Care Continuum Alliance Annual Conference

Attendees of The CCA Forum 2013 educational sessions will gain clear insights into innovative wellness and care management programs, and concrete takeaways that will help them implement, evaluate, and improve their own programs. The four concurrent educational sessions tracks focus on implementation of population health models and strategies from development to delivery. Stakeholders along the continuum of care will take home the tools needed to improve outcomes, demonstrate value and increase quality of care and service. This year, we again will have a juried format in which expert panels will choose one presentation from each track to receive the CCA Outstanding Leadership in Population Health Management Award.
