@physiotalk Speaking as a non-community physio (although I have done my fair share), it seems there are more community physiotherapists, in a wide-range of roles and services, with a greater evidence base behind what they do. So yeah, mostly positive? #Physiotalk

Sarah Seager @sarahseager94
#physiotalk A1. We are definitely more integrated with other community services and seeing more acute conditions, i.e. people don’t stay in hospital for as long
Joe Middleton @_JoeMiddleton
@physiotalk It seems there has been a growth in community physiotherapists moving into advanced roles, and roles that blur traditional boundaries (e.g. with nursing or OT). #Physiotalk
Jo Kitchen @J0Physio
@physiotalk We link in much more closely with leisure centres, charities, local exercise groups. Much more of a focus on self-management & accessing NHS when needed rather then weekly/6 monthly reviews #physiotalk
Srikesavan Sabapathy @Srikesavan
@physiotalk Hello, Trauma inpatient physiotherapist from Oxford #physiotalk
kate bennett @kategahr_kate
@_JoeMiddleton @physiotalk #physiotalk yes I would agree with this but in our Trust the more specialist community teams ie learning disabilities, mental health, are being cut massively leaving the general community teams to pick up the less specialist cases
Jo Fishpoole @JoFishpoole
@kategahr_kate @physiotalk I would absolutely agree. The role has certainly become more holistic, a lot of case managing and increased overlap with OT. I think we (Physios and OTs) are ok with that and appreciate the specialist skills of each other's professions...#PhysioTalk
Joe Middleton @_JoeMiddleton
I'm interested. How do you feel about the role being 'quite generic'? #physiotalk https://t.co/ERmqRC2GJ8
Bex Townley @BexTownley
#physiotalk just lurking - not a physio but my role and others like me (exercise specialist) look to community physios working with older ppl to refer into ev based falls prevention progs

Vasanth Sampath @Vas_Sampath
@physiotalk Physio role has changed significantly.. not just doing clinical but also Health care co-ordination roles.. Very challenging change #physiotalk
Janet Thomas @JanetThomas47
I'm in a Day Hospital (now an assessment and rehab centre) and we have certainly been evolving recently. Focus on frailty and accessing the right service for each individual #physiotalk
Jo Kitchen @J0Physio
@physiotalk Also a shift towards rehab that would have happened in hospital happening at home- supported discharge/ early discharge/ reablement #physiotalk
physiotalk @physiotalk
Great responses so far and hardly any forgotten hashtags #Physiotalk
Alistair Beverley @theLDphysio
@physiotalk In LD services has been a tightening up of some services. Very much postcode lottery whether or not you even have specialists in certain counties. Some LD services refusing to provide resp support too alas. #physiotalk

Vasanth Sampath @Vas_Sampath

Joe Middleton @_JoeMiddleton
@kategahr_kate @physiotalk Interesting. I have noticed that, locally, community services seem to be re-modelled frequently, boucing between specialist, defined services and more generalist services. #physiotalk

Pamela Kipps Hansford @PamKHphysio
@physiotalk Thanks Janet, always enjoy #physiotalk & #WeActiveChallenge2019 lets start by #celebrating the #PhysioCommunity #WeAHPsActive #LoveActivity
Jo Fishpoole @JoFishpoole
@kategahr_kate @physiotalk However, I do feel it has diluted other professions understanding of our roles. A lot of the community nurses I work couldn't tell you the difference between a Physio and an OT. I'm my role I'm trying to get away from the general term 'a therapist' #PhysioTalk
kate bennett @kategahr_kate
@theLDphysio @physiotalk #physiotalk we have no provision for respiratory patients in our Trust. It is left to the community teams and most of us have not done acute resp for several years. Equally not much provision for training around this area
Annabelle Long @AnnabelleLong75
@kategahr_kate @JoFishpoole @physiotalk @natbeswetherick @VickiG_physio #physiotalk I like that term....my senior in Acute elderly care told me that back in the day.... 14 years ago....dear Lord makes me feel old
Alistair Beverley @theLDphysio
@physiotalk Now you've said it... 🤦🏼‍♂️ #physiotalk
Catherine Hopkins @cathhoppo
@physiotalk Interested to read replies as an acute physio who refers to comm team. Not sure my referrals have changed! Might be more admission prevention/fire fighting than rehab in some areas in my experience #physiotalk

@physiotalk it has been good to see the increasing number of Consultant grade physios. eg Is @EstherClift in the #physiotalk house this evening? Don't think her post existed 10 years ago?
Janet Thomas @JanetThomas47
@cathhoppo @physiotalk Wondering why? How can we ensure that 'proper' rehab doesn't get lost in a squeezed service - because it works and saves money in the long run I'm sure! #physiotalk
Alistair Beverley @theLDphysio
@kategahr_kate @physiotalk I hear this a lot, wonder why this is? Trying to get training seems also a contentious issue #physiotalk
Annabelle Long @AnnabelleLong75
@physiotalk #physiotalk I think that good community services will be absolutely vital for the LTP but will it be properly resourced???
Sarah De Biase @sarahdebiase
@physiotalk #physiotalk Q1 Agree with holistic nature of work. Evolution also to manage constraints within the system including resource restrictions in other parts of health and care pathways; need to be more imaginative in how we enable older people to remain in their own homes for longer
The Rare Bird: Jay @wigmore_welsh
@physiotalk #physiotalk chipping in as a case manager for folks after severe accidents, community care seems incredibly hard to get hold of unless it's basic #msk

Natalie Patterson @PhysioNatalie
@physiotalk I am currently in a comm role and it seems to be they expect you to work remotely more and make it difficult to get into base. Scared for a band 5 as you can feel very isolated no matter how nice and supportive your team is. #physiotalk

Natalie Patterson @PhysioNatalie
@physiotalk Would also add that you’re often triaging for other services as the second you walk through the door they download all their medical issues on you &my to do lists are often filled with non-physio specific things e.g requesting med reviews or TVN referrals for seating. #physiotalk
Jo Fishpoole @JoFishpoole
@Vas_Sampath @physiotalk I think community services are managing an increasingly complex cohort of patients at home- and rightly so. But, we also need the training provision to develop and maintain the skills required #PhysioTalk

@kategahr_kate @sarahseager94 @KWestPhysio @Srikesavan @naojane18_PT @physiotalk It does nothing for us/them mentality 😔 Missed opportunity for that clinician, not a great exp for the pt and just ends up frustrating people #sigh #physiotalk

@KWestPhysio @kategahr_kate @physiotalk I get you. What would you be looking for though? Hr, SATS, increased work of breathing, cyanosis, auscultation and then if any abnormalities, refer on. How is this different from suspected cauda equina or MSCC? Haste needed for both. #physiotalk

@_JoeMiddleton @physiotalk We sure do! I try to go for primary, secondary, tertiary or community care... 🤷‍♀️ #PhysioTalk

@kategahr_kate @cathhoppo @sarahseager94 @KWestPhysio @naojane18_PT @physiotalk I understand...Smtims due to the pressures of wrkng in the acute set up we do forget that community colleagues are under the same pressure. Unfair for sure. We need a smooth comms system. This is going to be vital with the movement towards "home first" and "at home" #physiotalk
Alistair Beverley @theLDphysio
@KWestPhysio @kategahr_kate @physiotalk This isn't intended as being critical, just trying to relook at some of "the fear" around resp screening #physiotalk I'm not a resp specialist either by the way

@cathhoppo @kategahr_kate @sarahseager94 @KWestPhysio @Srikesavan @naojane18_PT @physiotalk 'Missed opportunity' is definitely the right phrase here. As an aside, secondary care-based therapists would love to hear about the success stories of patients who have managed to stay at home, or who have made great recoveries. #Physiotalk

@cathhoppo @kategahr_kate @sarahseager94 @KWestPhysio @Srikesavan @naojane18_PT @physiotalk Unfortunately, we usualy only hear about the "failures", and often not in a productive or learning-focussed way! #Physiotalk
Catherine Hopkins @cathhoppo
@Srikesavan @kategahr_kate @sarahseager94 @KWestPhysio @naojane18_PT @physiotalk You see I think this is beyond service pressures etc & is actually just common decency & good manners! You'd handover when they move internally, why is going outside of the acute setting any different? We are becoming very insular in our thinking - silo'd as they say #physiotalk
