physiotalk @physiotalk
Q What do you understand by the term ‘Social prescribing’ #physiotalk
Louise Brennan @Louise_Brennan_
@physiotalk @Hartey_PainPT @spiteri_anna @_lewadam Hello #physiotalk-ers! 😀

Aideen Larmer @AideenPhysio
@physiotalk Linking patients with community based initiatives that will help improve their health #physiotalk
Anya Lotay @AnyaPhysio1417
@physiotalk on acute medical rotation: I see it as suggestions and referrals made to teams working within the community that a patient can access in their own environment..eg: community therapy, exercise classes, volunteer befriending services, AGE uk, social services etc. #physiotalk
physiotalk @physiotalk
@mariand84 It is generally referral on to non medical or community based schemes. So could be gardening clubs for example... #physiotalk
Unity Physiotherapy @UnityPhysio
@physiotalk What Helen has already mentioned as well as building confidence with social activities they can then continue eg going with someone to an exercise class #physiotalk
Janet Thomas @JanetThomas47
@PamKHphysio @physiotalk So so pleased to hear that after having known the issues when at #WCPT17 #physiotalk

Naomi McVey 🐝 @NaomiMcVey
RT @physiotalk: Q What do you understand by the term ‘Social prescribing’ #physiotalk
Helen Harte @Hartey_PT
@physiotalk Overcoming barriers to social engagement through co-ordinated community and social response to reduce isolation #physiotalk
Louise Brennan @Louise_Brennan_
@physiotalk Hi all! Tweeting from the @StanstedExpress this evening! Always great to be back in the UK 🚂#physiotalk
Helen Caldwell @caldwell_pt
@UnityPhysio @physiotalk agree - there's a lot to be said for connecting people. Loneliness is unfortunately a prevalent thing #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Well I’ve learnt something new already. I never considered a referral to befriending or age UK to be social prescribing, although not sure what I thought it was to be honest! No I realise that I do quite a bit of that! 🤦‍♂️🤦‍♂️ #physiotalk
physiotalk @physiotalk
So - lets start a #socialprescribing list What can you access locally to refer onto? #physiotalk

Naomi McVey 🐝 @NaomiMcVey
@physiotalk Processes & systems to proactively link people with local groups, activities & schemes (usually community & voluntary) that help to improve health and wellbeing #Physiotalk
physiotalk @physiotalk
The definition seems to be based around 'non clinical' ot sure from reading ahead of tonight how much the definition does or doesn't encompass activity or exercise referrals.. #physiotalk
Helen Caldwell @caldwell_pt
@physiotalk We have Age UK, the council run otago classes, our local pharmacy run a lunchtime walking group #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Revival: one stop shop for minor house adjustment, benefits checks, key safe and lifeline provision, shopping support, befriending. Even offer a taxi service home for mobile vulnerable people. #physiotalk
Mari @mariand84
@physiotalk Ok. I can recommend a patient to go to a café for seniors in order to get to know a walking or gym buddy. But that is about it. #physiotalk
Naomi McVey 🐝 @NaomiMcVey
@IanTyrrell5 @physiotalk I think social prescribing takes to a different level in terms of being systematic in identifying people, services, needs etc - sometimes with link roles etc #physiotalk
physiotalk @physiotalk
On to question two... #physiotalk
Helen Caldwell @caldwell_pt
@IanTyrrell5 @physiotalk Oh yeah we have those things too! Forget about how much there is actually. The fire Service run a same home check too #physiotalk
Anya Lotay @AnyaPhysio1417
@physiotalk community therapy: 2-6 weeks waiting list, age uk/age concern numbers, local hospice for palliative conditions, social services+befriending, GP for exercise referrals, pulmonary/cardiac rehab #physiotalk
physiotalk @physiotalk
Q How important is it to underpin the ‘prescription’ with an effective conversation? #physiotalk
Ian Tyrrell @IanTyrrell5
@caldwell_pt @physiotalk Well now I know what it means I realise we have quite a lot locally. #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk What about referring/sign posting to mental health services? Would anyone consider that social prescribing? #physiotalk
Anya Lotay @AnyaPhysio1417
@physiotalk advice to discuss with pharmacist at a chemist, alcohol liaison, pendant alarm services #physiotalk
Helen Harte @Hartey_PT

Helen Caldwell @caldwell_pt
@Hartey_PainPT @physiotalk Slipper exchange? That sounds good! #physiotalk
Steve Nawoor @stevenawoor
@physiotalk Without context and play back of a persons understanding the next steps in shared decision making can be lost and impact reduced #physiotalk
Unity Physiotherapy @UnityPhysio
@physiotalk Think it does and could it be anything that increases social activity/connection and community activities rather than direct referral to them? #physiotalk
Mari @mariand84
@physiotalk Very important I would say. #physiotalk
Janet Thomas @JanetThomas47
@IanTyrrell5 @caldwell_pt @physiotalk Do you know how to keep track of them all! I don't, but luckily we can refer onto Local area coordinators who have their fingers on the pulse of what's available locally... #physiotalk
M-J Sharp @hipsterlife85
@physiotalk #physiotalk I think this is really important. Just saying 'here's a group/activity you can join' might not be enough in some cases. Maybe sometimes it needs 'feeding in' over more than 1 conversation?
Helen Harte @Hartey_PT
@caldwell_pt @physiotalk It was fantastic! Footwear and foot health being so important , especially in the frail elderly, obviously. #physiotalk
Helen Caldwell @caldwell_pt
@physiotalk Really important to discuss with the pt how they feel it may benefit them. If they can't see the point, then it's perhaps the wrong thing for them. All around the discussion of what's meaningful for them #physiotalk

@JanetThomas47 @IanTyrrell5 @caldwell_pt @physiotalk I wonder if I have this role in my area, who do they sit under ? local council or social services? #physiotalk
Naomi McVey 🐝 @NaomiMcVey
@physiotalk Community choirs, park run, local walking groups, community gardens/gardening, local library groups & activities, faith groups & coffee mornings, volunteering - so much out there when you think beyond usual boundaries of healthcare & physio #physiotalk
Unity Physiotherapy @UnityPhysio
@physiotalk Very, we need to know what someone enjoys, what motivates them before can explore options and discuss with them #physiotalk
Ian Tyrrell @IanTyrrell5
@JanetThomas47 @caldwell_pt @physiotalk Nope, constantly new forms, new referral criteria, people still wanting faxed referrals! They don’t make it easy to refer! Thankfully with revival there’s a worker on site in ed 7 days a week. Also a great care home liaison physio who’s only a call away #physiotalk
physiotalk @physiotalk
Another reminder coming up Can you guess what for? Yes, that's right.... The HASHTAG! #physiotalk
M-J Sharp @hipsterlife85

Helen Caldwell @caldwell_pt
@JanetThomas47 @IanTyrrell5 @physiotalk Local area coordinators? They sound good. I normally ask our OTs, they seem to know everything! #physiotalk
Helen Harte @Hartey_PT
@physiotalk Critical, and also depends on the goal. exploring motivation is so important for success. #physiotalk
Lesley Haig @ProfLesleyHaig
RT @physiotalk: Another reminder coming up Can you guess what for? Yes, that's right.... The HASHTAG! #physiotalk
francis rogan-physio @francisrogan
@physiotalk I think it requires understanding what someone is looking for. There are all sorts of levels. Sometimes easier when you know people involved in the activity #physiotalk
Anya Lotay @AnyaPhysio1417
@physiotalk I like to give leaflets, write names of services down, or discuss it with family members who can look it up on the internet (if patient isn't able to) - all very well suggesting a service but if they have no signposting to access what's the point? #physiotalk
Louise Brennan @Louise_Brennan_
@physiotalk This is what I was wondering...whether the primary goal is activity or not, activity will most likely happen - even if it's the act of leaving the house, walking to the venue. Action will ensue! #physiotalk
Mari @mariand84
@AideenPhysio @physiotalk That is medical prescription if you ask me #physiotalk
Unity Physiotherapy @UnityPhysio

Naomi McVey 🐝 @NaomiMcVey
@physiotalk Very! We all have different likes, dislikes, needs & wants #physiotalk
Steve Nawoor @stevenawoor
@physiotalk @hipsterlife85 SP isn’t just about the coal face of IA and offering a group etc... The wider aspect of involving communities and embedding a sustainable culture and awareness of wider social issues that can drive MSK pathology should be considered in prevention pre-primary care #physiotalk
Louise Brennan @Louise_Brennan_
@AideenPhysio @physiotalk I guess if it's for a health service, then no, but something designed to target wellbeing eg exercise class, choir etc then yes. What do you think? #physiotalk
Unity Physiotherapy @UnityPhysio
@AideenPhysio @physiotalk It is classes as ‘medical prescribing’ but may be part of ‘social prescribing’ IMO as without it someone may not engage #physiotalk

RT @physiotalk: Q What do you understand by the term ‘Social prescribing’ #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk I’ve probably neglected this side a bit. I often give advice about what’s important, how to improve, get more out of things but maybe am not forceful enough in saying “you must” rather than “you could” often pushed for time in ed though #physiotalk
Tom the Physio @LiftForLifePT
RT @physiotalk: Q How important is it to underpin the ‘prescription’ with an effective conversation? #physiotalk
Naomi McVey 🐝 @NaomiMcVey
@mariand84 @AideenPhysio @physiotalk Yes, social prescribing more about accessing local voluntary & community groups to support health & wellbeing, rather than referral to NHS or social care services #Physiotalk
Sam Haworth-Booth @SamHBooth
@physiotalk I think it can encompass any part of supporting patients to develop their integration into the community and improve their 'wellness', whether thats exercise club or support to sort out their benefits so that they can concentrate on 'rehab'. #physiotalk
Aimee Robson @AimeeRobson4
@physiotalk I’m gunna be controversial and say I’ve never actually ‘prescribed’ socially but regularly gained decision to be signposted. it’s more about shared decision making, signposting to what’s out there, gaining buy in. ‘Prescribing’ can imply paternalistic slant #Physiotalk

physiotalk @physiotalk
@AimeeRobson4 That's interesting - it is interesting with the use of the term prescribing Any other thoughts? #physiotalk
Helen Caldwell @caldwell_pt
@BexTownley @physiotalk Yes I guess its a bit of a grey area. Health care professionals can refer in, but also people can self refer in. So if it was just a conversation talking about it and then they go for it themselves could that be classed as social prescribing? Not sure. #physiotalk
Anna Spiteri @spiteri_anna
@physiotalk nice to see some @WeNurses following the conversation. Hello @MargaretSouthc1 #physiotalk
Naomi McVey 🐝 @NaomiMcVey

Louise Brennan @Louise_Brennan_
@hipsterlife85 @stevenawoor @physiotalk And the patient won't understand why you're referring them...considering what many expect from a physio appointment, we could confuse & alienate them altogether! #physiotalk
physiotalk @physiotalk
@caldwell_pt @BexTownley Our blog posting has some useful resources that might help tease out referral v social prescribing! #physiotalk
Anna Spiteri @spiteri_anna
@physiotalk must remember the hashtag### :) #physiotalk
Sam Haworth-Booth @SamHBooth
@physiotalk Essential, otherwise what is the point - people will not understand the importance & it can't be a shared decision based on a persons needs that encourages long term change. #physiotalk
Helen Harte @Hartey_PT
@stevenawoor @physiotalk @hipsterlife85 100% agree. This is about a shift in how communities function. Much bigger issue than just the individual #physiotalk
Pamela Hansford @PamKHphysio
@physiotalk #physiotalk Why be bound by 'the definition'? If the goal is enhancing #HealthandWellBeing through #FunctionalActivity & #CommunityParticipation surely #ParkRuns #SupportGroups #ExerciseGroups all qualify?
Helen Harte @Hartey_PT
RT @NaomiMcVey: @mariand84 @AideenPhysio @physiotalk And these https://t.co/LEbUj2sXDw https://t.co/qtj4eqUbfG #physiotalk

@NaomiMcVey @AideenPhysio @physiotalk Thanks! Very interesting 👌🏻 #physiotalk
M-J Sharp @hipsterlife85
@NaomiMcVey @mariand84 @AideenPhysio @physiotalk Thank you! #physiotalk
Unity Physiotherapy @UnityPhysio
@IanTyrrell5 @physiotalk I usually avoid the word must as people can feel forced into something including when they use ‘must…’ themselves ‘How do you think you would benefit from..’ then they can form a statement ‘I would like…’ which can be more supportive #physiotalk #langauage
Anya Lotay @AnyaPhysio1417
@BexTownley @physiotalk Personally, I tend to inform patients that there are exercise referral systems which help to encourage activity and find classes/groups to join..the GP referral is more to help with the financial burden of memberships to gym/classes #physiotalk
Tom the Physio @LiftForLifePT
@physiotalk Essential. That goes for everything we do, exercise, manual therapy, lifestyle changes etc. Without the conversation to patients you’ll likely seem like just another HCP telling them what’s best for them. #physiotalk
Helen Harte @Hartey_PT
@physiotalk @AimeeRobson4 It's such biomedical language. Offers kudos but is alienating to others... #physiotalk
physiotalk @physiotalk
@PamKHphysio Our GPs are now #parkrun prescribing https://t.co/mjkZJkZ663 #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk @AimeeRobson4 mmm it's an interesting point, while signposting is great, I think because social prescribing is still relatively new to healthcare and we're probably not as good at it atm as we could be... i think social signposting is the first step #physiotalk
Anya Lotay @AnyaPhysio1417
@BexTownley @physiotalk So the social prescribing is informing of useful services and activities, the referral from GP is more to allocate a person to assist service user in increasing activity levels and finding right thing for them..hope that makes sense! #physiotalk
Steve Nawoor @stevenawoor
@Louise_Brennan_ @hipsterlife85 @physiotalk Hence there wider move to community education in what could work and why. The financial envelope with CCGs needs to be considered in Optimal Value Pathways as to what can be offered #physiotalk
Anna Spiteri @spiteri_anna
@physiotalk Yes park run and slimming world #physiotalk
physiotalk @physiotalk
Halfway through the hour already! Still a few tweets slipping through without the hashtag I'm afraid #physiotalk https://t.co/y2hBK0SOTE
Naomi McVey 🐝 @NaomiMcVey
@PamKHphysio @physiotalk I guess it might come down to funding & access models? Exercise on prescription typically NHS funded/supported, while groups like @parkrunUK are founded & run by volunteers within a community? #physiotalk
Helen Harte @Hartey_PT
Argh tweetdeck has crashed on me #physiotalk Trying to keep up on my phone.
Aimee Robson @AimeeRobson4
@physiotalk Maybe it’s word semantics, but some patients need to be coached or coaxed into lifestyle changes so SDM fits for me as that balanced/adult conversation for patients to take on options that may feel out of comfort zone like ‘SP’. We help identify options & steer/coach #physiotalk
Ian Tyrrell @IanTyrrell5
@UnityPhysio @physiotalk Can see your point completely but sometimes we worry about language too much. We wouldn’t say “how would you think about taking anticoagulants” so I suppose is it really a prescription or is it just advice? #physiotalk
Anya Lotay @AnyaPhysio1417
RT @stevenawoor: @physiotalk @hipsterlife85 The infrastructure of how AHPs operate may need the shift out of the Rx rooms in the future to…
Helen Caldwell @caldwell_pt
@IanTyrrell5 @physiotalk I agree, I think the 'you must' terminology can alienate people and social prescribing the patient has to want to do. I think our job is to let they know about things and their benefits for their condition/situation and then for them to chose to do them #physiotalk
Naomi McVey 🐝 @NaomiMcVey
@AimeeRobson4 @physiotalk Good point Aimee - i feel the same about the term exercise prescription :) #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk @PamKHphysio I've heard the @thecsp was looking into something similar for physios...? I hope! #physiotalk
physiotalk @physiotalk
Getting specific now - a reminder of the SP definition first: Social prescribing, sometimes referred to as community referral, is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services #physiotalk
Anya Lotay @AnyaPhysio1417
RT @PamKHphysio: @physiotalk #physiotalk Why be bound by 'the definition'? If the goal is enhancing #HealthandWellBeing through #Functional
Helen Caldwell @caldwell_pt
@IanTyrrell5 @UnityPhysio @physiotalk I see what you're saying @IanTyrrell5. Is social prescribing a more formal way of signposting?#physiotalk
Linda Hindle @HindleLinda
Thanks for the prompt to join the chat tonight @NaomiMcVey. Joining late but very interesting discussion #Physiotalk
physiotalk @physiotalk
So with that definition in mind here is the next question How have you set up local referral schemes? #physiotalk
Unity Physiotherapy @UnityPhysio
@IanTyrrell5 @physiotalk Sometimes we do, shoulds/musts/buts are generally not helpful. Rather than advice I see it as joint exploration & decision making. Some language makes people feel guilty for not doing something (they may not want to or may need support etc) #physiotalk
Steve Nawoor @stevenawoor

Naomi McVey 🐝 @NaomiMcVey
@stevenawoor @AimeeRobson4 @physiotalk @DaniBrailsford Although - ‘prescription’ can indicate the whole process that is more than just recommending - ie steps to make it happen like referral, funding/payment, delivery of the intervention etc? #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk we have good links with our local exercise referral scheme, it's more prescriptive because we sign them up online and can them track if they went #physiotalk
physiotalk @physiotalk
And is this needed if each 'prescription' is going to be individualised to the person? #physiotalk
Bex Townley @BexTownley
@physiotalk blast that pesky #physiotalk (sorry I missed 2)

physiotalk @physiotalk
@NaomiMcVey @AideenPhysio @stevenawoor @AimeeRobson4 @DaniBrailsford The Kings fund say... There are many different models for social prescribing, but most involve a link worker or navigator who works with people to access local sources of support So - certainly organised and supportive #physiotalk

Janet Thomas @JanetThomas47
@BexTownley @AnyaPhysio1417 @caldwell_pt @physiotalk @cimspa Having looked at the social prescribing defns my thoughts (and that's all they are!) #hellomynameislee.. @LeeSpokes1

Unity Physiotherapy @UnityPhysio
@IanTyrrell5 @AimeeRobson4 @physiotalk Not keen on it either! If we need a label maybe could be improving social connections and activities? Any thoughts on alternative terms to social prescribing? #physiotalk

Ian Tyrrell @IanTyrrell5
@physiotalk @AimeeRobson4 Personally would prefer that! Not a fan of buzzwords or catchphrases! #grumpyoldman #physiotalk
Lesley Haig @ProfLesleyHaig
Think the term prescribe is quite practitioner-centred? Not helpful when trying to support new behaviours? #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk @NaomiMcVey @stevenawoor @AimeeRobson4 @DaniBrailsford It's interesting that 'most' involve a link worker because that hasn't come up yet in tonights discussion... how many know who their local link worker is/how to access them? #physiotalk
physiotalk @physiotalk
Q Do you review patients following a social prescription referral? #physiotalk
#hellomynameislee.. @LeeSpokes1
@HindleLinda also, has anyone asked any patients / service users what they would want to call the process- or does it even matter? #physiotalk

Bex Townley @BexTownley
@JanetThomas47 @AnyaPhysio1417 @caldwell_pt @physiotalk @cimspa #physiotalk and if this was a frailer older person who was a frequent faller I guess you wouldn’t socially prescribe line dancing for the social aspect !
Helen Harte @Hartey_PT
@physiotalk No because they're often discharged from services and SP seen as an extension of rehab, usually occuring at the end of a clinical intervention. Far from ideal- cant measure the impact then #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk I work in ed so only if they come back, and if they do it pretty much means it hasn’t worked! 😐 #physiotalk
Unity Physiotherapy @UnityPhysio
@physiotalk Only if still treating them otherwise they know they are able to contact if needed #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk The barrier often is that you are 'prescribing' at the point that you are encouraging their care to continue in the community rather than clinical setting which is often their last session! #physiotalk
Mari @mariand84
@Hartey_PainPT @physiotalk Agree with this one #physiotalk
Anya Lotay @AnyaPhysio1417
@JanetThomas47 @BexTownley @caldwell_pt @physiotalk @cimspa that sounds clinically reasoned and appropriate - all depends on patient goals too - if their goal is to be able to get from taxi to church then car transfers and outdoor mobility practice with a community therapist or exercise therapist may be more appropriate.. #physiotalk
Steve Nawoor @stevenawoor
@AideenPhysio @physiotalk @NaomiMcVey @AimeeRobson4 @DaniBrailsford We met ć the Chief Link Worker for the region & now have direct referrals via EMIS & vice versa for the SP team. We also have IST updates from the SP link worker in the next few weeks. @AideenPhysio it will be on the agenda for the ‘Think Tank’ event in October 👌🏼#physiotalk
Linda Hindle @HindleLinda
@LeeSpokes1 social prescription was used to encourage HCPs to think about social support alongside /instead of medication and interventions #physiotalk
Anya Lotay @AnyaPhysio1417
@JanetThomas47 @BexTownley @caldwell_pt @physiotalk @cimspa a patient who wants to improve their balance and reduce falls may benefit from more specific exercise groups or types of exercise suggestions #physiotalk

Unity Physiotherapy @UnityPhysio
@AideenPhysio @physiotalk I discuss with the patient before then and we work towards it, sometimes they start community activity prior to discharge #physiotalk
Helen Caldwell @caldwell_pt
@physiotalk This is a really interesting question in terms of reviewing the outcome of whether it's worked. I guess, depending on the service, they might (probably should) evaluate before and after. #physiotalk
M-J Sharp @hipsterlife85
@AideenPhysio @physiotalk So true! Hard to start thinking about it from the beginning of when you see them #physiotalk
Helen Caldwell @caldwell_pt
@AnyaPhysio1417 @JanetThomas47 @BexTownley @physiotalk @cimspa I think if I was referring in from a fall perspective I would term this as a clinical referral, although there is overlap as people get a lot socially from these classes #physiotalk
physiotalk @physiotalk
So - 5 mins left in tonights chat One final question Have you changed your thoughts about social prescribing after tonights #physiotalk
Pamela Hansford @PamKHphysio
@IanTyrrell5 @UnityPhysio @physiotalk #physiotalk But maybe we should say “how would you think about taking 'anticoagulant's/Rx of any sort” & follow this with and here are the pros & cons - 3/12 in ICU & Rehab reminded me that #InformedDecisionMaking needs to be our focus in all our dealings with 'patients'
Bex Townley @BexTownley
@caldwell_pt @AnyaPhysio1417 @JanetThomas47 @physiotalk @cimspa and in the devil is in the detail@of the overlap indeed :) great conversation and extremely helpful for me, thank you #physiotalk

@BexTownley @caldwell_pt @AnyaPhysio1417 @JanetThomas47 @cimspa Just watch for a posting in around 2 mins time :) #physiotalk
Mari @mariand84
@physiotalk Yrs, it has widen my perspective #physiotalk
Sam Haworth-Booth @SamHBooth
@AideenPhysio @physiotalk Hmm, not sure I agree - doesn't mean we can't, more that we are not yet in the habit of doing it 'til the end? #notaseasyinpractice #goalsetting #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Yes! 1) I didn’t know what social prescribing was. 2) turns out I do quite a bit of it 3) I don’t like the phrase social prescribing 4) buzzwords and catchphrases irritate me! #physiotalk
Aideen Larmer @AideenPhysio

Linda Hindle @HindleLinda
@caldwell_pt @physiotalk Also how would professional and individual define success? Attendance, clinical outcome, wellbeing, goal achievement... #physiotalk
Helen Harte @Hartey_PT
@physiotalk yes. Think we need to do more to measure the impact otherwise its tokenistic #physiotalk
physiotalk @physiotalk
As we draw to a close on tonights #physiotalk The Kings fund are holding a #socialprescribing event https://t.co/8x2kkhK6BF #physiotalk
Helen Caldwell @caldwell_pt
@physiotalk It's really made me think about the differences between signposting/social prescribing, and whether more formal processes are required and how we can make it easier/clearer for patients and staff #physiotalk
physiotalk @physiotalk
And @weAHPs are holding a #socialprescribing chat #physiotalk https://t.co/4xb8I8XomO
