Rachel Senior @RachelSenior13
@physiotalk Respiratory physio in Dorset. Interested in long covid and how I as a physio can help not harm #physiotalk
Dr Michelle Bull πŸ’™ @MichelleBull4
@physiotalk Hi all I'm Michelle from sunny Surrey 😎 #physiotalk
Ajin J Thomas @ajinjt

Hannah Malyon @HannahMalyon
@physiotalk Hi! Hannah from Berkshire. We are doing both virtual and face to face appointments #physiotalk
Francis @F_MagUidhir
@physiotalk In person, 6mwt, steer ramp tests and a lengthy discussion re: hx , meds and goals. 1 hour assessment #physiotalk
Francis @F_MagUidhir
@HannahMalyon @physiotalk Are patients mostly happy with the virtual appointments? #physiotalk
Jessica DeMars @BreathewellPT
@physiotalk Jessica in πŸ‡¨πŸ‡¦. Mainly virtual (60 min)but do some in-home (90 min) when there are Respiratory or cardiac concerns that may need further objective measures. Virtual is better for those with profound fatigue. #Physiotalk
Francis @F_MagUidhir
@physiotalk @HannahMalyon Good Q. Here in Switzerland we can't bill for virtual appointments which is frustrating during a pandemic! Would love to see it change. #physiotalk

@physiotalk MDT face to face assessments in pulmonary rehabilitation service #physiotalk
Hannah Malyon @HannahMalyon
@physiotalk @F_MagUidhir We mostly let the patient decide. We offer both. Unless it might be obvious from the referral which one is needed #physiotalk

@physiotalk We screen as part of the MDT/referral process . Standard tests though, CXR, ECG, Bloods including ferritin, BNP, DDimer if indicated. I then use really careful questioning to see if I need to try and advocate for my patients to have further investigations. #physiotalk
Dr Michelle Bull πŸ’™ @MichelleBull4
@darrenabrown @physiotalk Really interesting indeed #physiotalk
Jessica DeMars @BreathewellPT
@darrenabrown @physiotalk Agree Darren! Most people I have seen have had CXR +\- CT scan, PFT and echo/ECG; if not I help them get at least those done if SOB or tachycardia. I ask about HR (if they track it and notice changes). In person I check HR, SpO2 and BP. #physiotalk
Francis @F_MagUidhir
@physiotalk #physiotalk it is present in approx 90% of my patients. We don't use an objective measure. Do others use a standardised tool for this?
Dr Michelle Bull πŸ’™ @MichelleBull4
Really hoping the answer is yes on this one!! #physiotalk
Rachel Senior @RachelSenior13
@physiotalk I certainly try to! From my limited experience, sometimes it seems so obvious and sometimes very nuanced. #physiotalk

@darrenabrown @physiotalk #physiotalk I use four questions to determine mental or physical stress or fatigue after an effort that was previously easy. After first training session I ask pts to let me know how the next three days go. 90% report feeling fatigue. Not sure that constitutes PESE tho.
Hannah Malyon @HannahMalyon
@physiotalk Currently have gad 7 and phq 9. But will use relevant outcome measures based on symptoms when required. E.g. if patient is being mostly seen for BPD then we use appropriate to that. Open to suggestions. #physiotalk
Rachel Senior @RachelSenior13
@darrenabrown @physiotalk Which ones do they use #physiotalk πŸ€¦β€β™€οΈ

@Hartey_PT @physiotalk Goodness me no! At the Royal Berkshire the long covid service was born from the pain department, so maybe that is where thoughts of gad and phq came from. I have only been in post for 4 weeks. So learning as much as I can and growing as a team. #physiotalk
Francis @F_MagUidhir
@physiotalk @cathythomsonPT @darrenabrown Will reply tomorrow! Brain dead but the gist is ADLs that they now find difficult and how they currently cope with effort and consequences of exertion. #physiotalk
Hannah Malyon @HannahMalyon
@KWestPhysio @physiotalk Yes we do. #physiotalk
