Janet Thomas @JanetThomas47
Hello all! I'm on hashtagging duties tonight - so a reminder as we start to all put the hashtag #physiotalk in ALL of your tweets! https://t.co/oYNtXhCexH
Janet Thomas @JanetThomas47
[REDACTED USER] [REDACTED USER] [REDACTED USER] Whoops - meant that from my own account :) #physiotalk
Janet Thomas @JanetThomas47
[REDACTED USER] [REDACTED USER] Call Penrith north?! #physiotalk

[REDACTED USER] [REDACTED USER] #physiotalk Kate...outgoing agile chair..based in Winchester work as a community physio in Southampton.

[REDACTED USER] @JanetThomas47 [REDACTED USER] #physiotalk me......
Janet Thomas @JanetThomas47
Just to complete the weather picture - a nice dry day in Scotland after a lovely sunny weekend :) #physiotalk
Janet Thomas @JanetThomas47
[REDACTED USER] Just to clarify a little? Community = visits at home or any service not in acute? #physiotalk
@_JoeMiddleton
[REDACTED USER] Speaking as a non-community physio (although I have done my fair share), it seems there are more community physiotherapists, in a wide-range of roles and services, with a greater evidence base behind what they do. So yeah, mostly positive? #Physiotalk
kate bennett @kategahr_kate
[REDACTED USER] #physiotalk sorry
@_JoeMiddleton
[REDACTED USER] It seems there has been a growth in community physiotherapists moving into advanced roles, and roles that blur traditional boundaries (e.g. with nursing or OT). #Physiotalk
kate bennett @kategahr_kate
@_JoeMiddleton [REDACTED USER] #physiotalk yes I would agree with this but in our Trust the more specialist community teams ie learning disabilities, mental health, are being cut massively leaving the general community teams to pick up the less specialist cases
@_JoeMiddleton
I'm interested. How do you feel about the role being 'quite generic'? #physiotalk https://t.co/ERmqRC2GJ8
Janet Thomas @JanetThomas47
I'm in a Day Hospital (now an assessment and rehab centre) and we have certainly been evolving recently. Focus on frailty and accessing the right service for each individual #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] We are versatilists @natbeswetherick #physiotalk [REDACTED USER]
@_JoeMiddleton
@kategahr_kate [REDACTED USER] Interesting. I have noticed that, locally, community services seem to be re-modelled frequently, boucing between specialist, defined services and more generalist services. #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk we have no provision for respiratory patients in our Trust. It is left to the community teams and most of us have not done acute resp for several years. Equally not much provision for training around this area
kate bennett @kategahr_kate

[REDACTED USER] @kategahr_kate [REDACTED USER] [REDACTED USER] @natbeswetherick [REDACTED USER] 14 years ago - that's recent history! I think that anyone working in a community setting needs to be versatile! We never know who is going to walk through the door next! #physiotalk
kate bennett @kategahr_kate
@_JoeMiddleton [REDACTED USER] #physiotalk we don't tend to bounce just lose posts and then it falls (pardon the pun!) On the general community teams

@_JoeMiddleton
Prepare to get even busier! #Physiotalk
Janet Thomas @JanetThomas47
[REDACTED USER] [REDACTED USER] Wondering why? How can we ensure that 'proper' rehab doesn't get lost in a squeezed service - because it works and saves money in the long run I'm sure! #physiotalk
kate bennett @kategahr_kate
@NIHR_RDS [REDACTED USER] [REDACTED USER] #physiotalk [REDACTED USER] is an amazing success story but posts like these are few and far between. I asked how to influence this at PTUK 2018 during a talk and was told to wait and see if it happened or move jobs....
kate bennett @kategahr_kate
@sarahseager94 #physiotalk and organisations like [REDACTED USER]
Physio Companion @PhysioCompanion
[REDACTED USER] Hello! Tweeting from Iowa! #Physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk do you mean basic msk in the community is difficult to get hold of or unless your patient has a basic msk injury you can't get community care? #babybrain means I'm easily confused....
Janet Thomas @JanetThomas47
Why might that be? Surely need to link in for support as well as practical issues around referrals / equipment etc? #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] [REDACTED USER] #physiotalk not if all the money is going on building new hospitals!
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk yes! I am just a referral system at times!
Janet Thomas @JanetThomas47
[REDACTED USER] Do you think the response times are useful? Sorry - not really up to speed with LTP of course as in Scotland! #physiotalk
@_JoeMiddleton
[REDACTED USER] [REDACTED USER] Are services moving more towards 'generic' support workers, rather than profession-specific staff? #Physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk this is the system we use...we are bottom heavy on rehab assistants who are all OTAGO trained and amazing
Physio Companion @PhysioCompanion
[REDACTED USER] In the United States, we hope to have the ability to utilize diagnostic imaging on our own ie. X-Ray and cut out the middleperson. #physiotalk
kate bennett @kategahr_kate
@_JoeMiddleton [REDACTED USER] [REDACTED USER] #physiotalk all our rehab assistants and band 4s are generic
Janet Thomas @JanetThomas47
@_JoeMiddleton [REDACTED USER] [REDACTED USER] Not sure - but if they are appropriately trained is that an issue I wonder or not? #physiotalk
@_JoeMiddleton
@JanetThomas47 [REDACTED USER] [REDACTED USER] I suspect it is not an issue. Seems to be the way things are going across communtity and hospital-based services. #physiotalk
kate bennett @kategahr_kate
@JanetThomas47 [REDACTED USER] #physiotalk we have an urgent response service which aims (and usually succeeds) to respond in the appropriate timeframes. Lack of success is usually due to factors outside their control.
kate bennett @kategahr_kate
@_JoeMiddleton @JanetThomas47 [REDACTED USER] [REDACTED USER] #physiotalk ours have competencies to complete and competencies are checked annually

@_JoeMiddleton
@Jolockyear1 [REDACTED USER] [REDACTED USER] Did you feel prepared for that? I wonder whether pre-registration programmes are changing to reflect this? (It's not just community roles that are becoming more generic) #Physiotalk
Janet Thomas @JanetThomas47
[REDACTED USER] [REDACTED USER] [REDACTED USER] Really?! Again showing my lack of commissioning knowledge but is it that specific to excluse? #physiotalk
@_JoeMiddleton
This. This! THIS! I hate the language of admission avoidance/prevention. The goal should be right care, right time, right place. Any reduction in hospital admissions should be seen as a happy byproduct (and similarly, hospital admission is not a failure) #Physiotalk
Physio Companion @PhysioCompanion
[REDACTED USER] Barrier: Insurance Insurance Insurance!!! #physiotalk https://t.co/yX8qlepzgp
kate bennett @kategahr_kate
[REDACTED USER] #physiotalk restrictions placed by commissioners. We all know the Sherrington recommendations around ex provision yet only allowed to keep pts on caseload for 12 weeks...Can refer to exercise groups but large proportion of pts can't attend 🤷‍♀️🤷‍♀️🤷‍♀️
@_JoeMiddleton
@JanetThomas47 [REDACTED USER] [REDACTED USER] [REDACTED USER] I struggle with this also. It seems community services are so severly hamstrung by what they are 'commissioned' to do. #physiotalk
Janet Thomas @JanetThomas47
@_JoeMiddleton We've looked at how we reduce ED / admissions but its absolutely not what our service is set up to do! As you say, its a happy byproduct of effective interventions. #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk I go back to work tomorrow after 9 months off (mat leave) and will have a look as sure we have some somewhere.....
@_JoeMiddleton
@JanetThomas47 Unfortunately, it is usually an expected outcome as it = ££££££ #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk service spec. If longer than 12 weeks we have to demonstrate significant progress at weekly case meetings. We are expected to refer on to local exercise groups to continue but lots of barriers to attendance.
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk ideally we have patients off our caseload at 10 weeks.....
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] #physiotalk and strategic nappy change...not mine...
@_JoeMiddleton
[REDACTED USER] Barriers: Time. Information. Focus on acute response/hospital discharge --> loss of longer-term rehab services #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] [REDACTED USER] #physiotalk yes and no....it is great for the patient to have one person sorting Every thing...this is a common plaudit in our team. But equally it would be nice to concentrate on my strengths (which is being a physio) at times rather than the other stuff
Janet Thomas @JanetThomas47
[REDACTED USER] @Jolockyear1 @_JoeMiddleton [REDACTED USER] [REDACTED USER] Just ask! I'm sure most HEI's would be happy to have input. The key time is to be involved at revalidation as that's when programmes change - and local stakeholders are a key part of the process #physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] [REDACTED USER] Really? I'm always very happy to talk to acute clinicians. Any information is always gratefully received! I Will even call/ bleep back if it means I get more info lol #physiotalk
@_JoeMiddleton
[REDACTED USER] Now, there is the next bit of #physiotalk research!
kate bennett @kategahr_kate
@Jolockyear1 [REDACTED USER] @_JoeMiddleton [REDACTED USER] [REDACTED USER] #physiotalk unpopular opinion over here but I think some kind of community placement should be compulsory 🙈🙈🙈. Could be in any patient group though....

Janet Thomas @JanetThomas47
@sarahseager94 That's a great idea. Have you thought about using the Life Curve app to help with early intervention and self management? #physiotalk
@_JoeMiddleton
Forgive me for hijacking #physiotalk , but I must ask a question about language, Common parlance is to refer to 'community' and 'acute', but doesn't that misrepresent community services and therapists? Like, don't y'all see acute presentations too?! Thoughts?
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] [REDACTED USER] #physiotalk example. Huntington patient with a chest infection. Neuro don't treat chests. Care plan is not for admission. Community team get referral but we don't routinely see chests. My resp competencies are definitely out of date (last on a ward in 2014) 1/2
Physio Companion @PhysioCompanion
[REDACTED USER] Community engagement! Fall risk screenings and education for the elderly. Sports screenings for youth sports! Just have to find some good screening tools and get into the community! #Physiotalk
@_JoeMiddleton
[REDACTED USER] Advanced Practice roles Revolutionising pro-active, preventative care for older people living with frailty Champions of Public Health #Physiotalk
kate bennett @kategahr_kate
[REDACTED USER] [REDACTED USER] [REDACTED USER] #physiotalk it is hard enough getting funding/ leave for courses that are deemed to be appropriate so no chance of getting funding for respiratory update when we see resp patients maybe once a year. The service the patient needs isn't commissioned.
kate bennett @kategahr_kate
[REDACTED USER] @sarahseager94 [REDACTED USER] [REDACTED USER] [REDACTED USER] [REDACTED USER] #physiotalk permission to share not recognised and they wouldn't call my service back to check who I was in the end they took the phone to the patient who spoke to me and then said in my hearing that she was happy for her discharge plans to be discussed with me.
kate bennett @kategahr_kate
[REDACTED USER] @sarahseager94 [REDACTED USER] [REDACTED USER] [REDACTED USER] [REDACTED USER] #physiotalk but when they have a patient they need to get out they can't get on the phone quick enough and permission to share doesnt even come into it!

[REDACTED USER] @kategahr_kate @sarahseager94 [REDACTED USER] [REDACTED USER] [REDACTED USER] [REDACTED USER] 'Missed opportunity' is definitely the right phrase here. As an aside, secondary care-based therapists would love to hear about the success stories of patients who have managed to stay at home, or who have made great recoveries. #Physiotalk
@_JoeMiddleton
[REDACTED USER] @kategahr_kate @sarahseager94 [REDACTED USER] [REDACTED USER] [REDACTED USER] [REDACTED USER] Unfortunately, we usualy only hear about the "failures", and often not in a productive or learning-focussed way! #Physiotalk
