneB @physiotalk #physiotalk How cam we set up local mentorship / peer support ?
Euan McComiskie @EMAHPInfo
@IanTyrrell5 @ScottBuxton_1 @physiotalk @_JoeMiddleton You don’t need to be a physio to be a first contact practitioner. The current MSK model allows for others with appropriate skills to fulfill role. We hope in future that the FCP role will be open to other specialties (frailty, respiratory etc as well) #physiotalk
Katie Knapton @physiokatiek
#physiotalk
Laura Finucane @LaurafinucaneB
@physiotalk @eilymcc think its ok to call themselves a physio but the role is not seeing patients in same context - they are not being referred by GP for physio. Its about self management , signposting, identifying other issues etc #physiotalk #FirstContactPhysio
physiotalk @physiotalk
So - in the confusion of what a #FirstContactPhysio might be... Lots of mention of being an advanced practitioner So onto our next question... #physiotalk
Joe Middleton @_JoeMiddleton
Good question. So far, a First Contact Physio (not practitioner) seems to work in primary care as an alternative to a GP, seeing MSK complaints but otherwise doing everything (and more that a GP can). Am I on the right track here? #physiotalk
Ian Tyrrell @IanTyrrell5
@EMAHPInfo @ScottBuxton_1 @physiotalk @_JoeMiddleton So where does this FCP work sit amongst the ACP work? Are they the same? Different? Related? #confused #physiotalk
physiotalk @physiotalk
@PhysioCorinne @physionewlands #physiotalk
Amanda Hensman-Crook @AmandaHensman
@PhysioCorinne @LaurafinucaneB @physiotalk Encouraging local FCPs to make a buddy system e.g. maybe Skype call for 20 mins a week, get yo know of her FCP services in the next county and arrange meetings, regional FCP events, asking experienced FCP to help #physiotalk
Sally Banerjee @sallybanerjee
@physiotalk #physiotalk quicker access to msk practitioner reduce load on gps improve referrals onto primary and secondary care services
physiotalk @physiotalk
Q: Are the core capabilities sufficient and robust? #physiotalk https://t.co/llEsPws7Tq
Euan McComiskie @EMAHPInfo
@jobK2 @physiotalk For now @jobK2 but hopefully extending to other specialties soon. If we can prove the positive impact of the MSK model of #FCP then we can explore others #physiotalk
Scott Buxton @ScottBuxton_1
@AmandaHensman @IanTyrrell5 @physiotalk @_JoeMiddleton That's news I like hearing Amanda. Can I ask why the CSP haven't been so loud in shouting about non-msk FCPs? I presume it is for clarity of message? #physiotalk
physiotalk @physiotalk
RT @IanTyrrell5: @EMAHPInfo @ScottBuxton_1 @physiotalk @_JoeMiddleton So where does this FCP work sit amongst the ACP work? Are they the same? Different? Related? #confused #physiotalk
physiotalk @physiotalk

Amanda Hensman-Crook @AmandaHensman
@LaurafinucaneB @physiotalk @eilymcc Not hands on physio, assessment diagnosis investigation case management on referral signposting in to all services managing complex co morbidity #physiotalk
C McManus @bike2run
@physiotalk @LaurafinucaneB @eilymcc Considering what patients remember from an interaction, I think it’s important that they FIRST recall that they have seen a physio, then Advanced Practitioner/First Contact or whatever is a bonus if you want to get that in #physiotalk
Joe Middleton @_JoeMiddleton
Hang on. Is the amazing working being done by @thecsp about First Contact Practitioners or First Contact Physios, and are they different things? #physiotalk
Scott Buxton @ScottBuxton_1
@IanTyrrell5 @EMAHPInfo @physiotalk @_JoeMiddleton Got to be the same. Surely. Must be. Definitely are. Maybe? #physiotalk
Eily @eilymcc
@ScottBuxton_1 @physiotalk @LaurafinucaneB Interesting to learn about the differences between the UK and Australian public health care settings. Many GP clinics in Aus will have a physio on site too who can be seen independently from GP (primary contact) or on referral from GP. #physiotalk
physiotalk @physiotalk
@LaurafinucaneB @eilymcc Do you think it is clear to patients if they DON'T call themselves a physio? #physiotalk
physiotalk @physiotalk
@HELLSBELLS_DFYB #physiotalk
Amanda Hensman-Crook @AmandaHensman
@physiotalk They are and marry up to Advanced practice competencies #physiotalk
Euan McComiskie @EMAHPInfo
@physiotalk @IanTyrrell5 @ScottBuxton_1 @_JoeMiddleton Linked but not the same and none superior to the other. #FirstContactPhysio roles include some aspects of ACP but there are lots of other ACP tasks not expected of FCPs. Not sure if my explanation is helping or hindering?! @sarahwithers20 will be better to explain! #physiotalk
Laura Finucane @LaurafinucaneB
@ScottBuxton_1 @IanTyrrell5 @physiotalk @_JoeMiddleton need to consider scope of practice, so yes focus would be on MSK but you are looking at the patient holistically, consider the multi morbidities etc. I see it as part of the patients journey, the front end & not neglecting our core physio, should be integrated #physiotalk
physiotalk @physiotalk

Ian Tyrrell @IanTyrrell5
@ScottBuxton_1 @EMAHPInfo @physiotalk @_JoeMiddleton So confused! Do FCP’s only work in GP surgeries? I would see no difference in either an FCP in go land to an ACP in frailty/respiratory. See patients independently under guidance, asses, treat,prescribe as required. Do we need different branding? #physiotalk
