physiotalk @physiotalk
Good evening and welcome to tonights #physiotalk
physiotalk @physiotalk
Before we start with our questions - can you let us know who is joining us and where you are from #physiotalk
physiotalk @physiotalk
And just a reminder - you need to include the hashtag #physiotalk in all your tweets. This helps others to follow the discussion and us to collect the transcript afterwards
Ian Tyrrell @IanTyrrell5
@physiotalk Ian from Stoke (not originally) #physiotalk
physiotalk @physiotalk
@IanTyrrell5 Good evening! #physiotalk
Liz Cowan @liziophysio1
@physiotalk hi #physiotalk Liz Cowan, paeds physio in private practice from Warrington probably more of a browser than a contributor today but interested in the link between mental health and exercise
physiotalk @physiotalk
@liziophysio1 I'm sure you will have something to contribute! #physiotalk
lmac @Laineymac80
Elaine from Glasgow. Hello! #physiotalk
physiotalk @physiotalk

@caldwell_pt You just beat me to a hashtag reminder :) #physiotalk
Doireann Ni Mhuir @DoireannNiMhuir
@physiotalk Doireann in Kerry (Ireland). I work in neuro rehab, disability sport & adapted physical activity #physiotalk
Janet Thomas @JanetThomas47
Welcome #physiotalk
physiotalk @physiotalk
Thanks for joining us tonight - I'm sure there will be a few more once we get going with the questions #physiotalk
Alex MacKenzie @AlexMacKenzie11
Alex in Aberdeen, likely to be dipping in and out! #physiotalk
Karen West @KWestPhysio
@physiotalk Karen, community physio from Hampshire #physiotalk
physiotalk @physiotalk
The chat and questions are based on the #worldpt day materials - have you had a look? (Don't look now - wait until after the chat!) #physiotalk
Andrew Wilkinson @awilko86
@physiotalk Andrew, in Carlisle, awaiting starting a physiotherapy MSc at Cumbria. #PhysioTalk
Vicky R-C @vicky_reyco
Good evening. Vicky. Respiratory physio in Norfolk #physiotalk
physiotalk @physiotalk
So - lets get started with our first question #physiotalk
Shelley GötzO'Connor @sheldamar
RT @physiotalk: The chat and questions are based on the #worldpt day materials - have you had a look? (Don't look now - wait until after t…
physiotalk @physiotalk
@awilko86 Fabulous! #physiotalk
physiotalk @physiotalk
Q How often do you consider mental health issues during assessment and interventions with your patients? #physiotalk
Janet Thomas @JanetThomas47
BTW I should say hello! I'm on the @physiotalk account tonight from a slightly soggy Edinburgh #physiotalk
M-J Sharp @hipsterlife85
@physiotalk Joining in from my couch as per usual I am multitasking: watching old ER episode & blogging at the same time! #physiotalk
Karen West @KWestPhysio
@physiotalk Almost without exception. Most of my patients are dealing with major life changes - some sudden others a gradual adaptation to disability #physiotalk
Caryl Hunter @CarylAPhysio
@physiotalk Hi. Student Physio from Brookes #physiotalk
lmac @Laineymac80
@physiotalk I would say every assessment bcos of my experience working in an inpatient psychiatric setting. #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Probably more than I think I do. Often consider when dealing with unexplained neurology, non traumatic back pain and in the older patient group. I work in A&E so often explore these issues to identify barriers to progress #physiotalk
Helen Caldwell @caldwell_pt
@physiotalk Try to always consider, but sometimes I'm sure, when people have a mental health diagnosis, it focuses the consideration #physiotalk
physiotalk @physiotalk
Is it something you ask routinely? #physiotalk
Karen West @KWestPhysio
@physiotalk Understanding of patient's mood and cognition essential to work out how best to address behaviour change #physiotalk
Liz Cowan @liziophysio1
@physiotalk #physiotalk I work with many nonverbal kids with significant disabilities, many with intact cognitive levels. I often feel inadequate in terms of how to assess and assist them re what they think/feel about their therapy and life in general.
lmac @Laineymac80
@physiotalk Yes. It forms part of our teams initial assessment #physiotalk
physiotalk @physiotalk
@KWestPhysio That's an important point - we are often seeing people after a major life change or impact of LTC. do we consider their mental health needs alongside physical? #physiotalk
Aideen Larmer @AideenPhysio
We use the EQ5D questionnaire with all of our patients which includes mental health; it serves as a very good prompt to start talking about mental health which can sometimes be a difficult topic to approach #physiotalk
Alex MacKenzie @AlexMacKenzie11
Generally try to consider it given the tie between mental and physical wellness #physiotalk
Peter Droste @DrostePeter
@physiotalk #physiotalk very often especially cases with multiple dx. Most of the time you can tell right away based on initial conversation I realize it is very subjective
Doireann Ni Mhuir @DoireannNiMhuir
@hipsterlife85 @physiotalk Ha ha I’m doing the very same! It’s just a pity more physio’s don’t appear in the show! #physiotalk #retro
Janet Thomas @JanetThomas47
@AideenPhysio Ditto... It is an eye opener sometimes as to how they respond to this section #physiotalk
physiotalk @physiotalk
@DoireannNiMhuir @hipsterlife85 Perhaps one for a future #physiotalk The representation (or lack of it!) of AHPs / physios on the small and large screen :) #physiotalk
Karen West @KWestPhysio
@physiotalk Missing a part of the jigsaw if we don't. Need to consider patients holistically. We don't always do that as fully as we like to think we do #physiotalk
ash @ashrowlands
@physiotalk #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk In some form, whether it be life stresses in the younger demographic or social isolation/loneliness in the older patient. Massive area is the young socially deprived substance abuse category. #physiotalk
physiotalk @physiotalk
So - mostly something you consider - do you formally assess in any way? Any suggested tools? #physiotalk
Aideen Larmer @AideenPhysio
@JanetThomas47 Indeed! Some patients that I otherwise wouldn't have thought mental health was necessarily a topic we would need to cover will suddenly flag up as anxious or depressed in the EQ5D! #physiotalk
Alex MacKenzie @AlexMacKenzie11
Absolutely #physiotalk

M-J Sharp @hipsterlife85
@physiotalk I've often considered it when seeing patients on placements, and thought about it as part of my reasoning, but rarely asked it outright. I suppose I assume pts will say if they want to, but maybe not everyone feels comfortable #physiotalk
Ian Tyrrell @IanTyrrell5
@KWestPhysio @physiotalk I think if you are a black and white physio you can struggle to see past the presenting complaint. Why I love being distinctly grey! #physiotalk
Doireann Ni Mhuir @DoireannNiMhuir
@physiotalk @hipsterlife85 Absolutely! And whenever they appear, it is often incredibly unrealistic. Like walking a complete SCI in patellar bars. Gives people with significant impairments & life changing conditions unrealistic expectations #physiotalk
Karen West @KWestPhysio
@physiotalk Nothing formal. Tend to consider readiness to engage with subjective assessment, body language, eye contact. Not 100% reliable if you don't already know the person #physiotalk
Janet Thomas @JanetThomas47
@IanTyrrell5 @KWestPhysio @physiotalk Ahh - that's why I love working with older people. Never just one presenting issue! #physiotalk
physiotalk @physiotalk
We've had EQ5D mentioned which may flag up an issue Anyone use HAD or PHQ9 or similar? #physiotalk
Karen West @KWestPhysio
@JanetThomas47 @IanTyrrell5 @physiotalk I'm a bit of a navy and green physio - there's an OT struggling to get out! #physiotalk
Gráinne Daniels @grainne_daniels
@physiotalk No time in assessment for questionnaires. I don’t see more acute LBP patients but I quite like the StarTBack tool for screening for psychological factors in more acute presentations. #physiotalk
Aideen Larmer @AideenPhysio
@hipsterlife85 @physiotalk I think even if it is something they would comfortably talk to you about, pts often feel it is irrelevant and not something that needs to be brought up in a physiotherapy session #physiotalk
Alex MacKenzie @AlexMacKenzie11
RT @AideenPhysio: @hipsterlife85 @physiotalk I think even if it is something they would comfortably talk to you about, pts often feel it is…
lmac @Laineymac80
@hipsterlife85 @physiotalk I am comfortable asking bcos I spent a lot of time asking about it in psychiatric setting. I was definitely not confident to ask things outright when I had less experience of it. This should be important part of our education. Is it more part of PT curriculum now? #physiotalk
Karen West @KWestPhysio
@physiotalk Never tried one of these tools, would like to learn more about them #physiotalk
physiotalk @physiotalk
Some great hashtagging but there are a few creeping through without! Remember to include #physiotalk in all your tweets tonight
Ian Tyrrell @IanTyrrell5
@KWestPhysio @JanetThomas47 @physiotalk I’m the same! Got to be in A&E. I meant black and white and everything fitting in nice little packages, rules and procedures to follow, not just making it up as you go along! #physiotalk
physiotalk @physiotalk
So - onto our next question #physiotalk
lmac @Laineymac80
@physiotalk Copyright can be problematic! #physiotalk
physiotalk @physiotalk
How best to approach or address mental health issues with your patients once assessed or screened? #physiotalk
#hellomynameis Adele @ABNeurophysio
It's an integral part of our esd stroke mdt initial assessment and we undertake gad-7 and phq-9 to screen for problems at initial plus review and end points. Lucky to work with psychology in team #physiotalk
Pamela Hansford @PamKHphysio
@physiotalk #physiotalk @pamKHphysio From Cape Town celebrating first time in months as I now have WiFi & hand function has improved sufficiently to open Tweetdeck and keep up
Janet Thomas @JanetThomas47
@Laineymac80 @physiotalk Yup - I think our nursing staff have swapped tools recently because of this #physiotalk
Liz Cowan @liziophysio1
@physiotalk #physiotalk I don't but I am fortunate in private practice to be able to work with SALT and psychology so learn from them. Would like to be able to do more.
Ian Tyrrell @IanTyrrell5
@grainne_daniels @physiotalk I don’t really like the Startback score in hyper acute (again I work in A&E) so many false positives if only occurred when pain management and advice may make symptoms go quite quickly. That’s just my though. #physiotalk
lmac @Laineymac80
@physiotalk I deal same way as a physical problem- involve GP (with consent), encourage pt to seek help. Reassurance if appropriate #physiotalk
Steven Addingadoo @addingadoo87
@physiotalk Just like physical health issues... it’s all about recovery forcussed care #physiotalk

Karen West @KWestPhysio
@physiotalk Discuss access to psychological therapies, GP or OPMH. Can access OPMH direct which is great #physiotalk
Gráinne Daniels @grainne_daniels
@physiotalk Just talk openly: ask if patient would be willing to engage with either psych or wellbeing services- or if they feel that they have support network in place. Again most of my patients have already had Mh issues identified prior to attending though #physiotalk
Aideen Larmer @AideenPhysio
I think a lot of clinicians, particularly newer grads are afraid to ask about mental health for fear of ending up out of their depth - #socialpresrcibing and knowing where/how to refer is important especially with significant MH conditions #physiotalk
Karen West @KWestPhysio
@physiotalk Try to address issues of stigma from having a MH problem. #physiotalk
lmac @Laineymac80
@addingadoo87 @physiotalk So important to make it clear mental health issues are nothing to be ashamed of, help is available and it happens to so many even if it isn't outwardly obvious. #physiotalk

physiotalk @physiotalk
@Laineymac80 @addingadoo87 That's why its so good to see it the focus of this years #WorldPTday #physiotalk
physiotalk @physiotalk
So - onto our next question.... (We are nearly half way through!) #physiotalk
Ian Tyrrell @IanTyrrell5
@physiotalk Open discussion regarding how to address their problems, support networks. Encouraging people that there’s no need to be ashamed, or that there’s no stigma attached. A lot of positive reinforcement #physiotalk
physiotalk @physiotalk
Q What strategies do you use in promoting exercise for people with depression or anxiety? #physiotalk
Karen West @KWestPhysio
@physiotalk Already? #physiotalk
physiotalk @physiotalk
@KWestPhysio Time flies when you are having fun! #physiotalk
lmac @Laineymac80
@physiotalk @addingadoo87 So important that we work towards mental health being part of the big picture of healthcare. It has an enormous impact on ppl and their physical health. Good mental health is integral to good health. #physiotalk
Karen West @KWestPhysio
@physiotalk Engage them with activities of daily living - washing up can be more activity than some are used to. #physiotalk
Helen Caldwell @caldwell_pt
@physiotalk Looking at what their goals are, breaking it down, lots of positive reinforcement, again trying to find out what works for them. One size does not fit all! #physiotalk
physiotalk @physiotalk
Remembering the stats that have been tweeted out ahead of tonights chat - many of the people you see may be depressed #physiotalk https://t.co/3llEcQKEi3
Ian Tyrrell @IanTyrrell5
@physiotalk I often talk about achievement diaries with patients (generally only see people once) so encourage to make daily goals and try and achieve something each day and take note of it. #physiotalk
Alex MacKenzie @AlexMacKenzie11
@Laineymac80 @addingadoo87 @physiotalk And by physios being happy to talk openly to patients about mental health does help ‘normalise’ it and make it less of a taboo subject #physiotalk
Gráinne Daniels @grainne_daniels
@physiotalk Work with them to identify an activity that they value (is meaningful and important to them) and support them to work out a plan #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk I usually educate on the physiological benefits of exercise, often I skip past the 'exercise' part & focus on the meaningful activity part - having a reason to get up & out/meet people eg walking to the shops/joining a club - then the exercise is an added byproduct! #physiotalk
physiotalk @physiotalk

Karen West @KWestPhysio
@physiotalk Just completed MSc module in motivational interviewing - lots in that approach to help. Need to help person to become ready to change and then to work with them to realise that change. Building self efficacy essential especially if they haven't done exercise before #physiotalk
lmac @Laineymac80
@physiotalk Relevance is key. Finding something that someone can do daily and enjoys. Costs little and can be really simple- walk, youtube exercise video, yoga. If ppl enjoy they'll tend to stick with it! #physiotalk
PT. Umar Tijjani @88umarfaruk
@physiotalk Sorry for joining late. Umar Tijjani, student physiotherapist from Nigeria. #physiotalk
physiotalk @physiotalk
@IanTyrrell5 I like the idea of an achievement diary! A very positive slant on goals #physiotalk
Helen Caldwell @caldwell_pt
@IanTyrrell5 @physiotalk One achievement each day (as simple as getting dressed) and one gratitude each day I've found helpful for patients to lift mood. #physiotalk
physiotalk @physiotalk
@AideenPhysio So #Love Activity then :) #physiotalk
physiotalk @physiotalk

Helen Caldwell @caldwell_pt
@KWestPhysio @physiotalk Agreed. This is all in the #loveactivity approach and not dictating what someone should be doing, but facilitating them to come up with their own solutions #empowerment #physiotalk
Liz Cowan @liziophysio1
@physiotalk empathy. Its not really them and us, I snapped my TA last year - that complete change in independence level and lack of ability to enjoy the outdoors totally floored me. I learned so much about fear, lack of control, using a mobility scooter, the way society sees you etc #physiotalk
Karen West @KWestPhysio
@caldwell_pt @physiotalk Patient's own solutions more likely to be successful in the long term than ours. #physiotalk
Pamela Hansford @PamKHphysio
#physiotalk @PamKHphysio 3/12 in #ICU & #RehabUnit & 5/12 in #Frailcare bring the importance of #MentalHealth & its connection to #PhysicalActivity for ourselves & those with whom we work to our attention as areas of top concern
M-J Sharp @hipsterlife85
@AlexMacKenzie11 @Laineymac80 @addingadoo87 @physiotalk I agree, it makes it okay to talk about it #physiotalk
lmac @Laineymac80
@liziophysio1 @physiotalk Agree! Recent surgical experiences have opened my eyes to just how much of an impact WHAT a healthcare professional says! Even with my experience and knowledge it had a huge impact! #physiotalk
physiotalk @physiotalk
@PamKHphysio Can you tell us more - lived experience is so powerful in making us change practice! #physiotalk
Caryl Hunter @CarylAPhysio
@AideenPhysio I am looking forward to seeing how on my next placement specialising in Pain Management approaches this. #physiotalk
Doireann Ni Mhuir @DoireannNiMhuir
@physiotalk In Ireland we have Sports Inclusion Disability Officers (pan disability so including MH) & many would run ‘mood walks’ in nature in conjunction with OT/PT/services. Group activities can help begin & maintain involvement #physiotalk
Aideen Larmer @AideenPhysio
@liziophysio1 @physiotalk How an injury affects someones 'role' has a big difference - I used to see it a lot in orthopaedics - younger pts were much more likely to catastrophise their injury and unfortunately it tended to negatively impact on their progress #physiotalk
Liz Cowan @liziophysio1
@caldwell_pt @KWestPhysio @physiotalk #physiotalk yes because everyone is dealing with their own normal and trying to get back to it. I felt a lot of guilt about not being able to get out on sunny days and having to watch TV box sets. Establishing what drives people and what their own expectations are is important
lmac @Laineymac80
@KWestPhysio @caldwell_pt @physiotalk and hugely impactful if patient can feel empwered to find their own solutions even if they are guided to it! #physiotalk
physiotalk @physiotalk
Moving onto our next (but linked) question - but I think I see the way the answers are going... #physiotalk
physiotalk @physiotalk
Q What type of exercise is most effective for people with depression or anxiety? #physiotalk
Helen Caldwell @caldwell_pt
@CarylAPhysio @AideenPhysio My pain management experience (within an MSK placement), taught me the most about chronic pain/depression/strategies. Good luck, it's a great placement with loads of transferable skills #physiotalk
lmac @Laineymac80
@physiotalk in my experience depends on the extent of the anxiety and depression. I framed any engagement as a positive #physiotalk
physiotalk @physiotalk
Remember that if you want to look up the research there is a great #worldPTday resource page... https://t.co/igK7ibZOcs #physiotalk

@physiotalk @KWestPhysio No I don't think nearly enough! #physiotalk
Liz Cowan @liziophysio1
@Prof_IeuanEllis whilst I agree in part with the first quote, some people are not able to access exercise without help or encouragement. For some it is not a case of not making time. #physiotalk

Karen West @KWestPhysio
@physiotalk Probably not, easier if working in the community. Outdoor mobility practice, checking patients can manage their doorsteps all good opportunities to get out #physiotalk

physiotalk @physiotalk
Q How do you address issues with sedentary behaviour and low physical activity in people with more severe mental health issues? #physiotalk

@physiotalk Start slowly and build up. Functional and relevant. Change attitude that exercise= red face, out of breath. Can be a walk, stretching, yoga. Wont meet WHO exercise guidelines to begin with but aim to build. #collaboration #support #encourage #physiotalk

@JanetThomas47 @IanTyrrell5 @physiotalk @KWestPhysio Thinking about is it a bigger challenge for those who aee ward based - especially considering SCI pts as an example may have spent many mths on wards but ADL will often involve outdoor life. Functional & mental needs meant? #physiotalk
Aideen Larmer @AideenPhysio
@physiotalk Because I tend to be dealing with pain alongside the sedentary behaviour, if they have severe MH issues aswell I start away from activities that will cause pain/discomfort to begin with and just get into habit of movement and all the benefits this brings! #physiotalk

@physiotalk Sometimes it depends with the cause of that anxiety or depression. For instance if it's is because of prolonged immobility limiting the patient to participate on activity of daily life, a designed exercises program may reverse this sound effective. #physiotalk
