physiotalk @physiotalk
@Csp_Eoe @ESNEFT @JoeCurtinIE Thank you for your suggestion for tonights topic! We are really looking forwards to the #LoveActivity chat tonight #physiotalk
Alistair Beverley @theLDphysio
@physiotalk OK, so not sure why my phone had created an account there, I'll try from my actual account. Hi, live in Nottingham, working in Mansfield with young people and adults with LD and PD, sometimes both, sometimes one or the other. #physiotalk #goodstart
Claire Campbell @ClaireC_PT
@physiotalk 👋🏻👋🏻 Claire in Edinburgh, working in a pain management service. #physiotalk
James Pearson @jimmypeo17
@physiotalk Evening all, I'm likely to be lingering tonight. Currently work in a persistent pain management service #physiotalk
physiotalk @physiotalk
Welcome everyone! Lets get started with question one - but don't forget those hashtags... #physiotalk

Q: What are your reflections on the challenges of encouraging inactive patients to become more physically active? #physiotalk
AHP Suffolk @AHPSuffolk
@physiotalk @Csp_Eoe Hi, a little late to the party. Mel from AHP Suffolk in Sudbury 👋 #physiotalk
Vineeth Ravindran @r_vineeth
@physiotalk Hello from the Scottish borders. I work with adult critical care+medical inpatients and outpatient cardiac rehabilitation #physiotalk
Becca Knowles @Becca_Knowles_
@physiotalk Hi! Rotational Physio in Suffolk currently working in paediatrics, but still on-call with older patients and treat adults in private work #Physiotalk
AHP Suffolk @AHPSuffolk

RT @physiotalk: Q: What are your reflections on the challenges of encouraging inactive patients to become more physically active? #physiot
Ian Tyrrell @IanTyrrell5
@physiotalk It can be a massive battle. But think we need to move on from paternalistic approaches and move towards lifestyle coaching, promoting self ownership etc to get real buy in! #physiotalk
Unity Physiotherapy & Wellbeing @UnityPhysio
@physiotalk Understanding the person, what they enjoy, what’s important to them, what motivates them, their communication, how they relate to exercise/physical activity all help #physiotalk
Alistair Beverley @theLDphysio
@physiotalk I'll start with the blooming hastage lest I forget it. #physiotalk For many guys with PD I find most have a desire to be active but find it impossible due to inaccessibility of society. Sports, gyms, steps, lack of role models are simple but big barriers.
Claire Campbell @ClaireC_PT
@physiotalk I see people who have usually been to a number of services, so I have the benefit of hearing what hasn’t worked for them (dosage, type of activity, the meaning behind it). For some people, the are not engaging in pADLs, so sometimes, they start there #smallchanges #physiotalk
Rachel Jackson @Rachelej1971
Hi, Rachel, working in a community MSK service in Staffordshire.#physiotalk

Janet Thomas @JanetThomas47
@IanTyrrell5 @physiotalk Yes - with my older patients can be a real struggle as they are frail and often quite deconditioned and fatigued #physiotalk

Emma Garratt @Em_Garratt
@physiotalk Sorry I’m late, Emma in Henley-on-Thames. I work at an inpatient stroke rehab unit. #physiotalk
James Pearson @jimmypeo17
@physiotalk Sometimes very challenging to dispel hard and firm beliefs around the patients understanding of their pain experience/journey #physiotalk
Unity Physiotherapy & Wellbeing @UnityPhysio
RT @ClaireC_PT: @physiotalk I see people who have usually been to a number of services, so I have the benefit of hearing what hasn’t worked…
AHP Suffolk @AHPSuffolk
@IanTyrrell5 @physiotalk I agree it can be a battle. But with the majority of patients if you can connect with them and find an activity they enjoy it can snowball from there #physiotalk
Janet Thomas @JanetThomas47
@theLDphysio @physiotalk Inaccessibility a huge issue for my patient population as well. Lack of accessible or affordable transport often #physiotalk
Alistair Beverley @theLDphysio
@physiotalk Other reasons, lack of therapist time, poor services, lack of therapist's interest in it themselves, referral/problem based intervention leading to episodic treatment #physiotalk
Sarah BF Marl @SarahBFraser
@physiotalk Inclusive marketing can really help. The messages have got to be right, through accessible, inclusive communications. That’s why @AllForActivity is working hard to provide guidance in this area. #PhysioTalk
Ian Tyrrell @IanTyrrell5
@JanetThomas47 @physiotalk Yup! But then promoting progress diaries, technology (Fitbit, Apple Watch) etc can we coach people to see that progress doesn’t have to be massive just has to be one more step in some cases! #physiotalk
Becca Knowles @Becca_Knowles_
@physiotalk Major barrier themes definitely the social isolation, transport issues and lack of confidence leaving the house independently #physiotalk
Vineeth Ravindran @r_vineeth
@physiotalk Creating a good example is emulative in the unit.Age is no barrier! #physiotalk
CSP PHYSIO EAST @Csp_Eoe
@ClaireC_PT @physiotalk At the end of the day, goal focused, person centered and lets patients regain their independence. I'm sure challenging, but very rewarding #Physiotalk

Alistair Beverley @theLDphysio
@AHPSuffolk @IanTyrrell5 @physiotalk Having the skillset to this in limited time is a challenge in itself. Certainly not impossible but I've seen physios miss the mark horrendously on building relationships #physiotalk
Samuel Hobdell @shobdell
@physiotalk Coming from Norfolk, major transport issues leading to social isolation and therefore lack of confidence out in social situations #physiotalk
Rachel Jackson @Rachelej1971
I find patients’ barriers to exercise are pain, fatigue, thinking they will do damage, reconditioning, lack of motivation, lack of knowledge as to what is available.#physiotalk
physiotalk @physiotalk
Starting to head towards this- so lets move towards our next question #physiotalk
Emma Garratt @Em_Garratt
@physiotalk In my experience accessibility is a big barrier. Even for the most highly motivated individuals, transport to/from locations or sufficient support once there often prevents them from participating #physiotalk
physiotalk @physiotalk
Q: What strategies do you use to promote activity in people living with a long term condition #physiotalk
Becca Knowles @Becca_Knowles_
@Csp_Eoe @ClaireC_PT @physiotalk This is spot on! 🙌🏼 #physiotalk
Alistair Beverley @theLDphysio
@jimmypeo17 @physiotalk This has to be more of our job in all physio services these days. Allow patients to have a positive association with movement #physiotalk
Claire Campbell @ClaireC_PT
#Physiotalk
physiotalk @physiotalk
#physiotalk
Jo Kitchen @J0Physio
@physiotalk Transport is difficult- once unable to drive them the physical effort of getting to a place like a leisure centre where they can be supported to exercise is difficult #physiotalk
Ian Tyrrell @IanTyrrell5
@theLDphysio @AHPSuffolk @physiotalk Yup! But remembering to ask #WhatMattersToYou should be the stand point! I think the more experience you gain the better you are able to build relationships! Being open and honest and personable is helpful too! #physiotalk
Claire Campbell @ClaireC_PT
@physiotalk Referring people to exercise classes, walking groups, gym referral schemes is challenging due to people’s perceived and real barriers. Finances, access and transport, understanding what goes on in a class and even the temperature of a pool can all put people off. #Physiotalk
CSP PHYSIO EAST @Csp_Eoe
@IanTyrrell5 @physiotalk Very much echoes the teachings of a Health Coaching Course I recently participated in @healthcoachwsh. Empower, explore accessibilty, and above all, find the barriers, which turn into 'why not'? #Physiotalk
Jo Kitchen @J0Physio
@physiotalk Fatigue also major factor- when just getting up dressed & food takes effort ++ then adding more activity is a challenge #physiotalk
Activity Alliance @AllForActivity
Our research shows that psychological barriers about ability and suitability can be the biggest overcome. Lack of confidence too. Our Ten Principles can help organisations and professionals to support disabled people to be more active. https://t.co/wua8nmdjQt #Physiotalk
Alistair Beverley @theLDphysio
@physiotalk Build relationships, trust, we work with people after all. From there you can find interests and build on shared goals #physiotalk
Janet Thomas @JanetThomas47
@physiotalk Often my patients can be more or less housebound - so its just getting them on their feet as much as possible during the day Sit to stands during the ad breaks is a start! #physiotalk
Emma Garratt @Em_Garratt
@J0Physio @physiotalk Totally agree. Post-stroke driving can be off limits, for many long term. Public transport is so restrictive and many may not be able to manage this alone which then introduces another barrier of needing to be accompanied! #physiotalk

AHP Suffolk @AHPSuffolk

Mark @MarkphysioNg
@physiotalk Good evening, my name is Mark... I'm from Nigeria I work in the surgery unit of physio department of lagos University teaching hospital... #physiotalk
Philip Hulse @HulsePhilip
@IanTyrrell5 @JanetThomas47 @physiotalk I know it's becoming a bit clichéd but shared decision making really does empower patients and prevents the parent child relationship from taking hold. It can be a challenge though not to step in with well I think you should...That's where #socialprescribing is best. #physiotalk
AHP Suffolk @AHPSuffolk
@theLDphysio @IanTyrrell5 @physiotalk And I realise having plenty of time doesn’t solve everything, as you say skill set is key too. #physiotalk
Ian Tyrrell @IanTyrrell5

Graeme Paul-Taylor @GraemeGpt
@physiotalk Introducing people to communities that can support change, & helping people to make choices / pursue goals key #physiotalk
Alistair Beverley @theLDphysio
@JanetThomas47 @physiotalk Start with small steps and realistic goals, yes #physiotalk
Jo Kitchen @J0Physio
@theLDphysio @physiotalk Therapist knowledge also needs time and investment, both things like new machines at the gym and local options. If I haven’t been to an exercise class I’m hesitant to recommend as it could be inappropriate #physiotalk
James Pearson @jimmypeo17
@physiotalk Looking to establish their values/interests around pain, sometimes asking 'if we had a silver bullet what would you want to get back to and Why!?' This might be a useful place to 1start, but sometimes patients just tell you in the narrative if given enough time #physiotalk
physiotalk @physiotalk
@ClaireC_PT @IanTyrrell5 @JanetThomas47 Have you used this tracking at all with patients? #physiotalk
CSP PHYSIO EAST @Csp_Eoe
A brilliant resource to look at following our #Physiotalk to help people with disabilities to be more active.
Becca Knowles @Becca_Knowles_
@physiotalk Working with well established community services (NHS, charities, leisure centres etc) which have long-term capacity and groups so individuals get physical activity and social interaction #physiotalk
Alistair Beverley @theLDphysio
@J0Physio @physiotalk We definitely have a responsibility for this. I fear there are a number of physios who may not be doing this. We need to keep up with the evidence #physiotalk
physiotalk @physiotalk
RT @physiotalk: Q: What strategies do you use to promote activity in people living with a long term condition #physiotalk
Jo Kitchen @J0Physio
@physiotalk I try really hard to make it sustainable long term. Things they enjoy, maybe stretches/gentle ROM/bed mobility, a group option and a more high intensity home option so stays interesting long term #physiotalk
Claire Campbell @ClaireC_PT
@J0Physio @physiotalk Depending on the cause of the fatigue, increasing activity may not have any impact. Graded activity - building slowly and steadily with a good set back plan can help with confidence, better sleep, better QoL #physiotalk
physiotalk @physiotalk
@Becca_Knowles_ Do you find capacity is an issue? Especially with local referral schemes #physiotalk

AHP Suffolk @AHPSuffolk
@physiotalk Goal setting is first on the list and identifying barriers to introducing activity. Sometimes it’s not pain but the worry that exercising will make them structurally worse or eventually negatively influence pain. Can’t assume everyone has the same concerns #physiotalk
Rachel Jackson @Rachelej1971
I try & inform patients of simple functional exercises & of local activities that maybe pertinent to them. #physiotalk
Spotsylvania Med Ctr @spotsylvaniamed
@physiotalk A: Challenges tend to stem from fear because patients aren't already exercising, as well as pain and a lack of knowledge about benefits and where to begin. #PhysioTalk
Unity Physiotherapy & Wellbeing @UnityPhysio
@physiotalk Listening, finding out things like: values and motivating factors, what they enjoy or used to enjoy, what they struggle with and what’s easy, what barriers there are, if they like structure or not to activities, if they they the social aspect of activities #physiotalk
Aimee Robson @AimeeRobson4
@physiotalk I’m no longer in clinical practice 😢 but in recent years found @gretchenrubin and her work around habits. I found actually this applies to patient care too and #activation - an approach based around tendencies of how we form habits #physiotalk https://t.co/YmmvlnygaF
Emma Garratt @Em_Garratt
@J0Physio @theLDphysio @physiotalk Having a decent up to date resource pack of known good places for onward referral is really helpful. Also useful to have as a handout for patients/relatives. Our band 5 put this together for us. But requires times and effort to keep up to date! #physiotalk
Philip Hulse @HulsePhilip
@physiotalk find something they like. The #escapepain model seems to have been a massive help in my #msk service @RJAH_NHS due to peer support. Running the groups out of the clinical setting in a community hall or leisure centre really helps people manage their own condition.#physiotalk

Becca Knowles @Becca_Knowles_
@physiotalk For some services capacity is definitely an issue, less so the self-funded groups but there are so many services/groups etc which are poorly advertised we/patients don’t know about and capitalise on - part of the battle is establishing those connections! #physiotalk #community
CSP PHYSIO EAST @Csp_Eoe
@AHPSuffolk @physiotalk What can we do as a profession/AHPs to help lighten these common themes among our patients? #Physiotalk
Janet Thomas @JanetThomas47
@J0Physio @Em_Garratt @theLDphysio @physiotalk Do you check the qualifications of the instructor? At least then you know the class is appropriate for your level of patient? #physiotalk

Claire Campbell @ClaireC_PT
@physiotalk It depends what their values and goals are. I try to make activity fun, but acknowledge the unintentional activity they do (playing with children, walking for transport, etc) and try to enc them to be more mindful of the health benefit of doing more of that. #physiotalk

Ian Tyrrell @IanTyrrell5
@physiotalk I’ve tended to started to explore the psychosocial aspects more with the long term conditions. Often stress, mental health, other things can manifest as physical symptoms, need to be able to dig that little bit deeper! #physiotalk
Claire Campbell @ClaireC_PT
@Becca_Knowles_ @physiotalk This is a big issue for me - I work across 3 councils with a mix of towns and rural. I get my head around one timetable and then they release a Summer version!! #Physiotalk
