@CarylAPhysio @AnneMarieLogan2 @physiotalk Absolutely, and how some people may expect them to be "normal" because they are "normal" some of the time, but can have horrendous flare ups of their condition at other times #physiotalk
physiotalk @physiotalk
And how do you address this perception? #physiotalk
Matt Dennies @MDennies_Physio
@CarylAPhysio @physiotalk Lack of adaptability in exercise prescription, a ‘one size fits all’ approach due to clinicians lack of understanding of long term conditions #physiotalk
Bex Townley @BexTownley

I think the pacing and timing of exercise is critical. Back to my patient group of migraine, there is evidence it helps in migraine but needs to be gradually built up. We can encourage patients with that. #physiotalk
Rachael Hinton @RHintonPT
@BexTownley @physiotalk Absolutely! I've known very disabled young people be asked to move! #physiotalk

@CarylAPhysio People may use all of their reserves just getting through the day and acting as if they’re fine, leaving nothing left for exercise. And if they are able to exercise, others may not understand why they may be limited in duration or intensity #physiotalk
Tina @LivingWellPain
@AnneMarieLogan2 Sounds right to me, I'm not sure that physical exercises for someone with a hidden disability is any different to those with a visible disability? #physiotalk
Caryl Hunter @CarylAPhysio
What are everyone's thoughts on this? Does the term disability have a different meaning for clinicians and patients? #physiotalk
Rachael Hinton @RHintonPT
@level7therapy Yes, perceptions and beliefs are a massive barrier! #physiotalk
physiotalk @physiotalk
@darrenabrown @KellyOBrien25 Do you use a framework like this? #physiotalk
Charlie Winward @level7therapy
Physical activity versus exercise? #physiotalk
Mari @mariand84
@CarylAPhysio @physiotalk if the physio is not aware of the hidden disability it can definitely pose a huge problem. For example prescribing exercises/rehab that an individual is not able to perform. Exercises/rehab must be tailored 4 the individual #physiotalk

Matt Dennies @MDennies_Physio
@mphysio99 @CarylAPhysio Great point, really important to educate patients about energy conservation and fatigue. If expecting them to turn up to exercise class/add exercise into their week, may require helping them organise their week around it. #physiotalk

Nearly half way through and some great chatting - and keep up the hashtagging! #physiotalk
Janet Thomas @JanetThomas47
@RHintonPT Nothing specific, but I think its probably those with multiple hidden conditions? Lots of my older patients have issues with eyesight, hearing, continence etc #physiotalk
Anne-Marie Logan @AnneMarieLogan2
@MDennies_Physio @mphysio99 @CarylAPhysio I agree. Often the patient's goals are more subtle than that. Leaving their bedroom, walking to get milk. I think the way we phrase encouraging activity has to be engaging and relevant #physiotalk
Mary @mphysio99
@CarylAPhysio @_lewadam Great idea! Routines and schedules can really help people to fit exercise into a routine, but if it’s not flexible enough, it doesn’t always allow for unexpected events which have the capability to create set backs #physiotalk
Lewis Lovegrove @_lewadam
@CarylAPhysio @mphysio99 Definitely. Also takes away potential pt guilt with them accepting all their effort has been put into exercising on a particular day, allowing them to rest and complete another task the following day. Kind of eliminates burn out of never really finishing daily tasks #physiotalk
physiotalk @physiotalk
I can't spell #physiotalk :)
Bex Townley @BexTownley
@MDennies_Physio @mphysio99 @CarylAPhysio I think part of this is bcs in leisure sector, 60 minutes is standard - this doesn’t have to be so #physiotalk
Tylah Marston @marston_physio
@JanetThomas47 @RHintonPT Totally agree! Especially noticed during my MSK Outpatients placement recently - so many people came in with one referred MSK problem, but had multiple other conditions either linking to presenting complaint, or often exacerbating their symptoms! #physiotalk
Caryl Hunter @CarylAPhysio
@BexTownley @MDennies_Physio @mphysio99 Do we need to educate pts on the use of pacing, or maybe how to approach that conversation with those around them? #physiotalk
Rachael Hinton @RHintonPT
Specific conditions or ages? What about continence/gynae issues? #physiotalk
Jo Kitchen @J0Physio
@RHintonPT @physiotalk #physiotalk Mental health conditions, in particular social anxiety when lots of exercise is social! Hearing and visual loss also makes it harder for people to manage groups
Lewis Lovegrove @_lewadam
@mphysio99 @CarylAPhysio 100%. It's then trying to factor in 'flexi time' to allow for such events? And reassuring the patient that such set backs will happen #physiotalk
Eily @eilymcc
@RHintonPT @physiotalk Oops #physiotalk
Caryl Hunter @CarylAPhysio

Caryl Hunter @CarylAPhysio
Is there one area that you find more difficult than others? #physiotalk
physiotalk @physiotalk

Matt Dennies @MDennies_Physio
@CarylAPhysio @BexTownley @mphysio99 I've found lots of patient's feel validated when fatigue is explained to them, they often say 'I can't wait to go and tell my family about this'. Definitely important for pt and their family/friends to understand it #physiotalk

Jo Kitchen @J0Physio
@physiotalk @RHintonPT Which is more of a problem then to manage long term conditions #physiotalk

@RHintonPT Instead of addressing the 'obvious elephant in the room' per se, more hidden disabilities require more time to discuss and find out what really limits them, which more than likely has a big impact on their rehab! #physiotalk
physiotalk @physiotalk
@AnneMarieLogan2 @CarylAPhysio And do they include exercise / physical activity?? #physiotalk
Rachael Hinton @RHintonPT
@darrenabrown @_lewadam @mphysio99 @AnneMarieLogan2 @CarylAPhysio @BexTownley @MDennies_Physio @HIVandRehab And the challenge of that is massive! https://t.co/gPixwpuGXZ #physiotalk

@J0Physio @RHintonPT @physiotalk Would you also say that patients may be less forward in addressing them? Or even knowing about our willingness to accommodate these? #physiotalk
Tylah Marston @marston_physio
@mariand84 @RHintonPT Exactly! We screen for CES as a routine red flag - so why not try to drop in the topic of pelvic floor and continence when discussing this during Ax? #physiotalk

@RHintonPT #PhysioTalk listening to and understanding the individuals needs, barriers, facilitators and personal goals ... shouldn't be different to any other aspect of rehab, but my patients say that our approach of understanding uncertainty is key https://t.co/0IF1QabImI

@CarylAPhysio @RHintonPT @physiotalk Yes- it might actually be no the follow up to a couple of UTAs or DNA that you find out #physiotalk
Anne-Marie Logan @AnneMarieLogan2
@physiotalk @CarylAPhysio No, covers all aspects of migraine management but explains the importance of exercise. Not possible to cover everything in 2 hours #physiotalk
physiotalk @physiotalk
@J0Physio @CarylAPhysio @RHintonPT Which means we must ask the question - do we discharge people who DNA without question? Have we asked why they didn't attend? #physiotalk

Tylah Marston @marston_physio
@CarylAPhysio @J0Physio @RHintonPT @physiotalk Totally! They may not see them as 'directly important' to their current complaint, or the impact it may have on them for rehab. Some patients may not see it as important to 'tell the physio' - how does this link in with movement and rehab? Key to addressing! #physiotalk
