T2 Sick or not you need to pay attention to details, make sure are all your Drs are copied on what's relevant, check your prescriptions for errors, tell them you just had a blood test, travel with your medication list for starters. #PatientChat @dpepe88

T2 You have to have actionable strategies. Don't talk about it; be about it. Speak to your care team, doctors, advocates, etc. Coordinating care doesn't have to be complicated; it just has to work for the patient. #patientchat

A2: I would love to see a global patient portal. It is not only difficult for a patient to remember which portal they have to go to to communicate with which doctor, but then we have to share tests, reports etc.. This would be a great start! #patientchat

Search google play for caregiving apps. I use Caring Village - but still searching for perfection - right now it keeps everyone in the loop. #patientchat

T4: Successful care coordination is... Specialists reading each others notes before/during follow-ups. Including, labs, radiology, and recently started medication(s). Less chance of being stuck between strong opposing views and choosing. #patientchat
