T1: #patientchat coordination in care is a must as if you are looked after by more than one specialist you need to ensure that everyone is communicating especially if they are from different hospitals. Ensuring all correspondence is sent to all your Drs not just 1 is important

Coordinated care produces best patient outcomes. Specialists have different areas of focus & it's important 4 all to be updated/on same page so recommendations, medications, etc. don't conflict/negatively interact. All involved in your care need to know the goals. #PatientChat

T2 Sick or not you need to pay attention to details, make sure are all your Drs are copied on what's relevant, check your prescriptions for errors, tell them you just had a blood test, travel with your medication list for starters. #PatientChat @dpepe88

T2 You have to have actionable strategies. Don't talk about it; be about it. Speak to your care team, doctors, advocates, etc. Coordinating care doesn't have to be complicated; it just has to work for the patient. #patientchat

A2: I would love to see a global patient portal. It is not only difficult for a patient to remember which portal they have to go to to communicate with which doctor, but then we have to share tests, reports etc.. This would be a great start! #patientchat

Ultimately, responsibility rests w/the patient. Some offices have designated care coordinators & sometimes family can help, but you are your own best advocate, and, burdensome as it may be, you must communicate w/all involved to make certain your goals are met. #PatientChat

@carlyflumer @patientchat When my dad was navigating the system on his own we assumed he wanted his independence as always, but when my sisters went with him to an appointment his Dr said "I didn't know he had people." and it makes a difference to treatment. #PatientChat

T2: #patientchat this is hard but requires work from both the HCP and the patients everybody ensures everyone is up to date. This can be helped by good Gp/family doc, it’s in the patients interest to keep everyone upto date

T3: Ultimately in my opinion it is the responsibility of the patient to coordinate a care team for themselves. The care team should include all specialists and their PCP. I would like to see the PCP take an active role in leading that team. #patientchat

Search google play for caregiving apps. I use Caring Village - but still searching for perfection - right now it keeps everyone in the loop. #patientchat
